The achievement of Minimum Service Standards (SPM) for Severe Mental Disorders (ODGJ) services at Tanjungwangi Public Health Center significantly dropped from 67.5% in 2024 to 9.7% in 2025. Objective: To analyze the underachievement of the mental health program's SPM target based on the 5M input and POACE process framework. Methods: A qualitative research with a descriptive-evaluative approach was conducted from June to July 2026. Informants were selected using purposive sampling, including the Head of the Community Health Center, Head of Administration, Mental Health Program Coordinator, Pharmacist, cadres, and ODGJ caregivers. Data gathered via in-depth interviews, observations, and document reviews were analyzed using the Miles and Huberman model. Results: The input analysis revealed the absence of the Community Mental Health Executive Team (TPKJM), lack of clinically trained mental health personnel, and poor drug logistics management. In terms of the process, key barriers included passive case finding, lack of medication adherence tracking cards, and low digital competency regarding the SIMKESWA application. The low administrative SPM figure reflects severe underreporting rather than a lack of actual clinical service delivery. Conclusion: The low achievement of the Mental Health Program's SPM is driven by structural management deficits and suboptimal digital reporting systems.
