Ankle joint dislocation is an injury that causes pain, deformity, and functional impairment, necessitating accurate radiographic examination to establish a diagnosis. Standard Anteroposterior and Lateral radiographic projections do not always optimally demonstrate hindfoot alignment; therefore, the addition of the Hindfoot Alignment View (Saltzman) projection is required. This study aimed to examine the radiographic technique for ankle joint dislocation cases involving the addition of this projection at Prof. Dr. R. Soeharso Orthopedic Hospital, Surakarta. A qualitative descriptive method with a case study approach was employed. The study was conducted at the Radiology Department of Prof. Dr. R. Soeharso Orthopedic Hospital, Surakarta, between November 2025 and April 2026. Data were collected through observation, interviews, and documentation involving one radiologist, one orthopedic surgeon, and three radiographers, followed by descriptive analysis. The results indicate that radiographic examination for ankle joint dislocation cases at the hospital utilizes the lateral projection supplemented by the Hindfoot Alignment View (Saltzman) projection. The examination is performed in a weight-bearing standing position with a 40° caudad beam angulation, allowing for clearer visualization of the alignment relationship between the tibia and the calcaneus. This additional projection provides optimal information regarding hindfoot alignment and assists physicians in evaluating malalignment to support the diagnosis. In summary, radiographic examination for ankle joint dislocation cases at Prof. Dr. R. Soeharso Orthopedic Hospital employs the lateral projection supplemented by the Hindfoot Alignment View (Saltzman) projection based on clinical indications. This additional projection offers optimal visualization of the alignment between the tibia and calcaneus, thereby facilitating the evaluation of malalignment and supporting both diagnosis and patient management.
