Recording inventory in a local government agency requires orderly, centralized, and traceable data entry. This study designs and builds a web-based information system for incoming and outgoing stock at the Personnel and Human Resource Development Agency (BKPSDM) of Medan City. Requirements were gathered through observation, interviews, and a literature review, while the system was developed using the Waterfall model, consisting of requirements analysis, system design, implementation, testing, and deployment. The application was built with the Laravel 10 framework (PHP), a MySQL database run through XAMPP, and Visual Studio Code as the code editor. It serves two user roles, administrator and employee, and provides modules for users, categories, items, incoming items, outgoing items, and stock. Black box testing of 24 scenarios, comprising 20 for the administrator role and 4 for the employee role, showed that every scenario produced the expected output. The system therefore works as designed and can be developed further to support inventory management in the agency.
