Abstract

Background: Burnout among healthcare workers is a persistent occupational health concern, while spirituality has been examined as a potential personal and organizational resource for coping with chronic work stress. However, the evidence remains heterogeneous because spirituality and burnout are defined and measured in different ways. Objective: This systematic review examined the direction and consistency of associations between spirituality-related domains and burnout among healthcare workers, with attention to burnout dimensions, potential mediating mechanisms, and contextual variation. Methods: PubMed, Scopus, and Cochrane were searched through April 2026 using terms related to spirituality, religiosity, spiritual well-being, spiritual care, and burnout. Eligible primary studies involved healthcare workers and reported spirituality-related measures together with burnout outcomes. Two reviewers independently performed study selection, data extraction, and quality assessment using the modified Newcastle–Ottawa Scale for the cross-sectional studies included in the final synthesis. Because of conceptual and methodological heterogeneity, findings were synthesized narratively. Results: Seventeen cross-sectional studies were included, with sample sizes ranging from 30 to 4,092 participants. Most studies reported inverse associations between spirituality-related measures and burnout, particularly emotional exhaustion, although several findings were mixed or non-significant. Variation in spirituality constructs, burnout instruments, occupational settings, and adjustment for confounders limited direct comparison and causal interpretation. Conclusion: Spirituality may function as a protective resource against burnout, but the available evidence does not establish causality. Longitudinal and intervention studies using clearly defined constructs and validated measures are needed, alongside organizational strategies addressing workload, support, and working conditions.
