Occupational Safety and Health (OSH) management is essential for maintaining industrial operations, particularly where incident reporting and maintenance follow-up require timely coordination. At PT Takaki Indoroti Prima, reporting practices created limitations in data organization, status monitoring, traceability, and response time. This study aimed to design and implement a web-based ticketing and monitoring information system to support integrated reporting, tracking, and prevention of OSH-related incidents. The system was developed using the Waterfall model through requirements analysis, system design, implementation, testing, deployment, and maintenance. The application was built with Laravel and supported ticket creation, status monitoring, user management, machine and spare-part management, work-history recording, report printing, and dashboard-based information presentation. Functional validation was conducted using Black Box Testing on login, ticket input, user input, machine input, spare-part input, and ticket-management functions. The recorded test matrix comprised 20 scenarios, and all scenarios produced outputs consistent with the expected results, corresponding to a 100% pass rate for the documented functional tests. The implemented system improved reporting structure, strengthened traceability, facilitated monitoring of corrective actions, and provided more consistent operational documentation. These results indicate that the system can serve as a practical digital foundation for integrated maintenance and OSH reporting at PT Takaki Indoroti Prima.
