Background: Immunization is an effective measure for preventing vaccine-preventable diseases and reducing morbidity and mortality among children. Maternal adherence to basic immunization schedules is a crucial factor in the success of immunization programs. This study aimed to identify factors associated with maternal adherence to basic immunization at the Budi Indah Maternity Clinic in Palembang City in 2025. Methods: A quantitative approach was employed. Analysis was conducted to determine the relationship between maternal characteristics and factors and adherence to basic immunization. Results: The majority of mothers (57.7%) were adherent to basic immunization, while 42.3% were non-adherent. Factors associated with adherence were access to health services and attitude. Access to health services was the most dominant factor (OR = 2.29). Age, education, occupation, knowledge, and family support did not show a significant relationship with basic immunization adherence. Conclusion: Maternal adherence to basic immunization is associated with access to health services and attitude, with access to health services being the most dominant factor. Improving ease of access to health services requires attention to support adherence to basic immunization. Keywords: access to health services; basic immunization; maternal adherence; attitude.
