This study aimed to analyze the determinants of user acceptance of Hospital Information Systems (HIS), identify the conceptual structure of the existing literature, and explore implementation challenges and future research opportunities based on the Unified Theory of Acceptance and Use of Technology (UTAUT) framework. A Systematic Literature Review (SLR) was conducted on 13 peer-reviewed articles retrieved from Scopus, PubMed, and ProQuest, supplemented by citation searching using Publish or Perish and Consensus. Article selection followed the PRISMA 2020 guidelines using the Population–Concept–Context (PCC) framework, while methodological quality was assessed using the Mixed Methods Appraisal Tool (MMAT) 2018. Bibliometric analysis was performed using VOSviewer through keyword co-occurrence mapping and Total Link Strength (TLS) analysis, followed by thematic synthesis based on the UTAUT framework. The findings identified performance expectancy, effort expectancy, social influence, and facilitating conditions as the primary determinants of HIS user acceptance. The bibliometric analysis revealed five thematic clusters representing technological acceptance, user perceptions, technology adoption, behavioural readiness, and electronic information exchange and interoperability. Overall, the findings indicate that successful HIS implementation depends not only on technological readiness but also on organizational support, adequate infrastructure, continuous user training, and user-centered implementation strategies to achieve sustainable digital transformation in hospitals.
