This study aims to develop a web-based information system for monitoring and evaluating social assistance programs in Kadi Wanno Village. The research addresses the need for more structured management of beneficiary data, assistance programs, distribution records, monitoring indicators, and evaluation reports. The study uses an applied research approach with observation, interviews, and document analysis as data-collection techniques. System requirements are analyzed using the PIECES framework, while system design uses Unified Modeling Language (UML) through use case, activity, and class modeling, complemented by a relational data model and interface design. The system is developed using the Waterfall model and CodeIgniter 4 framework based on the Model-View-Controller architecture. Functional verification is designed using Black-Box Testing with test cases derived from the identified functional requirements. The proposed system integrates beneficiary management, program management, verification, distribution recording, monitoring dashboards, evaluation records, and reporting into one web-based platform. The contribution of this study is a village-level monitoring and evaluation information system that emphasizes traceable data management and periodic reporting rather than merely recording recipients. The empirical testing results, including the number of test cases and successful execution rate, should be inserted from the actual field testing before journal submission.
