<![CDATA[Continuity of midwifery care provides an opportunity to monitor maternal and infant conditions throughout successive stages, from pregnancy to contraceptive selection. This case study involved Mrs. A, a 27-year-old G1P0A0H0 woman who experienced back pain during the third trimester without accompanying warning signs. Care was provided at PMB Puji, Batam City, using the SOAP management approach and referring to midwifery service standards. The care period extended from February 4 to April 20, 2026, covering pregnancy, childbirth, newborn care, postpartum care, and family planning. During pregnancy, the mother underwent regular assessments and no complications were identified. Labor progressed spontaneously without obstruction, resulting in the birth of a healthy male infant with an APGAR score of 9/10. Postpartum monitoring indicated satisfactory recovery, although the mother initially reported limited breast milk production. Breastfeeding education and oxytocin massage were provided as part of the care. Across three newborn assessments, the infant remained in good condition, with body weight increasing from 3,600 grams to 4,400 grams. For family planning, the mother selected a three-month injectable contraceptive after receiving counseling and reported no complaints during the subsequent evaluation. The sequence of care demonstrates the implementation of continuous midwifery services according to the maternal and infant conditions and needs at each stage. Consistent follow-up is necessary to identify changes in maternal and infant conditions at an early stage and to determine appropriate management.]]>
