Electronic Medical Records (EMR) are part of the digital transformation of healthcare services aimed at improving effectiveness, efficiency, and overall service quality. This study seeks to analyze the factors hindering EMR management in outpatient registration using the 5M approach (Man, Money, Material, Machine, and Method). Employing a qualitative descriptive design, data were collected through interviews, observations, and documentation to capture a comprehensive picture of the challenges faced. The findings revealed obstacles related to limited human resources, inadequate facilities and infrastructure, inconsistent SOP implementation, incomplete data entry, and insufficient funding. These barriers collectively reduce the effectiveness of EMR management and negatively impact healthcare service quality. The study emphasizes the importance of strengthening human resource competencies, upgrading facilities, ensuring consistent SOP application, improving data completeness, and securing sustainable financial support. Addressing these aspects will enhance EMR utilization, support better patient care, and contribute to the long-term success of digital healthcare transformation.
