Abstract

Diabetes during pregnancy can be divided into pregestational diabetes and gestational diabetes. The risk of fetal Diabetes Mellitus pregestational (DMpG) patients can be either macrosomia or low birth weight. The purpose of this study was to investigate the differences in the administration effect of insulin and Zingiber officinale extract on birth weight of Rattus norvegicus pregestational diabetes mellitus model. This research is an experimental research using post test only control group design. Samples contained 30 pregnant Rattus norvegicus and divided into 5 groups, where K0 (negative control group) and K1 (positive control group) given aquadest 1cc, K2 given insulin 1IU, K3 given ginger extract 500mg/ kg BW, K4 given insulin 1IU and ginger extract 500mg/ kg BW. Treatment was administered during the first 16 days of pregnancy. On the 17th day, the rats were terminated and then the birth weight was measured with the scale. Data analysis using One-way ANOVA test followed by Tamhane test. The results showed that there was a difference of birth weight in the insulin treatment group and ginger extract treatment group (p-value = 0,037 < α =0,05). The conclusion of this study was Zingiber officinale can be used as a single therapy or combination with insulin of Rattus norvegicus pregestational diabetes mellitus model.

Abstract

Stroke is the leading cause of death in Indonesia with the significant increase in prevalence each year. The dominant stroke is ischemic stroke which is often caused by abnormal blood lipid profile which is the increase of total cholesterol to High-Density Lipoprotein (HDL) ratio that is an increase of total cholesterol and decrease of HDL. The risk of ischemic stroke will increase if there is an increase of total cholesterol to HDL ratio because it can stimulate the formation of atherosclerosis. The increase of total cholesterol and the low of HDL level after stroke attack will inhibit the recovery, both of them can influence outcomes and increase mortality in ischemic stroke. This study aim is to determine the correlation between total cholesterol to HDL ratio with outcomes prediction of acute ischemic stroke.This study is an analytic study using cross-sectional study design. Sampling studied are all of the acute ischemic stroke patients at General Hospital of Cut Meutia North Aceh District from May to August 2017 used total sampling that met inclusion and exclusion criteria. Analysis of the correlation between total cholesterol to HDL ratio with outcomes prediction of acute ischemic stroke uses Spearman correlation test with a significant degree (α) of 0,01. The result of this study showed that the mean of total cholesterol to HDL ratio was 3,7 (moderate risk) and outcomes prediction of acute ischemic stroke was good. There is a correlation between total cholesterol to HDL ratio with outcomes prediction of acute ischemic stroke (r=- 0,623;p=0,000). Lower total cholesterol to HDL ratio was correlated with better outcomes of acute ischemic stroke. Total cholesterol to HDL ratio can be used as a predictor of ischemic stroke outcomes.

Abstract

Diabetes mellitus (DM) is a metabolic disease with characteristics of hyperglycemia that has an effect on increasing morbidity and mortality in the mother. One of the complications of pregestational DM (pGDM) is preeclampsia (PE). The perinatal mortality rate increases 20x in pGDM women with PE complication. Exposure to hyperglycemia causes an increase in oxidative stress that increases apoptosis of trophoblast cells which in turn causes the decrease of proliferation and the number of trophoblastic cells and leads to endothelial dysfunction and hypoxia. The purpose of this study was to distinguish the effect of insulin administration and ginger extract on the number of trophoblast cells of R. norvegicus. This research was an experimental research using post-test only control group design. The population used were pregnant R. norvegicus. The sampling technique was simple random sampling with independent variables are insulin and Z. officinale extract and the dependent variable is the number of trophoblast cells. Data analysis of the number of trophoblast cells used wereKruskal-Wallis and Mann-Whitney test. The result showed that there was a significant difference in the trophoblast cell number with p-value <0,05 (p = 0,001) on positive control with insulin treatment group. The conclusion of this study was the presence of ginger extract can be used as a single therapy or combination with insulin for pregestational diabetes.

Abstract

HISTORY: The girl 15 years old, had main complaint, there was a mass in her pedis since she got 2 years old, look red, no pain although in walk, size equal with the skin. The mass got bigger and pain in walk. The mass get bleedy after had crushed by stone, after that bleedy always happen when there was a contact, although soft touch. Patient had history contact with tuberculosis patient.INTERESTING POINTS FOR DISCUSSION: Tuberculoma is a mass that happened because infected bacil mycobacterium tuberculosis, hemangioma is a soft tissue mass that formed by vascular. hemangioma commonly happened in children about 4-10% in children’s tumor. It happened in a girl 3-5 more times than in a boy. Both of them are rare happen in the same time, especially in pedis. From this case, we  can found both type in the same patient.SOLUTION AND RATIONALE: For this case of soft tissue tumor,we use open biopsy. After we found the result , the best treatment for hemangioma and tuberculoma of the pedis for this case is below knee amputation.FINAL OUTCOME: For this case, the best treatment is amputation, and after amputation, the patient never complain about pain anymore, she feel satisfied and overall her quality of life is improving.

Abstract

Pesantren as dormitory-based educational institutions familiarize their students with sleep patterns and certain activities, including sports that increase reaction time. This study aims to prove the relationship between sleep patterns and reaction time with the ability of concentration in adolescent girls. This cross-sectional study was conducted on 27 Wachid Hasyim boarding school students whose average age was 17.49 years (IK 95% 17.33-17,66). Subjects filled out the Pittsburgh Sleep Quality Index (PSQI) and Krawietz Concentration Scale (KCS) questionnaires and underwent the Ruler Drop Test. All three consecutively measured sleep patterns, concentration ability and reaction time. The KCS questionnaire assessed four components: 1) Control of Focus (CF); 2) Uncontrolled Focus (UF); 3) Reading and Listening Focus (RLF); And 4) Control of Focus Before Sleep (CFBS). The results show that the subject has a PSQI score of 6.41 (95% IK 5.29-7.52) that exceeds the normal limit. However, subjects had a rapid reaction time (55.6%) and a KCS score of 216.78 (95% IO 958.06-225.49; p = 0.044) higher than their mean value. There was no relationship between sleep patterns with concentration ability (p = 0.487). There is a negative correlation trend between sleep patterns and CFBS (r = -322; p = 0.050) or with reaction time (r = -394; p = 0.069). There was a relationship between reaction time and overall concentration (p = 0.048) and partially on RLF + CFBS (p = 0.029). There is a positive correlation between the reaction time in the form of the catch distance response with CFBS (r = 0.326; p = 0.049). The effort of pesantren in maximizing the use of time that caused poor sleep pattern was found did not decrease the ability of student concentration. Exercise that increases reaction time is found to reduce their ability to start sleeping

Abstract

Cancer has the ability to develop resistance to chemotherapy and increased prevalence of this drug-resistant cancer requires further research and development of treatment. There are several mechanisms that can stimulate chemotherapy resistance, such as drug inactivation, drug effectiveness mechanism, HER2 role, PI3K pathway, DNA damage repair, and apoptotic inhibition. The PI3K pathway is an important signal transduction pathway in the growth and development of cancer. This pathway plays a role in several functions, such as proliferation, cell growth, and tumor survival. PI3K activation has been identified in some cases of cancer, including breast cancer. This pathway undergoes excessive activation resulting in resistance to chemotherapy and increased metastasis.

Abstract

Diabetes mellitus is a chronic disease that occurs in the community, with 1.9% prevalence of in the world is and the seventh leading cause of death in the world. It is estimated to increase up to 21.3 million by 2030 if the pattern of Diabetes management is not well developed. In 2007, the prevalence of Pregestational Diabetes Mellitus in Indonesia was 12% and had an impact on placental development that was important during pregnancy for fetal survival with placental weight as the indicator. Diabetes Mellitus pregestational therapy is insulin, however, it often causes complications. Oral hyperglycemic drugs with ginger dose extract of 500mg/kgBW significantly decreased serum glucose levels in diabetic-induced rats. Ginger extract is expected to decrease blood glucose levels so as to increase the weight of placenta. The purpose of this study was to determine the effect of insulin and ginger extract on the weight of placenta Rattus norvegicus model of diabetes mellitus. This research is an experimental research using post test only control group design. The population used in this study was pregnant Rattus norvegicus with simple random sampling. The independent variables are insulin and ginger extract and the dependent variable is the weight of placenta. Data analysis for placental weight using Kruskal Wallis was continued by Post Hoc Bonferroni test. Test results for placental weight p>α (0.63 > 0.05) mean that there is no difference in placental weight between negative control, positive control, and treatment group. The conclusion of this study is there is no significant difference between groups, however, based on mean data the weight of placenta Rattus norvegicus model of diabetes mellitus with insulin and ginger extract therapy are higher than the control group

Abstract

ABSTRACTThe occupancy rate of obsgyn ward ‘Aisyiyah Bojonegoro Hospital is still very low (33,5%) compared to occupancy target (50.0%) in last five years. The purpose of this study was to identify the expectations of consumers to 4Ps (Product, Price, Place, Promotion) obsgyn inpatient services in mind, heart, spirit and the mission, vision, and values ‘Aisyiyah Bojonegoro hospital. Further arranged on marketing efforts with Value-Based Matrix models on marketing 3.0 for an alternative answer to the problems of dwelling of obsgyn ward ‘Aisyiyah Bojonegoro Hospital. This is a descriptive with cross-sectional approach study. The data was taken in January-August 2016. The data used are primary and secondary data. Primary data was taken with a questionnaire guidance to 152 patients respondent from obsgyn clinic specialist and 22 patients respondent from Maternal and Child Health (MCH) outpatient 'Aisyiyah Bojonegoro Hospital. The sampling method was accidental sampling. Secondary data such as mission, vision, and values are taken from the profile hospital hospital. Analysis of the respondents expectations and the hospitals mission, vision, and values are taken to be FGD material with hospital management to explore alternative marketing efforts that will be done. The researchers analysis from FGD results lead to the conclusion in the form of 19 marketing efforts recommendations based on value-based matrix in marketing 3.0 by identifying consumer expectations and the mission, vision, values hospital. These marketing efforts recommendations is expected to increase the occupancy rate of obsgyn ward ‘Aisyiyah Bojonegoro Hospital.Keywords:Marketing Efforts, Value Based Matrix, Marketing 3.0.

Abstract

Diabetes mellitus was metabolic disease which had chronic hyperglycemic characteristic  .The risk of fetal death and malformation which were higher in pregestational diabetes. The complications could be suppressed through insulin therapy. The reasons about insulin side effect made herbal therapy began to develop, such as ginger (Zingiber officinale). This research aimed to prove the difference effect of Zingiber officinale extract and insulin on the number of fetal dead or resorbsion, and malformations in Rattus novergicus model of pregestational diabetes mellitus. Thirty rats were divided into 1 negative control group and 4 groups which were modeled with type 1 DM with streptozotocin (STZ) i.p dose 50mg/kgBB in citrate buffer 0,05M pH 4,5. Rat with blood glucose levels >200mg/dL after 2 days of induction, were sampled and bundled. The treatment groups were given 1IU insulin, ginger extract 500mg/kgBB, and 1IU insulin plus ginger extract 500mg / kgBB. The 17th day of pregnancy fetal sample was observed macroscopically. Analysis data with One Way ANOVA and Post Hoc with LSD. There were significant differences in fetal malformation with p value was 0,012 and death or resorbsion were 0,001. Ginger extract could be used as a single therapy or combination therapy to reduced the number of fetal death or malformation.

Abstract

CD4 T-lymphocyte cell counts and pneumonia in HIV-infected childrenAbstractBackground Pneumonia is a major cause of morbidity and the most common cause of mortality in infants and children who are infected with Human Immunodeficiency Virus (HIV). For HIV-infected children, the risk for developing pneumonia has been closely related to an individual’s having a lower CD4 T-lymphocyte cell count.Objective To evaluate for an association between CD4 T-lymphocyte cell count and pneumonia in HIV-infected childrenMethods. A cross sectional study was conducted among all children diagnosed with and without pneumonia who had HIV at Adam Malik Hospital, Medan. Data were taken from patients’ medical records between January 2008 to December 2015. We studied demographic, clinical, radiological and CD4 T-lymphocyte cell counts on admission, as well as mortality outcomes.Results  A total of 174 HIV-infected children were included in this study. Pneumonia was found in 47 children with 39 months as the median age of onset. The mean difference in CD4 levels was 0.912 cells/mm3 units (95% CI to -5.79 to 7.62) with higher mean in the pneumonia group than in the non-pneumonia group, but no significant difference. In addition, no significant association were found between nutritional status and incidence of pneumonia [OR 1,050; 95% CI 0,537 to 2,053); P = 0.886) or between immunodeficiency status and pneumonia, [OR 0.986;(95% CI 0,25 to 3.885);P= 0,984]. The common opportunistic infection in our subjects besides pneumonia were diarrhea, oral candidiasis and pulmonary TB, or some combination there of.  Severe malnutrition was also present in some subjects.Conclusions There is no significant association between CD4 T-lymphocyte cell count and pneumonia in HIV-infected children Key words CD4, HIV, pneumonia, children
