Abstract

Atrial septal defect (ASD) is a non-cyanotic congenital heart disease characterized by the presence of defects on atrium septal wall. ASD is 10% of congenital heart disease. This defect is the third most common congenital heart disorder with an estimated incidence of 56 out of 100,000 live births. With the improved echocardiographic technique that can diagnose a defect in asymptomatic abnormalities, the estimated incidence is increased to 100 out of 100,000 live births. The disorder is more common in women than men with a ratio of male: female 1: 2.

Abstract

Background: Atherosclerosis is the underlying process of coronary heart disease. Atherosclerosis is preceded by endothelial dysfunction caused by systemic mechanical and chemical stressors that may occur throughout the blood vessels. Recent studies have found the link incidence of atherosclerosis in the coronary arteries and other arteries as well. Erectile dysfunction (ED) is a clinical manifestation might be caused by atherosclerosis in iliac or pudendal artery. Previous studies have established the relationship between coronary artery involvement and the incidence of ED, but the odds of risk has not been well established. Methods: This was an age matched-paired case-control study. Erectile dysfunction in CHD patients who had undergone coronary angiography was checked by IIEF - 5 questionnaire. The severity of coronary artery lesion was assessed with a Syntax score from coronary angiography results. Moreover these results were assessed by a single experienced observer, blind method and were shown consistency test. Then, the risk of coronary artery lesion severity of the ED was analyzed by chi square test using SPSS version 20. Result: There were 86 subjects consist of 57 subjects in the case group and 29 subjects in thecontrol group. Stable CHD patients with high Syntax scores had 2.75 times risk for development of ED compare with low Syntax scores patients (OR : 2.75, 95 % CI : 1.08 to 6.95, p = 0.03). The severity of coronary artery lesions assessed with Syntax scores were not statistically signifi cant as an independent factor as the incidence of ED. Conclusion: Stable CHD patients with higher severity of lesions in coronary artery have a higher risk of erectile dysfunction than patients with lower severity of the lesion but was not statistically significant as an independent factor on the incidence of ED.Keywords: Severity of coronary artery lesions, erectile dysfunction, stable coronary heart disease

Abstract

The prevalence of Chronic Venous Insufficiency (CVI) give a high social impact. InEurope, approximately 40% of the adult population affected by venous disorders. In general, the prevalence of CVI is about 5% - 15%. The prevalence is increasing by age.

Abstract

Background: Cardiovascular disease is one of major problems in developed and developing countries. Atherosclerosis process begins with endothelial dysfunction. Lipoprotein is important factor in atherogenesis. Previous study stated that about 50% of cardiovascular events happened in individuals with normal or low LDL, therefore LDL plasma level alone is not enough to identifyindividuals with major adverse cardiac events. Individuals with small dense LDL predominant have 3 times fold to have cardiovascular risk. The goal of this study is to know whether the level of sdLDL has impact on in hospital major adverse cardiac events (MACE) of acute coronary syndrome patients. Methods: This was a cross sectional study, enrolling patients with acute coronary syndrome admitted and hospitalized in ICCU of Dr.Sardjito Hospital since September of 2013 until June 2015. The small dense LDL (sdLDLD) level was measured with previous formula using routine blood lipid component. Major adverse cardiac events (MACE) were determined upon observation during horpitalisation and defi ned as death, reinfarction, cardiogenic shock, acute heart failure, ventricular tachycardia or ventricular fi brillation, prolonged angina pain, and the need for immediate coronary intervention. Results: There were 159 patients with mean age 60.80 ± 9.8 years involved in this study. Onehundred eighteen (118) or 73% of patients were male. The mean of sdLDL level in patients with MACE was 108.34 ± 37.94 g/dl and mean sdLDL level in patients without MACE was 105.54 ± 43.10 g/dl. The level of sdLDL in patients without MACE was lower than patients with MACE (p=0.705). In this study we found the cut- off sdLDL level is ≥ 108.085 for higher sdLDL level and < 108.085 for lower sdLDL level. The higher sdLDL level have the prevalence ratio of 1.25 to develop MACE, however the value was not statistically signifi cant.Conclusion: The sdLDL level did not correlate with MACE in hospitalised patients with acute coronary syndrome.Keywords: small dense LDL, acute coronary syndrome, MACE

Coronary circulation is the circulation of blood in the blood vessels of the heart muscle (myocardium). The vessels that deliver oxygenrich blood to the myocardium are known as coronary arteries. The vessels that remove thedeoxygenated blood from the heart muscle are known as cardiac veins.

Abstract

Background: Right ventricular (RV) volume overload that occurs in patients with atrial septal defect (ASD) could lead to the development of pulmonary arterial hypertension (PAH). RV function has been found depressed in some patients with PAH. Myocardial performance index (MPI) is one of methods to assess RV function. The aim of this study was to assess correlation between RVfunction using RV-MPI with PAH severity in patients with ASD. Methods: We conducted a cross sectional study between July 2012-July 2013. We enrolled 67patients with secundum type ASD. RV-MPI, defi ned as the sum of isovolumic relaxation time and isovolumic contraction time derived by ejection time, was measured from tricuspid infl ow and RV outflow. The normal range is 0.28-0.32. Pulmonary artery systolic pressure (PASP) was measured from echocardiography. Results: There were 67 patients (54 women) with mean age was 38.01±14.29 years (11.9% with no PAH, 11.9% mild PAH, 26.9% moderate PAH and 49.3% severe PAH). The mean PASP was 67.16±32.33 mmHg and mean MPI was 0.44±0.25. We found signifi cant correlation between PASP and MPI (r=0.73; p=0.000). In addition, we classifi ed the PAH severity based on PASP and there was signifi cant differences of MPI between groups (no PAH, MPI 0.25±0.07; mild PAH 0.24±0.05; moderate PAH 0.24±0.12; severe PAH 0.63±0.21; p=0.000). Furthermore, in severe PAH group, the correlation between PASP and MPI remain signifi cant (r=0.42; p<0.05). Conclusion: These result demonstrate a correlation between PAH severity and MPI in patientswith ASD. Specifi cally, RV function found decreased in ASD patients with severe PAH.Keyword: myocardial performance index (Tei index), pulmonary arterial hypertension, right ventricular function, atrial septal defect
