Abstract

The appropriate use of antihypertensive have important role for the successful of hypertension therapy. The study aims were to evaluate the use of antihypertensive medicines and cost of hypertension therapy in General, Askes and Jamkesmas inpatient at IRNA-B ward in X hospital from July to December 2010 period. Design of the study were descriptive analytical and retrospectively. Inpatients with primary hypertension and concomitant disease (hypertensive heart disease with and without congestive heart failure and hypertensive renal disease with renal failure) at IRNA-B ward in X Hospital were included to the study. The results showed that the appropriateness of the selection of antihypertensive was 87.0%. Non-formulary antihypertensive medicines were 39.3%. The lowest of medicines cost of hypertension therapy was shown in Askes inpatient. There were differences in the cost of hypertension therapy among Askes with General inpatient and Askes with Jamkesmas inpatient. There was no diference drug cost therapy between General and Jamkesmas inpatient.

Abstract

The aims of the research are to determine the Significant effect factor among mixmg rate, mixing time and their interaction on the physical properties and stability of green tea emulgel and to determine the optimum composition area which can produce good emulgel. In our previous investigation was determined the optimum composition between emulsifying agent (Tween 80 and Span 80) and gelling agent (Carbopol) This research determined the optimum mixing process, particularly on mixing rate and mixing time usig the factorial design with two factors (mixing rate and mixing time) and two levels (high level and low level). The physical properties and stability parameter investigated were including viscosity, spreading ability and viscosity shift atter storage at predetermined temperature and humidity for a month. The results showed that mixing rate Significantly influence on the viscosity and spreading ability and the mixing time significantly influence on the Viscosity shift. The optimum composttion area was not produced because the factorial design equation for viscosity respond and viscosity shin were not significant so can not be used to predict responds.

Abstract

lsoflavones in soybean and unfermented soybean product are in glycoside form which could not be absorbed by the body and should be hydrolyzed before it is metabolized. Hydrolysis by bacteria producing β-glucosidase occur throughout the gastrointestinal tract. Probiotic microorganisms, like Lactobacillus and bfiidobacterium, have an endogenous enzyme , β-glucosidase that plays an important role in changing the profile of isoflavones during fermentation. The study was to investigate TLC profile and genestein content in fermented soybeanmilk and to formulate the freeze-dried fermented soybeanmilk into effervescent granules preparation. Soybeanmilk was fermented with Lactobacillus sp., and then freeze-dried. The soybeanmilk, before and after fermentation, were then extracted and analyzed by TLC-scanner. The freeze-dried fermented soymilk was prepared into four formulations of effervescent granules with variations in citric acid-tartaric acid ratio. Granules were prepared by the wet method and theirs physical characteristics were evaluated. Extracted granules were analyzed by TLC-scanner. Genistein content in the fermented soymilk has increased 145,32% as compared to the unfermented soymilk. All four effervescent granules have good physical characteristics except that the moisture content were above 0.7%. Formula containing citric acidtartaric acid ratio of (1 : 1) produced effervescent granules with the best physical characteristics.

Abstract

Talok (Muntingia calabura L.) leaf is a traditional medicinal plant which empirically used to treat diabetes mellitus. This research aimed to know hypoglycemic effect of combination dose of talak leaves infuse-metformin and glibenclamide on Na2EDTA induced-diabetic rats. Diabetic animal model were conducted by induction of Na2EDTA dose of 70 mg kg bw for 3 days. The test groups divided into six groups which were treated with: 50 mg kg bw CMC-Na suspension (negative control 1st group), 0.9 mg kg bw glibenclamide (2nd group), metformin 90 mg kg bb (3rd group), 900 mg kg bw talok leaves infuse (4th group), combination of talok leaves infuse-glibenclamide (450 mg : 0.45 mg/kg bw) (5th group), combination of talok leaves infuse-metformin (450 mg : 45 mg kg bw) (6th group). The levels of blood glucose were examined in all 6 experimental groups on day 0, 4th and 7th. The data obtained were analyzed by one way ANOVA (p < 0.05). The results showed that all of combination could decrease the blood glucose levels significantly. The combination treatment of talok leaves infuse-glibenclamide showed a higher hypoglycemic effect than either drug alone, while combination treatment of talok leaves infuse-metformin showed a similar hypoglycemic effect to either drug alone.

Abstract

Sambiloto (Andrographis paniculata Nees) is a medicinal plant containing andrographolide as its primary bioactive component, which has been shown to inhibit breast cancer cell proliferation. Andrographolide is a diterpene lacton and sparingly soluble in water. The aim of this study is to prepare microspheres of A . paniculata ethylacetate fraction by microencapsulation using spray drying method. The ethyl acetate fraction of A. paniculata herbs was microencapsulated using PVP K30 and HPMC as the coating polymer in different ratios of 1:5; 1:7,5 and 1:10. Microspheres containing the ethyl acetate fraction of A. paniculata herbs were characterized in term of recovery factor, morphology, particle size distribution, water content and entrapment efficiency. The results showed that the A. paniculata microspheres have the recovery factor of 19,50% to 31,64%, spherical shape varies from smooth surface to irregular shape, particles size of 1,44% to 19,23 µm, moisture content of 5,15% to 13,44%, and entrapment echiency between 94,83 to 112,15%.

Abstract

Drug prescribing and drug use become more complex, as the number and potency of available drugs increase, leading to a variety of Drug Related Problems (DRPs). From 15 studies conducted in several countries, 7.1% of the overall hospital admissions were related to DRPs and 59% of them might be prevented. For the last few decades, attention to DRPs in some countries like USA, Australia and England have increased. A study on pediatric inpatients was carried out in 2007 by monitoring medical records detected nearly 60% DRPs with an average of 3 DRPs per patient and other study done showed that more than 78% of geriatric patients had in average 3 DRPs. A retrospective study to identify morbidities related to DRPs has been conducted in 2010 in 5 hospitals having clinical pharmacy unit. Data were collected from medical records or clinical pharmacy documents and interviews with the head of pharmacy unit and clinical pharmacist. DRPs data were then analyzed and categorized according to a combination of American Society of Hospital Pharmacists (ASHP) and Pharmaceutical Care Network of Europe (PCNE) classifications. Results of the study revealed that there were 266 DRPs identified with an average of two DRPs per patient and anti-infection drugs were mostly involved. Duration of a treatment longer than necessary was found in 17.3% cases and had resulted in high cost drug treatment (32.7%), whilst poly-pharmacy, inappropriate drug form and new indication for drug treatment presented were hardly found. Pharmacist was the most likely who made intervention in DRPs (61.3%) and 48,9% intervention was proposed, but was rejected by prescriber.

Abstract

Gambier (uncaria gambier) is known to have antioxidant properties and some studies attributed to the presence of polyphenols. The objective of this study was to investigate the potential of various gambier available in Indonesian market as free radical scavenger and to evaluate its antihyperglycemic potency as α-glucosidase inhibator. Isolation of catechin was carried out by extraction with distilled ethyl acetate as solvent. Analysis of catechin in the gambier dried extracts were carried out by TLC method. The molecular weight and catechin content of gambier dried extracts were determined by analyzing mass spectra and spectrophotometry, respectively. The free radical scavenging aactivity ofextracts were measured by using 1, 1-diphenyl1-2-picrylhydrazyl (DPPH) as stable free radical compound. The antihyperglycemic potency of gambier dried extracts were asseyed, as α-glucosidase inhibator. The results showed that various gambier available in the market were very active as antioxidant, indicated by the IC50 vvalues of gambier dried extracts were 4.6 to 18.2 µg/mL. Whilst th IC50 for glucosidase inhibition varies from 40.45 to 52.43 µg/mL, and classified as a moderate antidiabetic.
