Abstract

Reproductive health issues remain prevalent among adolescents due to limited access to information. Fluor albus is a common concern among women. Global surveys indicate that approximately 75% of women experience at least one episode of abnormal vaginal discharge during their lifetime. Promotive and preventive interventions, including targeted health education, are crucial to improve adolescents' knowledge and attitudes regarding reproductive health and hygiene. This study investigated the effectiveness of the "Make a Match" educational method in enhancing knowledge and attitudes about leukorrhea prevention among adolescent girls. This quasi-experimental pre-posttest control group study examined the impact of the "Make a Match" method. The study population comprised female adolescents in Sitinjak Sub-district, South Tapanuli Regency. A total sample of 30 participants was recruited using total sampling. Data were collected using questionnaires, posters, and leaflets. Data analysis involved paired t-tests and Wilcoxon signed-rank tests. The results of the Wilcoxon tests revealed statistically significant differences in knowledge and attitude scores before and after the intervention (p < 0.001 for both variables). These findings demonstrate a significant improvement in both knowledge and attitudes related to leukorrhea prevention following the "Make a Match" health education intervention (p < 0.05). In conclusion, this study provides evidence that the "Make a Match" health education method effectively enhances knowledge and attitudes regarding leukorrhea prevention among adolescent girls. These findings underscore the need for enhanced community-based health education programs, particularly for adolescents, focusing on reproductive health.

Abstract

Patient non-adherence to antihypertensive medication is a significant risk factor for increased morbidity and mortality in individuals with hypertension. Hypertension, a prevalent cardiovascular condition, is classified as a degenerative disease. Consistent antihypertensive treatment is crucial for improving patient quality of life. This study employed a cross-sectional survey design. The sample consisted of hypertensive patients who presented at the Belawan Health Center between July 22 and 26, 2024. Consecutive sampling was used to recruit 75 participants. Data were collected via interviews regarding participant characteristics and the administration of the Morisky Medication Adherence Scale-8 (MMAS-8), Hypertension Fact Questionnaire (HFQ), and World Health Organization Quality of Life-BREF (WHOQOL-BREF) questionnaires to assess medication adherence, hypertension knowledge, and quality of life, respectively. Analysis revealed strong positive correlations between medication adherence and quality of life (ρ = 0.677, p < .001), and between hypertension knowledge and quality of life (ρ = 0.700, p < .001). Furthermore, both medication adherence and hypertension knowledge significantly predicted quality of life (F = 53.704, p < .001). Multiple linear regression analysis (Y = 30.488) indicated that a one-unit increase in adherence was associated with a 2.680-unit increase in quality of life (p < .001), and a one-unit increase in knowledge was associated with a 2.704-unit increase in quality of life (p < .001). In conclusion, there are strong positive correlations between both medication adherence and hypertension knowledge with the quality of life of hypertensive patients at the Belawan Health Center.

Abstract

Hypertension is a worldwide public health disease today. Low knowledge about diet can risk an increase in blood pressure in hypertensive patients. This study aims to determine the effect of nutrition education with booklet media on knowledge about diet in patients with hypertension. The research design used was a quasy experiment with a non-randomized one-group pretest-posttest design. This research was conducted at the Lubuk Pakam Health Center. The research time starts from November-December 2023. The subjects of this study were all hypertensive patients in the Lubuk Pakam Health Center work area, as many as 202 people. The sample size was obtained from as many as 66 people using a purposive sampling technique. The inclusion criteria for research samples are hypertensive patients aged 40-60 years and subjects willing to become respondents. Data was collected through questionnaires and a Food Frequency questionnaire (FFQ). Data were analyzed using the T-paired test. The results showed that the mean value of knowledge of hypertensive patients about diet before being given nutrition education with booklet media was 46.9, with an SD value of 17.05. Still, after being given nutrition education with booklet media, it was 69.7, with an SD value of 12.5. Providing education through booklet media is significant to the knowledge score of hypertensive patients about diet. The study concludes that booklet media can contribute to increasing the knowledge score of hypertensive patients when choosing the ingredients and types of food they consume.

Abstract

Diabetes mellitus (DM) is a significant global health issue, with its prevalence continuing to rise in Indonesia. Self-care is a key component in managing DM, yet many patients face various obstacles in its implementation. This study aims to deeply explore the challenges faced by individuals with type 2 diabetes in managing their condition independently. Using a descriptive qualitative approach, the study analyzes the experiences of seven diabetes patients receiving care at Royal Prima Hospital. The social ecological model serves as the analytical framework to identify factors influencing self-care behavior, including individual internal factors, interpersonal relationships, healthcare systems, and the social environment. Data was collected through in-depth interviews and analyzed using thematic content analysis. The findings reveal several barriers to effective self-care in diabetes management. Firstly, at the individual level, a lack of knowledge, limited resources, non-adherence to medication, and financial constraints were major obstacles. Secondly, insufficient social support and misinformation about alternative treatments also influenced patient behavior. Thirdly, at the healthcare service level, long waiting times and inadequate social support were identified as challenges. Lastly, limited accessibility due to transportation issues and community resource constraints further hindered self-care. Patients require comprehensive education, easy access to medical tools, and psychological support to manage their condition effectively. Families and communities must be equipped with accurate knowledge about diabetes and trained to support patients. Healthcare providers should improve service quality by adopting a holistic approach and collaborating with communities to ensure optimal care. With collective efforts, it is hoped that the quality of life for diabetes patients can improve, and complications resulting from the disease can be prevented.

Abstract

Pregnancy exercise offers numerous benefits for both mother and child. However, many pregnant women do not participate in this activity. This study aims to identify factors influencing pregnant women's participation in pregnancy exercise at Darussalam Health Center, Medan City. A cross-sectional study was conducted on 60 pregnant women visiting the Darussalam Health Center. Data were collected using a structured questionnaire and analyzed using Chi-square tests and logistic regression. The findings indicate that most pregnant women did not participate in pregnancy exercise. Knowledge of the benefits of prenatal exercise, positive attitudes toward exercise, and partner support were significant predictors of participation. Multivariate analysis confirmed that these three factors were independently associated with exercise participation. This study highlights the importance of knowledge, attitudes, and partner support in promoting prenatal exercise among pregnant women. Health education programs focusing on these factors could encourage more women to engage in regular physical activity during pregnancy.

Abstract

Families play a crucial role in caring for stroke patients, but they often face various challenges. This study aims to explore the experiences of family members as caregivers for stroke patients. Using a phenomenological approach, this research examines the participants' direct experiences to uncover the phenomenon's essence. Data was collected through in-depth interviews over three months, from May to July 2024. In-depth interviews were used as the main instrument for gathering qualitative data from nine informants. The interview process was conducted in-depth and systematically, with triangulation techniques to enhance data validity. Data analysis was performed using a thematic content analysis approach. Data was continuously analyzed during the collection process, focusing on recurring keywords and phrases. The results were then grouped and organized into relevant subthemes. The findings indicate that caregivers often experience sleep deprivation, physical exhaustion, and social isolation. Additionally, they face financial challenges due to loss of income and increased healthcare costs. Despite these challenges, caregivers derive satisfaction from their role. This study highlights the need for a comprehensive support system, including respite care, financial assistance, and educational resources, to improve caregiver well-being and enhance the quality of care for stroke patients.

Abstract

Aneurysmal bone cyst (ABC) is benign lesion of long bone metaphysis, but it can also become locally aggressive lesion. ABC affects mostly medullary region of long bone metaphysis that predominantly affects children and young adults. Meta-diaphysis of the long bones and posterior elements of the vertebrae are the most common sites of involvement. ABCs can involve every skeletal site, but the most common locations are the metaphysis of the long bones, followed by the spine. However, diaphyseal lesions may occur. Patients with ABCs typically present with insidious onset of pain, swelling or a palpable mass. The imaging appearance of ABC is variable; however, a lytic and expansile lesion with fluid-fluid levels is the most common presentation. Although incidence of intracortical variety of ABCs is rare but surgeons might evaluate its severity in view of aggressive benign or malignant lesions of diaphysis. We present the case of a 11-year-old female with left shaft ulna ABC.

Abstract

A foreign body that enters the airway through aspiration may become lodged in the trachea, bronchi, or larynx. The size, shape, and type of the foreign body determine where it should be lodged. Children are more frequently impacted. Unexpected inspiration during play or fighting while holding something in their mouths can result in accidents. The first study of choice for any child being evaluated for foreign body aspiration is still a plain film chest X-ray. Computerized tomographic examinations have a slightly higher sensitivity. We report a case of aspiration foreign body in a 12-year-old boy with the main complaint of choking. On physical examination, there were decreased breath sounds and wheezing in the upper left lung area. Chest x-ray revealed a radioopaque needle pin-like foreign body and no abnormalities in both lung. The results of a chest computed tomography (CT) scan showed a needle was abaout 4cm long and located in left bronchus. Once the diagnosis is made, removal of the foreign body with bronchoscopy is performed in these cases. Chest CT scans may need less time for evaluation and provide finer image resolution, making it easier to identify the position, size, and form of AFBs. As a result, chest CT scans offer great potential usefulness in the investigation of suspected AFBs.

Abstract

Sepsis is a life-threatening organ dysfunction caused by dysregulation of the body's response to infection, and is one of the leading causes of death in intensive care units. In sepsis, the systemic inflammatory reaction that occurs activates the coagulation system and increases the consumption of clotting factors, leading to a serious complication of coagulopathy known as disseminated intravascular coagulation (DIC). DIC is a clinicopathological syndrome that is a complication of various diseases characterized by the activation of systemic blood coagulation and intravascular fibrin production, which can cause thrombosis of small- and medium-sized blood vessels and can even cause organ function failure along with the consumption of platelets and coagulation factors, resulting in a clinical picture of bleeding. In 35% of severe sepsis cases, and in shock sepsis DIC causes high mortality, which is associated with increased severity of illness and serious organ failure with poor outcomes. This article aims to summarize the evolving understanding of DIC pathogenesis in patients with sepsis and its implications for current and future management strategies.
