Abstract

Background: Giant Cell Tumor (GCT) is a benign tumor but can be malignant and spread to other organs. Common site of GCT develops in long bone, especially in the knee. A lesion that develops in flat bone is considered rare. This case report aims to recognize GCT of bone and its predilection and help improve diagnostics and manage this case to provide better results.Case Report: A seven-year-old female came with complaints of lump and intermittent pain in left-back. The lump is gradually getting more prominent. The pain is localized in the back mid-area, worsen with any touch around the area. On examination, there is deformity and mass on the posterolateral side at the 9th rib level around 3 cm in diameter. There was tenderness on palpation and limited range of motion (ROM). The cytological and radiological finding suggests GCT of bone. The patient had resection surgery of the lesion. The histopathologic results on the operative specimens confirmed the diagnosis of GCT.Discussion: An unusual location of GCT lesions needs to be a concern because they can be underdiagnosed not to receive optimal therapy. Inadequate therapy can increase the incidence of recurrence and malignancy changes. In this case, the patient had resection surgery. Postoperatively, the patient is in good condition with no sign of local recurrence.Conclusion: GCT should be included in the differential diagnosis of rib-originated tumors.  Tumor lesions in unusual places other than long bones and occurs on the immature patient should be particular concern and confirmed diagnosis.

Abstract

Background: Flexor tendon injuries are potentially disabling, as flexor tendons are essential to hand function, playing a vital role in all types of grip, including power and fine pinch grips. However, there has been no consensus regarding the most effective repair technique for this pathology.Methods: A systematic search was conducted based on PRISMA guidelines to identify relevant studies through PubMed, Google Scholar, and Cochrane. A total of 9 studies (266 tendons from 108 patients) were included.Results: In a comparison between the Modified Kessler and 4-stranded Cruciate techniques, the Four-stranded Cruciate Suture produces a higher 2 mm gap strength (I2 = 93%, p < 0.00001), higher ultimate strength (I2 = 99%, p = 0.02), and better Functional Outcome as measured using the Strickland Criteria (I2 = 0%, p < 0.0001). In a comparison between the 2-Stranded and the 4-Stranded Kessler technique, the 4-Stranded Kessler technique produces a higher 2 mm gap strength (I2= 98%, p =0.02) and higher ultimate strength (I2= 60%, p <0.00001).Conclusion: The current systematic review and meta-analysis suggests that the 4-stranded cruciate repair technique has better strength and functional outcomes than the modified Kessler, especially in zone II and III injuries. The 4-stranded Kessler is also proven to have better strength compared to the 2-stranded Kessler.

Abstract

Background: Giant cell tumor is locally aggressive benign tumor that occur more common in distal femur, proximal tibia, and distal radius, and has tendency for recurrence and has capability for metastatize. Giant cell tumor in foot ankle are rare, and comprise less than four percent of giant cell tumor of the bone.Case Report: A 33 years old man presented with complains of pain and swelling in right ankle since 6 months ago with no history of trauma. Physical examination revealed increasing swelling over the distal-posteromedial aspect of lower leg and limitation ankle movements due to pain. Routine blood investigations showed slight increase of alkaline phosphatase. X-ray and Magnetic Resonance Imaging showed the lesion with morphology suggestive of giant cell tumor. Histopathology examination showed multinucleated giant cells and spindle shaped of mononuclear cells suggestive of giant cell tumor. The patient was treated with en-bloc-excision followed with distal tibial reconstruction using reverse intramedullary tibial nail combined with T-plate (tibio-talar arthodesis).Discussion: The main treatment of giant cell tumor of the bone is surgical removal to achieve tumor-free by eradication of the tumor, with various surgical techniques for reconstructing the cavity left. In this case we performed distal tibial reconstruction using reverse intramedullary tibial nail combined with T-plate.Conclusion: The patient at 12-month follow up is doing well, walking comfortably without any pain, has no limitation in range of motion with no signs of recurrence and good MSTS score (90).

Abstract

Background: Open fractures are a typical case in the orthopedics field. Infection in an open fracture can cause osteomyelitis. An antibiotic susceptibility testing of patient specimen bacteria with open fractures aimed to determine suitable antibiotic agents with which to treat the infectious diseases caused by these bacteria.Methods: This research is a descriptive study to assess antibiotic susceptibility in cases of open fracture grade III in Dr. Soetomo General Academic Hospital, Surabaya. Total sampling was performed on the microbiological culture results of patients diagnosed with open fracture grade III after debridement from October 2018 to September 2019. The identification of the microbes was based on Gram-positive and Gram-negative categories, and the classification was based on susceptibility to antibiotics, whether sensitive, intermediate, and resistant.Results: The examination of the data from the microbiological culture results of patients with a diagnosis of open fracture grade III after debridement from October 2018 to September 2019 in Dr. Soetomo General Hospital Surabaya resulted in 56 research subjects who met the criteria. Acinetobacter baumannii was the most common bacterial species found in the microbiological examination of patients with open fractures, at 15.84%. Cefazoline and ceftriaxone showed low susceptibility. Levofloxacin showed a relatively good value of susceptibility against both Gram-positive and negative bacterial groups.Conclusion: The antibiotic susceptibility pattern of bacteria from specimens in open fracture grade III patients in Dr. Soetomo General Academic Hospital Surabaya varies between each species of bacteria isolate.

Abstract

Background: Osteochondroma is a benign bone tumor that most cases are asymptomatic. The symptoms may occur as a complication of osteochondroma, which is rarely happened. Surgical intervention is needed to treat the complications.Case:  A 20-year old male football athlete presented with pain on his left thigh's lump after he fell during his training. Mass and tenderness on the medial side of the left distal femur were found. Plain X-Ray showed an irregular mass on the left distal femur's surface with a fracture through the stalk of pedunculated osteochondroma. The patient underwent total excision of the osteochondroma.Discussion: The fracture of the osteochondroma often occurs in teenagers because they are more active physically. It happens due to direct or indirect injury. The treatment of this fracture is controversial between observation or surgical excision. In this case report, the patient could demonstrate full weight-bearing right after the surgery.Conclusion: Fracture as a complication in osteochondroma often happens to teenagers, and surgery proved to give a good outcome in this case.
