Abstract

Background: Bullous pemphigoid is the most common autoimmune subepidermal blistering disorder, accounting for approximately 80% of subepidermal immunobullous diseases. It mainly affects elderly patients aged 60–80 years and typically presents with tense bullae, widespread pruritus, and erosive skin lesions. The disease may arise from genetic predisposition and environmental triggers, including drugs that disrupt immune tolerance to dermal–epidermal junction antigens. Objective: This case report aimed to describe the clinical features of bullous pemphigoid, its possible relationship with acyclovir use, and treatment outcomes in a rural health-care setting. Method: This study used a qualitative single case report approach based on clinical observation, patient history, physical examination, and treatment evaluation. Results: A patient developed generalized tense bullae after receiving acyclovir therapy for herpes zoster. The lesions resembled bullous pemphigoid and were suspected to be related to acyclovir-induced immunological activity and its chemical structure. Management was carried out with systemic corticosteroid therapy and supportive care. Conclusion: Acyclovir may act as a rare trigger for bullous pemphigoid. The patient showed improvement in clinical signs and symptoms after seven days of steroid treatment. This finding emphasizes the need for careful monitoring of cutaneous adverse reactions following antiviral therapy, especially in resource-limited rural clinical settings.

Abstract

Background: Low adherence to iron (Fe) tablet supplementation among pregnant women remains a major barrier to preventing maternal anemia and its associated adverse pregnancy outcomes. Objective: This study aimed to analyze the factors influencing pregnant women’s adherence to Fe tablet consumption, with particular emphasis on antenatal care (ANC) visits and husband’s support. Methods: An analytical observational study with a cross-sectional design was conducted among 96 pregnant women attending the Anggalomoare Public Health Center who met the inclusion criteria. Data were collected using a validated questionnaire and analyzed through descriptive statistics, Chi-square tests, and multiple logistic regression to identify factors associated with adherence to Fe tablet consumption. Results: Knowledge (p<0.001), attitude (p<0.001), husband’s support (p<0.001), and ANC visits (p=0.007) were significantly associated with adherence to Fe tablet consumption, whereas satisfaction with healthcare services was not significantly associated (p=0.074). Multivariable analysis identified maternal attitude as the strongest predictor of adherence, followed by husband’s support and maternal knowledge. Conclusion: Improving pregnant women’s adherence to Fe tablet supplementation requires strengthening positive maternal attitudes, increasing husband’s involvement, and enhancing maternal knowledge through quality ANC services and family-centered health education, thereby supporting better maternal and fetal health outcomes and reducing the risk of pregnancy-related anemia.

Abstract

Background: Optimal adjustment of window width (WW) and window level (WL) is essential for improving anatomical visualization on abdominal CT scans in patients with intra-abdominal masses. Objective: To analyze the effects of different WW and WL settings on abdominal CT images and determine the optimal values for evaluating intra-abdominal mass cases. Methods: This quantitative observational study analyzed abdominal CT scans from six patients with radiologist-confirmed intra-abdominal masses. Images were reconstructed using different WW and WL combinations, and three experienced observers independently assessed the resulting anatomical information to identify the most appropriate image settings. Results: Significant differences were observed in the anatomical information produced by the various WW and WL settings. The combination of WW 300 HU and WL 60 HU provided superior visualization of abdominal anatomical structures and lesion characteristics compared with the other settings evaluated. These parameters consistently yielded clearer image contrast and improved diagnostic interpretation across the observers. Conclusion: Variations in WW and WL significantly influence the diagnostic quality of abdominal CT images in patients with intra-abdominal masses. A setting of WW 300 HU and WL 60 HU is recommended as the optimal protocol for enhancing anatomical visualization and supporting accurate radiological assessment in routine clinical practice.

Abstract

Background: Stunting is a global malnutrition issue affecting children's development worldwide. Machine learning (ML) has been applied to predict stunting; however, studies on international collaboration and ML application across various geographical contexts remain limited. Objective: This study aims to analyze publication trends and collaboration patterns in stunting prediction research using machine learning and identify the most relevant ML methods. Methods: A bibliometric analysis was conducted using the Scopus database, focusing on publications from 2019 to 2024. The data were analyzed through descriptive statistics and science mapping, including trend topics, co-occurrence networks, and international collaboration analysis. Results: The study found that regression-based algorithms and deep learning are the most widely used machine learning methods for stunting prediction. International collaborations between countries with high stunting prevalence, such as Indonesia, Bangladesh, and Ethiopia, were also identified. Conclusion: This study highlights the importance of developing locally tailored predictive models and strengthening international collaboration to improve the effectiveness of stunting prediction models. Cross-sector and interdisciplinary collaborations are also essential for more holistic solutions.

Abstract

Background: Osteoarthritis (OA) is a prevalent degenerative joint disease worldwide, and its pathophysiology remains incompletely understood. Clinical studies in humans face limitations due to variability in disease progression, slow onset, and numerous confounding factors. Therefore, animal models are essential for exploring OA mechanisms and potential interventions. Objective: This study aims to identify various methods for developing OA animal models and the histopathological scoring systems used to assess disease severity. Method: A systematic literature review was conducted using databases including PubMed, ScienceDirect, and Google Scholar, targeting articles published between 2015 and 2025. Keywords included “osteoarthritis,” “animal models,” and “histopathological assessment.” Totally 18 articles were analyzed. Result: The review found that OA models can be induced through natural aging, genetic modification, surgical destabilization, mechanical loading, or chemical induction (e.g., monoiodoacetate injection). Histopathological evaluation, commonly using Hematoxylin-Eosin and Safranin-O staining, is crucial to determine cartilage degradation and disease progression. Among scoring systems, the Mankin and OARSI methods are most widely used. OARSI scoring is simpler and faster but requires expertise for consistent interpretation. Conclusion: In conclusion, selecting appropriate animal models and histological scoring systems is critical for OA research, depending on study goals and available resources.

Abstract

Background: Health promotion is a hospital institution's effort to ensure the fulfillment of the public's right to obtain health-related information and education, particularly within the hospital environment, and to ensure the provision of holistic, effective, efficient, integrated, and sustainable health services. Objective: This study aims to examine the determinants impacting the implementation of health promotion programs at Idaman Hospital in Banjarbaru. Design: This study used a quantitative approach with a cross-sectional design. The sample consisted of 156 respondents selected using probability sampling techniques. Data analysis was performed using chi-square tests and logistic regression to identify determinants of hospital health promotion (PKRS) implementation. Results: The analysis revealed a significant relationship between PKRS implementation and regulations (p=0.003), assessment (p=0.004), intervention (p=0.008), and monitoring and evaluation (p=0.001). Logistic regression analysis revealed that the most dominant factor was monitoring-evaluation with a p-value of 0.003 and an odds ratio [Exp(B)] of 5.421, indicating that PKRS implementing units were 5.4 times more likely to succeed when implementing a continuous evaluation system. These findings recommend strengthening the evidence-based clinical audit system, human resource capacity through tiered training, and integrating the PKRS program into the hospital's strategic plan.

Abstract

Background: One of the psychological factors that influence childbirth is anxiety and fear during childbirth. Pregnancy anxiety and depression are major problems in society due to their high prevalence. In Pakistan, 48.4% of women experience depression during pregnancy, and in Canada, 31.2% of pregnant women experience depression. Meanwhile, in Indonesia, 53.8% of pregnant women experience severe anxiety in the third trimester. Objective: The purpose of this study was to determine the effect of endorphin massage on anxiety during the third trimester of pregnancy in Karangraharja Village, North Cikarang District, Bekasi Regency in 2022. Methods: This study used a pre-experimental method with a one-group pre-test post-test design. The population of all pregnant women in the third trimester in Karangraharja Village was 100 people. The sampling technique used purposive sampling with stated criteria. The sample in this study was 40 respondents. Data analysis used the Wilcoxon test and multiple logistic regression. Results: The results of statistical tests showed that endorphin massage had an effect on anxiety in the third trimester of pregnancy in primigravida mothers with a p value <0.05. Conclusion: There was an effect of endorphin massage on anxiety in the third trimester of pregnancy in primigravida mothers in Karangraharja Village.

Abstract

Background: Brain tumors are intracranial neoplasms that affect neurological function and patients' quality of life. Although their incidence is lower than that of lung and breast cancer, brain tumors are progressive and have a high mortality rate. Both benign and malignant tumors can cause serious complications due to limited space within the skull and their proximity to vital functional centers. To date, there has been no research on the characteristics of brain tumor patients in Ambon City. Objectives: This study aims to describe the characteristics of brain tumor patients at Dr. J. Leimena General Hospital from 2021 to 2024 based on age, sex, clinical symptoms, type, and location of the tumor. Method: Quantitative descriptive with a cross-sectional approach, using medical records with total sampling technique. Results: The most common age group was 56-65 years (36.1%), female (63.9%), and the most frequent clinical symptom was headache (32.8%). The most common tumor type was extra-axial (50.8%), and the most frequent tumor location was in the frontal lobe (39.3%). Conclusion: Brain tumor patients are predominantly aged 56-65 years, mostly female, with headache as the most common symptom. The most frequent tumor type was extra-axial, most commonly located in the frontal lobe.

Abstract

Background: Poor sleep quality is a common issue among medical students and may negatively affect cognitive functions, particularly memory, which plays a crucial role in learning. Objective: This study aimed to analyze the influence of sleep quality on memory function among medical students at the Faculty of Medicine, Universitas Muslim Indonesia, from the 2022, 2023, and 2024 cohorts. Methods: A cross-sectional design was applied to 91 students selected using proportional stratified random sampling. Sleep quality was assessed with the Pittsburgh Sleep Quality Index (PSQI), while memory performance was measured using the Everyday Memory Questionnaire (EMQ). Data were analyzed using Two-Way ANOVA. Results: Sleep quality significantly affected memory function (p = 0.000), with students reporting poor sleep showing higher memory impairment. Cohort level also had a significant effect (p = 0.000), where senior students demonstrated the highest prevalence of poor sleep quality (75%) and memory impairment (94.4%). Moreover, a significant interaction between sleep quality and cohort level (p = 0.018) indicated that the impact of sleep quality on memory varied across academic years. Conclusion: These findings highlight the importance of implementing interventions such as sleep management education, stress regulation, and regular monitoring of sleep quality, particularly for senior students.

Abstract

Background: Cardiovascular disease remains a major cause of morbidity and mortality among postmenopausal women, with central obesity and dyslipidemia identified as key risk factors. Waist circumference is a simple anthropometric measure that may reflect lipid profile changes, particularly in low-density lipoprotein (LDL) levels. Objectives: This study aimed to determine the relationship between waist circumference and LDL levels in postmenopausal women in the working area of Puskesmas Negeri Laha-Tawiri, Ambon. Methods: A cross-sectional study was conducted involving 60 postmenopausal women selected through purposive sampling. Waist circumference was measured using a standardized tape, and LDL levels were assessed through laboratory blood tests. Data were analyzed using descriptive statistics and Pearson correlation test with a significance level of p < 0.05. Results: The analysis revealed a significant positive correlation between waist circumference and LDL levels (r = 0.42, p = 0.003), indicating that greater abdominal obesity was associated with higher LDL levels. Conclusions: Waist circumference is strongly related to LDL levels among postmenopausal women, suggesting that it can serve as a simple and non-invasive screening tool for early detection of dyslipidemia. Preventive strategies focusing on lifestyle modification are recommended to reduce cardiovascular risk in this population.
