Abstract

Background: Molecular weight is one of the chitosan characteristic that affects the effectiveness of application for wound healing in dental extraction. Chitosan could improve wound healing process because of its anti inflammation component and accelerateproliferation phase. Angiogenesis is one of the main component in proliferation phase because it can preserve cells function to accelerate wound healing process. Purpose: the aim of this experiment is to account blood vessels in wound healing of dental extraction using chitosan with different molecular weight. Materials and Methods: Thirty six male Rattus Norvegicus were divided into 3 groups. Group I (without chitosan), group II was given low molecular weight chitosan, group III was given high molecular weight chitosan. Rats were sacrificed on day 7 and on day 14 post extraction then they were examined by hystometri to see the change in blood vessels number. Result: Data were statistically analyzed with One Way ANOVA and LSD with degree of significant p<0,05 showed significant difference between high molecular weight chitosan group and low molecular weight chitosan group after 7 and 14 days observation. Conclusion: High molecular weight chitosan was found more effective to increase blood vessels amount at 7th day and decrease blood vessels amount that 14 day in wound healing of dental extraction process.

Abstract

Background: Immediate Denture is once of the Prosthodontics therapy to patients who will be take some tooth extraction in the anterior teeth or sseveral teeth, or even in the exostosis case. Patients who for some reason to have to anterior teeth removed, will experience the aesthetic and psychological disorders. Purpose: The immediate denture intended that the patient does not experience toothless due to tooth extractions and patients can immediately use the artificial tooth immediately without waiting for the healing process. Case: Complete denture will prepare and then it the same time some teeth are planned to be extraction on theworking cast will be remove by cutting it and the complete denture inserted immediately after the extraction teeth and alveolectomy on the patient. Case Management: Control on 3 days, 7 days, 4 weeks, and 8 weeks post-insertion to see the resorption process after tooth extraction, so it maybe need to be relining and rebasing the artificial teeth. 7-day control the healing process already looks better and so with the retention. At 4 weeks after insertion of control, the artificial denture become unstable and it need to be denture relining and rebasing process using a soft liner. At 8 weeks after insertion control, the alveolar ridge has been perfect healing process, and the dentures should be relining and rebasing re-use by self cure acrylic.

Abstract

Background: Oral candidiasis is an infection caused by Candida albicans. Candida infection can occur in patients with Diabetes Mellitus (DM) due to the high sugar content in the whole saliva and immunosuppresive condition of the patient. Purpose: to discuss case and treatment of acute erythematous candidiasis in patients with diabetes mellitus. Case: A 68 year old male came to RSAL dr. Ramelan Surabaya complaining of tendernes, burning and painful when eating and drinking in the left side of the tongue and. The tendernes was felt since about 8 months ago after the patient had a stroke. He had uncontrolled diabetic and hypertension.Three months later the patient felt the pain was getting worse with heartburn and pain. It was treated by given medication Nystatin drop and cefadroxil regularly until the drug ran out. During the first 2 months of treatment the tongue looked better but the pain and burning persisted. Shortly after,the tongue becomes dirty again. Case Management: Screening for diagnosis in this case include pathology anatomy examination by scrabbing the lesion, complete blood count(CBC) and blood sugar test. The result was positive hypae, normal CBC, and blood sugar level 324mg/dl. Patient was diagnosed acute erythematous candidiasis and treated with anti fungal systemic, mouthwash and topical antiseptic for oral case and reffered to interne specialist for diabetic condition. Patient was cured in 36 days. Conclusion: Controlling or eliminate predisposing factor in this case (DM) is very important in the management of acute therapy erythematous candidiasis. Comprehensive treatment for oral candidiasis in diabetic patient must include controlling predisposition factor and the right choose of anti fungal agent.
