Abstract

Fat embolism is a clinical syndrome characterized by the systemic spread of fat emboli which often occurs due to fractures in long bones. We report a 29 year old man with a diagnosis of a fracture of the medial third of the right humerus accompanied by thrombocytopenia which could be one of the clinical manifestations of pulmonary embolism. A 29 year old man came to the emergency room at RAA Soewondo Pati Regional Hospital with complaints of pain in his right hand 2 hours before entering the hospital. The patient felt pain after falling and slipping at home. Physical examination revealed tachycardia and tachypnea. Humeral x-ray revealed discontinuity in the medial 1/3 of the right humerus. The patient then underwent treatment for the fracture and the day after a routine blood test was carried out. Routine blood tests revealed thrombocytopenia.Fat embolism is a dangerous complication of long bone fractures. Often this fat embolism is not recognized until the patient shows symptoms in several systems, especially the respiratory system such as respiratory distress and neurological system disorders ranging from motor disorders to seizures. Apart from that, fat embolism can also disrupt the coagulation system so that thrombocytopenia can occur. In this patient there was tachypnea and thrombocytopenia which could be early symptoms of fat embolism. Fat embolism is a complication of long bone fractures that is rare but dangerous if not properly recognized. Immediate management of fractures and collaboration with anesthesiologists and internists are important to prevent fat embolism.

Abstract

The maternal and child health program in Sukoharjo district includes several reports on childbirth, postpartum, immunization reports, nutritional counseling, midwife courses, home visit reports, and family planning services, both in hospitals, health centers, maternity homes, and midwives. The maternal and child health surveillance that has been carried out so far has only been carried out to fill in data reports to health agencies at the central level. It has not been presented and analyzed properly, let alone disseminated the information to those who need it. Therefore, efforts are needed to improve the quality of epidemiological data and information so that planning and evaluation are well coordinated to improve the quality of the analysis results of surveillance data studies in terms of timeliness and sensitivity of reports. This research aims to determine the weaknesses in the need for a maternal-child health surveillance system and a maternal-child health surveillance system. The intervention method used is through on-the-job training. Meanwhile, district officers who have been trained previously (tiered training) train maternal and child health workers at community health centers. From the results of the analysis, it can be concluded that the maternal and child health program surveillance system is generally running well.

Abstract

Most of the unexpected events occur in the hospital wards. An Early Warning System (EWS) is a system created to identify worsening patients outside the Intensive Care Unit (ICU). EWS is one of the requirements that must be met to get hospital accreditation. Hospitals have to choose the appropriate EWS to get optimal outcomes. This study aims to describe some of the EWS in the wards that have been researched and developed, as well as their performance in predicting severe adverse events (SAE). This study is a literature review design, conducting a systematic review by selecting relevant articles on Pub Med and Science Direct using the keyword "Early Warning Systems" in the 2018-2023 period. Out of 269 articles, only 12 articles that met the criteria. The selected articles are then systematically reviewed and analyzed. Based on the extraction results of 12 articles, 9 EWS were grouped into National Early Warning Score (NEWS) and its variants, Modified Early Warning Score (MEWS) and its variants, and EWS integrated with Electronic Medical Record (EMR). In choosing an EWS, hospitals must consider the complexity of the cases being managed and the capabilities of existing resources. The recommended EWS in hospital wards with limited resources is weighted EWS or EWS with combinations, such as NEWS and MEWS with variants. Meanwhile, hospitals that have used EMR can choose EWS integrated with EMR to increase their predictive value for SAE, as well as improve protocol compliance.

Abstract

The World Health Organization reported 2.7 million perineal injuries in women giving birth in 2009, reaching 6.3 million in 2020. A study was conducted to determine postpartum mothers' knowledge about kegel exercises to prevent perineal wounds. The study involved 30 respondents, with 83% having less knowledge than others. The majority had less knowledge of housework, tertiary education, and health workers. The study suggests that more knowledge is needed from health workers, electronic media, and print media. Respondents' knowledge about Kegel exercises for healing perineal wounds was based on information sources, the majority had little knowledge with health workers as many as 15 people (50%) and the minority had enough knowledge with electronic media as many as 2 people (7%).

Abstract

At the global level, the prevention of stunting is one of the Sustainable Development Goals (SDGs). Indonesia is currently working towards achieving the second Sustainable Development Goal, which entails ending hunger, ensuring food security, improving nutrition, and supporting sustainable agriculture. As a part of this objective, efforts are being made to decrease the prevalence of stunting in children by 2025. Attention towards stunting is crucial as it can adversely affect children's physical and cognitive development well into adulthood if not addressed appropriately. This paper conducted a literature review from various sources, with a focus on google scholar and prioritizing sources from the last five years, as well as research on the population in Indonesia. It was determined through the analysis of several sources that maternal, child, and environmental factors pose various risks for stunting in Indonesia. Maternal factors that may play a role in child development include the mother's age, upper arm circumference, height, breastfeeding or complementary feeding practices, early initiation of breastfeeding and food quality during pregnancy. A history of low birth weight or prematurity, male sex, neonatal illness, frequent and recurrent diarrhea, infectious diseases, and lack of immunization are among the child factors associated with developmental outcomes. Factors such as living in a low socio-economic environment, limited family education, especially maternal education, insufficient household income, open defecation in rivers, gardens or inadequate latrines, consumption of untreated drinking water and high exposure to pesticides are also associated with the prevalence of stunting.

Abstract

Background: The COVID-19 epidemic has impacted persons across all age demographics, including children. An opposing viewpoint is that despite the extensive discussion surrounding the neutrophil-to-lymphocyte ratio as a potential indicator of clinical symptoms in pediatric COVID-19 cases. This systematic literature review aims to explores the correlation between CT readings, neutrophil-to-lymphocyte ratio, and clinical symptoms in pediatric cases with COVID-19. Method: This systematic review examined the existing research on Cycle Treshold (CT) and Neutrophil-To-Lymphocyte Ratio (NLR) values as indicators of the clinical progression in children with COVID-19. An extensive search of databases Scopus revealed studies that satisfied our inclusion criteria. They utilized CT valuess and the neutrophil-to-lymphocyte ratio as indicators for predicting clinical symptoms. The inclusion criteria for this study were specifically centered around juvenile patients with COVID-19 from scopus databased. The chosen studies underwent a thorough evaluation to determine their quality and relevance. Various nations, including Korea, China, Italy, the United States, Egypt, South Africa, India, Japan, Brazil, and others. Result: The papers included in the analysis demonstrate a noteworthy correlation between CT readings and clinical symptoms in children with COVID-19 Pediatric COVID-19 cases with elevated CT values demonstrated a greater propensity for severe clinical manifestations, including respiratory distress and pneumonia. Upon reviewing the available research, it is evident that hematological parameters, specifically the neutrophil-to-lymphocyte ratio, significantly impact forecasting the severity and clinical prognosis of juvenile COVID-19 cases. Conclusion: The neutrophil-to-lymphocyte ratio and CT values were identified as dependable indicators of illness severity in pediatric patients. The results emphasize the significance of hematologic measures, specifically the neutrophil-to-lymphocyte ratio, as helpful markers for evaluating the clinical symptoms and outcomes of juvenile COVID-19 patients.

Abstract

The primary objective of this study is to investigate the extent to which depression, stress, anxiety, and burnout in Indian seafarers. The study collected data from 109 Indian seafarers through Google Forms from March 2023 to April 2023. Descriptive statistics and inferential statistics were used to summarize the data based on selected demographic factors. The results showed that Indian seafarers suffered from mild depression (Mean =13.54) Seafarers also suffer from moderate levels of anxiety (Mean= 10.81). The levels of stress are found to be of normal level (Mean=13.28). The average scores for the components of burnout, namely Disengagement, and Exhaustion, among seafarers are 20.03 (SD= 3.673) and 20.43 (SD= 4.051) respectively. These findings suggest that seafarers exhibit moderate levels of burnout. Also, the correlational analysis shows a positive moderate correlation between total scores of depression, anxiety, stress, and components of Burnout with a value of 0.392 at a 1 % level of significance. The study highlights the need for mental health interventions and support services for seafarers to prevent and manage mental health issues in this population.

Abstract

The low number of cases found compared to the incidence of Tuberculosis (TB) shows that the control of this disease becomes a serious problem. The stigma is suspected to be one of the obstacles to solving the TB problem in Indonesia. This study aims to analyze the ability of the Jaga-rasa Jaga-tangga program to decrease TB stigma in two Public Health Centers (PHCs) in Sleman Regency, Indonesia. This is a quasi-experimental study design with a control method. Both intervention and control groups get health education about TB stigma. The intervention group also gets an education about the Jaga rasa Jaga tangga (take care of your neighbors as well as take care of their feelings) program. The outcome measure was the TB stigma scale that measure before and after the intervention. Data were analyzed using paired t-tests and independent t-tests. 106 respondents were involved in this study divided into intervention and control groups. Age (p=0.006) and social status (p=0.015) are associated with TB stigma in the community. There were significant changes in the TB stigma scale after the intervention (paired =0.305; p=0.001). The Jaga-rasa Jaga-tangga program along with health education reduces TB stigma more effectively than health education only. Further studies are needed to confirm this result.

Abstract

Development of galenic preparation formulations that can be used to make anti-inflammatory cream preparations. Snail seromucoid contains glycoproteins, mannose carbohydrates, glucose, N-acetyl muramic acid and the α-1 globulin-oromucoid fraction which functions as a bioimmunostimulator. Durian peel (Durio zibethinus L.) contains a polar polysaccharide polymer compound such as D-galacturonic acid which has anti-microbial properties. Chitosan is a polymer compound resulting from derivatization of chitin which has antimicrobial polycationic properties; biodegradable, and biocompatible with animal body tissue. The aim of the research was to examine the effectiveness of a galenic anti-inflammatory cream preparation based on snail seromucoid, durian skin extract, and chitosan for healing acute and chronic wounds in vivo. The research method is experimental research, namely the effectiveness of anti-inflammatory galenic cream preparations made from snail seromucoid, durian peel extract and chitosan on various types of wounds in vivo. Analysis of observation data using the one way anova test. A galenic preparation based on a combination of 2% snail seromucoid, 2% durian peel extract polysaccharide and 2% chitosan ratio 1:1:1 is effective in the healing process based on the formation of granulation tissue thickness in incision injuries, hot objects injuries, second degree burns and ulcers. diabetes. The bioformulation galenic preparation of 2% snail seromucoid, 2% polysaccharide of durian peel extract; 2% chitosan and their combination with ratio 1:1:1 is effective as an anti-inflammatory galenic cream preparation so that it can be applied for healing chronic wounds, acute wounds and diabetic ulcers.

Abstract

This study aims to analyze the knowledge and attitudes of surgical room staff in providing early breastfeeding initiation (IMD) at Efarina Hospital, Pematangsiantar. This type of research is cross-sectional, which is a research design by conducting observations at the same time. The study was conducted in the surgical room of Efarina Hospital, Pematangsiantar, North Sumatra in 2024. This study will be conducted in January-March 2024. The population in this study are nurses and midwives who are surgical room staff at Efarina Hospital, Pematangsiantar in 2024, which is 10 people. After conducting research on the description of knowledge and attitudes of Surgical Room Officers in providing early breastfeeding initiation (IMD) at Efarina Hospital Pematangsiantar in 2024 with 10 respondents, the following results were obtained: The majority had sufficient knowledge, namely 4 people (40%), while the minority had good knowledge, namely 3 people (30%) and less knowledge, namely 3 people (30%). The majority had an attitude of agreement, namely 7 people (70%), while the minority had an attitude of disagreement, namely 3 people (30%).
