Abstract

Glaucoma is a neurodegenerative disease with certain characteristics of progressive damage to the optic nerve, loss of retinal ganglion cells (RGC) and their axons, accompanied by typical visual defect. Increased intraocular pressure (IOP) is the main risk factor for glaucoma. Some study stated that RGC death is still progressing despite the use of IOP-lowering medications. The main aim of glaucoma treatment remains to lower IOP, however neuroprotective agents have increased interests due to its effects to prevent or slow down the rate of RGC death other than IOP-dependent mechanisms. Production of glutamate and nitric oxide is known as one of the contributing factors of RGC death, and neuroprotective agents that work by targeting the production of glutamate and nitric oxide can be a valuable option to prevent further RGC death.

Abstract

Background: Psoriasis is a chronic, recurrent, inherited immunologic disorder. Psoriasis is characterized by changes in epidermal cell growth and differentiation. The mechanism of skin inflammation involves the activation of cytokines, chemokines, and growth factors that results in impaired regulation of keratinocytes, inflammatory cells, and blood vessels. This study aimed to evaluate incidence and profile of psoriasis patients at Martha Friska Multatuli Hospital, Medan, in January-December 2022. Method: This is a descriptive study using retrospective, cross-sectional design, where data collection is carried out from the medical record data of Martha Friska Multatuli Hospital. Results: Most of psoriasis cases (42 cases; 65.63%) in Martha Friska Multatuli Hospital is existing patients. Psoriasis was reported to be more prevalent in women (33 cases; 51.57%). Based on body mass index (BMI), psoriasis was found the most in those whose BMI <24,9 (34 case; 53.14%). Based on age, psoriasis was found the most in elderly (>60 years) with 26 cases (40.64%).

Abstract

Background: Morphea is a chronic inflammatory disease characterized by sclerosis of the skin which belongs to the group of idiopathic sclerotic diseases. Frontoparietal linear morphea is a unilateral atrophy linear plaque on the forehead that can extend to the eyebrows, nose, lips, and scalps, causing alopecia. The aim of this case report is to establish the diagnosis of frontoparietal linear morphea. Case: A 20-year-old woman presented with a dark brown spot on her forehead. Histopathological examination showed a basket-weave hyperkeratosis and flattening of the rete ridges of epidermal layer, and pigmented cells in the basal cells of the dermal-epidermal junction. Based on the results of physical examination and histopathology, patient was diagnosed with linear morphea en coup de sabre. Patient was treated with topical and systemic corticosteroid. Discussion: Morphea is a rare fibrotic disease of the skin and underlying tissue, characterized with diffuse thickening, induration, or atrophy. Histopathological biopsy examination can be used to confirm the diagnosis and rule out differential diagnosis. Histopathological biopsy of early phase morphea can show inflammation, the dermis looks dense with thickened collagen fibers accompanied by inflammatory cells, especially perivascular lymphocytes between collagen fibers, flattening of the epidermal rete ridges, and atrophy. Topical therapy with corticosteroid as anti-inflammatory drug can be used in active lesions of early-stage linear morphea.

Abstract

Background: Artocarpus altilis is a plant whose every part is beneficial for health. In Indonesia, the distribution of this plant is only found in certain places and has not been cultivated. Breadfruit is basically rich in flavonoid, tannin, and saponin compounds that are known to be potential as antidiabetic and antiinflammatory. Diabetes is a group of metabolic disease characterized by hyperglycemia, due to lack of insulin production and/or sensitivity. Type 2 diabetes is a degenerative disease that is a global health problem. Type 2 diabetes is treated using antidiabetic agents and/or insulin. Inflammation is a local reaction of the body characterized by certain symptoms. Treatment of inflammation using synthetic drugs is associated with various side effects. Objective: This review is aimed to provide information about antidiabetic and anti-inflammatory activities in breadfruit plant. Results: Substance of breadfruit plant with antidiabetic efficacy was found in the stem, leaf, and fruit, showed by results of phytochemical test. Antidiabetic activity was proven by in vivo, in vitro, and AlCl calorimetry tests, while anti-inflammatory substance was found in the leaf and fruit, and the anti-inflammatory activity was showed by results of in vivo and in vitro tests. Conclusion: Breadfruit plants can be potentially developed as an alternative treatment for diabetes and inflammation.

Abstract

Background: Pyoderma gangrenosum (PG) is a complex neutrophilic dermatosis, characterized by sterile, painful, necrotic ulcer, and is associated with systemic conditions. PG manifests as papules and vesicles that evolve into painful ulcers. Incidence of PG is relatively rare in Dr. Moewardi Hospital, so it is important to identify and correctly diagnose PG based on its’ histopathological features. Case A 54-year-old man complained of painful scab on several body parts. Hematoxylin and eosin staining in the epidermal layer showed a basket-weave orthokeratosis with necrotic tissue, acanthosis, and basal cell hypermelanosis. In the dermis layer there is a lymphocyte and neutrophil cell infiltrate with slight appearance of leukocytoclastic. Discussion: Pyoderma gangrenosum is a reactive, non-infectious inflammatory dermatosis (neutrophilic dermatoses). In this case, PG occurs in 54-year-old man as painful scab on back and legs. Classical PG characterized by papules, pustules, crusted or necrotic plaque, which typically have undermined, overhanging, dusky purple edges with surrounding induration and erythema. In this patient, the epidermal layer showed ulceration and slight appearance of leukocytoclastic vasculitis in the dermis area and lymphocyte infiltrate with slight neutrophil in the perivascular accompanied by erythrocyte extravasation, which support the diagnosis of PG.

Abstract

Striae or strechmark is a form of benign skin lesion manifested as inflammatory streaks perpendicular to the direction of skin tension which has cosmetic morbidity and possesses risk of secondary infection in severe case. Various therapeutic modalities have been introduced, such as topical therapy, physical therapy using devices, lasers, and the latest is celullar therapy using platelet-rich plasma. Such varied treatment modalities are reported to have varied results.
