Abstract

Background: Regarding the eye problem caused by narrow-angle that may be undetected or untreated, a database should be collected. Narrow angle in adult is a risk factor of glaucoma. The data of narrow-angle in adult population in Indonesia are lacking. Objective: To estimate the prevalence of narrow-angle according to the van Herick grading and ratio in Javanese population. Method: A cross the sectional study, community based ophthalmology study was carried out in Yogyakarta. One thousand two hundred twenty two (1222) adults aged 20 years and older were selected by stratified random sampling from each village of 18 subdistricts and examined at a KOWA SL15 portable slit lamp. The anterior chamber depth was graded according to the Van Herrick standard protocol. Grades I and 2 were classified as narrow-angled eyes. Result: Of 1222 subjects, 98 eyes (48 right eyes and 50 left eyes) of 50 subjects were diagnosed as narrow angle. The prevalence rate was 4.1%. The subjects were 13 men, and 37 women. The number of narrow-angle eyes increased with age 40 years and older for both men and women (OR = 2.77, 95% Cl 1.09-7.09). Conclusion: There are significantly more patients with upper van Herrick ratios or grades in Javanese population. The Van Herrick ratio increases with age and narrow-angle glaucoma occurs more frequent in women more than in men. Abstract

Backgraund: The prevalence and outcome of childhood cancers in developing countries is not well documented. In 2001, the Yogyakarta Pediatric Cancer Registry (YPCR) was developed in collaboration with the Saskatchewan Cancer Registry in Canada. Its purpose was to collect and analyze demographic, treatment and outcome data in children newly diagnosed with cancer at Dr. Sardjito General Hospital, Indonesia, Objective: The objectives of this study were to analyze clinical profiles and treatment responses in children under 15 years of age diagnosed with neuroblastoma in 1 January 2000 – 31 December 2004. Materials and Method: A retrospective review of health records was conducted. Information on all patient diagnosed with cancer from 1998 onwards was transcribed onto a standardized data capture form, and then entered into the computerized YCR data base for review and analyses. Result: A total of 27 children with neuroblastoma were identified within the 5 year study period. Twenty two patients (81.5 %) had stage IV disease, 4 (14.8%) had stage II/111, and 1 (3.7%) stage 1. Male to female ratio was 1.5:1. Mean age at diagnosis was 3.1 years. Nineteen patients (70%) were treated on the OPEC protocol, 3 (11%) per the N6 protocol, and 1 (4%) on the NS4E protocol; 4 (15%) were not treated. Of the OPEC treated group, 10 (53%) patients are surviving, 6 (16%) relapsed and 8 (42%) were lost to follow up. Conclusion: In keeping with published reports, the majority of children with neuroblastoma presented with advanced stage disease. The 3-Years free survival was 20% for patients treated on the OPEC protocol. More effective treatment protocols and improved surveillance of children following discharged from hospital are challenges that need to be addressed. Keywords Neuroblastoma-Sardjtio Hospital-outcome

Abstract

Background: Diarrhea is one of the infectious diseases and is the main cause of under-fiveyear-old children's morbidity and mortality in Indonesia especially children aged less than two years. Many diseases or health conditions of the people result from or get worse from their behavior. One of the ways to change the attitude people's behavior in health is the increase of knowledge and people's behavior with health educational approach using various methods. Objectives: This study was aimed at finding out the difference between increasing knowledge and attitude of mothers about diarrhea of under-five-year-old children and their treatment at home by using participatory method of education and informative method. Methods: This was a quasi experimental study using nonrandomized pretest-posttest control group design. The subjects were mothers with under-five-year-old children. There were 68 samples, 34 of whom were in the treatment group and the other 34 were in the control group and each was in Tegalrejo village and Dukuh Dungus village, Grabag Sub-District, Purworejo Regency. Data analysis using chi-square was used to test the group's homogeneity, t-test was to test the difference of average rate of knowledge and attitude, and multiple regression analysis was used to know the effect of other variables or the dependent variables. Results: The results showed statistical significant difference (p0.05). In terms of mothers' increasing knowledge and attitude toward diarrhea from pretest 1 to posttest, both methods statistically showed significant increase (p0.05). Conclusion: Health education through participatory method is more effective than increasing knowledge and this was less effective to increase mothers' attitude on diarrhea compared with informative method. Abstract

Background: The objective of this study was to analyze the situation of the empowerment of poor community for making pregnancy safer (MPS) in rural areas including the empowerment of health efforts and fund mobilization and transportation aid to refer deliveries. Method: It was a descriptive explorative study held in three districts, i.e. Kupang, Timor Tengah Selatan (TTS) in Nusa Tenggara Timur, and Ponorogo in East Java. In each district, two poor sub districts were chosen and in each sub district, ten poor villages were chosen. The analysis unit in this study was village. The data were gathered with in-depth interview, structured interview, secondary data, and Focus Group Discussion (FGD), and were analyzed descriptively. Result and Conclusion: The result shows that the empowerment of traditional birth attendants' activities, integrated health service post activities, village delivery house activities, and cadre activities performed by the community varied. The empowerment for village medicine post activities was relatively low. The empowerment of the community in fund mobilization on maternal health was hard to do due to the implementation of social safety net on health sector (JPSBK) fund as well as PKPS BBM on health sector. The empowerment to providing transportation in villages seems to invoke minimum responses. Keywords: empowerment, making pregnancy safer, traditional birth attendance integrated services post, village delivery house
