Abstract

Background: AKI is an indicator to determine the health statusin a country. The maternal mortality ratio in Indonesia was228/100.000 live births in 2008 to 220/100.000 live births in2010 and there was an increase to 359/100.000 live births in2013, while the MDGs target for 2015 was 102 per 100.000 livebirths. In Indonesia, the main causes of maternal death arebleeding, hypertension and infection. Antenatal care has beenproven effective in early detection of conditions that causematernal death. social approaches also need to be consideredas indirect causes such as socio-demographic factors,unavailability of birth attendants, costs. In public healthsciences, the social approach is a social determinant of health.The use of Antenatal Care is not optimal due to the limitedsupport of the health system and the influence of socioeconomicconditions and the rural environment.Purpose: To identify determinants of antenatal care visits inrural areas in Indonesia based on data analysis of Riskesdas2013.Methods: This type of research is quantitative using data fromthe 2013 Basic Health Research. Respondents in this studywere mothers who had been pregnant and gave birth during theperiod of January 1st 2010 to 2013 aged 10-59 years and live inrural Indonesia Results: After a statistical test was carried out, the antenatalcare visits in rural Indonesia were 90.41%. The factors that mostinfluenced antenatal care visits in rural areas were informationon knowledge of the availability of midwives for practice (Pvalue= 0.001, OR = 2.01) and information on knowledge of theavailability of posyandu (P-value = 0.001, OR = 1.93). Conclusion: Antenatal care visits in rural Indonesia are stillquite high. There is a relationship between individual factors andhealth system factors on antenatal care visits in rural areas.Individual factors cannot stand alone without health systemfactors. Health system factors are more important thanindividual factors. There needs to be an increase in the qualityand quantity of information for health cadres and the communityabout the importance of antenatal care visits.

Abstract

Background: Anambas Islands Regency is a district that iscategorized as a DTPK area. In the health context, rural andremote areas are often associated with limited public transportation,poor road infrastructure, long distances to health carefacilities and difficulties in recruiting and retaining healthworkers. As a result there is a significant impact on the provisionof adequate health services. The availability of insufficient resourcesat every health facility in Anambas Islands Regency isalso an obstacle because in implementing JKN every healthfacility in Anambas Islands Regency must meet thecredentialing requirements set by BPJS Kesehatan.Objective: Analyzing the implementation of JKN policies byhealth service providers in Anambas Islands Regency. Methods: Descriptive research with qualitative methods usinga single case study design Results: Implementation of policy NHI by health care providersboth in health centers and hospitals are still many haveconstraints such as limited power specialist, especially inhospitals, general practitioners definitive still lacking in somehealth facilities, the limited infrastructure in health centers andhospitals that cause will not want the patient should be referred.This adds to the burden of transportation costs to society asocean freight rates are quite expensive though some things canbe addressed as a problem of information and improvement ofbureaucratic structures, but it can not prevent the public to bereferred.Conclusions: The implementation of the JKN policy is notsuitable for implementation in Anambas Islands Regency as aDTPK area because the benefits received by the communityfrom JKN are limited due to the many obstacles faced by healthservice providers.

Abstract

Background: According to WHO more than 50% of infant andchild mortality is related to malnutrition, a nutritional problemthat needs to be solved quickly and appropriately. Based on theresults of PSG Kadarzi years 2012-2015 the prevalence ofemaciation according to indices of BB /TB or BB/PB was 7,43%,7,32%, 9,81%, and 7,6%. Those number fluctuate but if thisnutritional problem is ignored it will cause problems ofdevelopment in the future. Consider this phenomenon we canfind out how the financing of prevention of malnutrition in CentralLombok regency issued by government and society. Objective: Identify financing countermeasures of malnutritionamong children under five years old in the district of Central Lombok, West Nusa Tenggara (NTB) Methods: This research is a descriptive study and crosssectionalresearch design with quantitative dates. This researchwas conducted in Central Lombok regency, West NusaTenggara Province. The subjects of this study were the motheror malnourished children keeper as many as 30 people. Thedata analysis was conducted by descriptive the financingcountermeasures of malnutrition in Central Lombok regency. Results: From the search results and interviews it showed thatthe malnutrition countermeasures costs spent by thegovernment was IDR2.816.913,00/person/year and the amountof public expenses was IDR7.874.100,00/person/year. Thelarge proportion of the cost of government was 26,35% and bythe public was 73,65%.Conclusion: The society expenses were 2,79 times as high asgovernment expenses on malnutrition countermeasures onchildren under five in Central Lombok regency.

Abstract

Background: Since 1980s the Ministry of Health has usedseveral approaches to determine staffing needs, usingprojections based on public health status, demographicchanges and health programs that exist. This study describesthe distribution of health personnel gap in eastern Indonesiadue to see the injustice of health workers in eastern Indonesiaso that the government can deal seriously and firmly to theproblems of the distribution of health personnel, particularlyareas that are difficult to reach. Purpose: This study aims to determine the unequal distributionof health personnel in the health centers in Eastern Indonesia. Methods: This study is a quantitative research with quantitativemethods using cross sectional design. Using a large-scalesecondary data from the Indonesia Family Life Survey (IFLS)East. Results: This study indicate that there is a big gap in equity ofhealth workforce in primary health care in eastern Indonesiaexperienced vacancies even some of primary health care bothdoctors, nurses, midwives and midwife. This is evident from thedistribution of health personnel imbalance between primarycare located in areas with high and low number of population;high and low poverty rates; geographic location based on theurban/rural and remoteness even provinces. Conclusion: Primary health care in eastern Indonesia moreexperienced vacancies in particular labor of doctors andmidwives, as well as the low number of community healthworkers to prove that primary health care is oriented onpromotive and preventive neglected. Optimizing the role ofgovernment as a regulator and facilitator to give more focus andhelp the areas that lack health worker especially in the provinceof East Nusa Tenggara, Maluku and West Papua are more likelyto be a shortage of public health vacancy even doctors andmidwives.

Abstract

Background: The main factor that determines the quality ofeducation in higher education is the quality of the lecturers in it.Quality nursing services need to be supported by nursingresources that are produced from quality educationalinstitutions according to established standards. In fact, thecurrent nursing education does not have a national standard sothat the quality of its graduates varies.Objective: This study aimed to describe and determine thecharacteristics of the lecturers that include educationalqualifications, functional, certification, and the ratio of faculty innursing studies program and see the extent to which facultycontribute to meet the accreditation status of private nursingstudies program in Yogyakarta.Methods: Analysis of a cross sectional quantitative data byunivariate analysis.Result: The status of accreditation in private nursing courses inYogyakarta (40%) accreditation of B and (60%) accreditedprograms C. length stand of Nursing program did'nt showevidence of accreditation better. Based on the educationalqualifications of teachers who are eligible according to thestandard Accreditation B (35%), which does not meet thestandards yet accredited B (5%), the variable functional and nolecturer certification courses that meet the standards of BAN PT.The ratio of lecturers in private nursing studies program is stillnot ideal proved to be only 10% of nursing study program thathas a good ratio. From observations and interviews obtainedevidence several nursing program still face many obstacles inimproving the quality of human resources and implementationof accreditation.Conclusion: The status of accreditation of private nursing inYogyakarta is still low, faculty of Nursing Prodi also still don'tmeet the qualifications in accordance with Law 14 of 2005. Ittakes a strong commitment from study program organizers andthe government to foster private nursing colleges in Yogyakarta.

Abstract

Background: Cancer is one of the catastrophic diseases whichhave the characteristics of high cost, high volume, and high risk.Based on Basic Health Research (Riskesdas) 2013, 65%among 240.000 new cases of advanced cancer per year areseeking medical assistance where the curative treatment nolonger effective. Palliative care is an approach that is no longereradicates the disease, in contrary, it's an approach to increasethe quality of patient's life until the end of life period (death).Objective: This research aimed to evaluate the correlation oflength of stay (LOS) and direct cost of terminal cancer patientswho get palliative intervention or not receive palliativeintervention.Methods: This study used a retrospective cohort design whichinvolving 120 patients (29 cases and 91 controls) that takenfrom medical records of hospitalized patient in CiptoMangunkusumo Hospital (RSCM). The inclusion criteria consistof patient has been diagnosed as terminal cancer disease, aged≥18 years, and palliative care consultation.Results: The results of the bivariate analysis using Mann-Whitney test showed a significant correlation between directcost and palliative intervention (p-value = 0,041) and nosignificant relationship between length of stay (LOS) andpalliative intervention (p-value = 0,873).Conclusion: There was a significant relationship betweenpalliative intervention and room expenditure rates, visites,actions, and drugs. Therefore, it's necessary to further researchto explore variables that influence LOS and direct cost inpalliative care intervention patients with terminal cancer.

Abstract

Background: Nurses are the biggest health personnel in thehospital. Despite their important role in providing services topatients, their job satisfaction is still low. Leadership is one of thefactors affecting the nurses’ job satisfaction. Objective: The research was conducted to measure therelationship between head nurse leadership style and nurses’job satisfaction in hospital wards. Methods: This study was an analytic research using crosssectionaldesign. This research was conducted in the DistrictHospital on May 2015. Subjects were 156 nurses who work inthe general and special wards. Questionnaire was divided into 3sections, demography variable, job satisfaction using Mueller-McCloskey Satisfaction Scale and leadership style using“Multiple Leadership Questionnaire 5x”. Results: Marital status, educational level, and type of wardhave a significant relationship with job satisfaction with p valueof 0,033, 0,010, and 0,007 respectively. Transformationalleadership style also has a significant relationship to jobsatisfaction, providing four times greater job satisfactioncompared to passive avoidant style (p = 0,01). Conclusion: There is a relationship between head nurse’leadership style and nurse job satisfaction. Hospitalmanagement has to give serious attention on nursingleadership to enhance attending nurses’ job satisfaction.
