Abstract

Introduction: Fatty liver is a broad term for a condition in which there is an increase triglyceride level in hepatocyte. In Indonesia there is not much research about this condition. Fatty liver is a major health problem in West countries, and is slowly increasing in Asian countries. For many individual with fatty liver, have no specific complaint or can have no symptoms at all. The aim of this study was to analyzed the relationship between fatty liver images on abdominal CT scans with dyslipidemia. Method: This study is an observational analytic study with a case – control type of research with the aim of knowing the relationship between CT scan images of fatty liver and dsylipidemia. The data in this study were all medical records who went to the Radiology Department for abdominal CT Scan at the Haji Adam Malik General Hospital for a period of one year from January 1st, 2020 to December 31st 2020. Results: The subjects on the abdominal CT scan results with fatty liver and high triglyceride is 5 peoples out of 12 (41.7%). The test results obtained p value = 0.,688 (p>0,05), in other words there is no significant relationship between fatty liver and high triglyceride level. Conclusion: No significant relationship was found between the incidence of fatty liver and the incidence of dyslipidemia.

Abstract

Introduction: Parkinson disease is a wide-spectrum disease that can be accompanied by motor and non-motor symptoms. Non-motor symtoms can occurred before the existance of motoric symptoms until the terimal stage of the disease, where cognitive disturbance is one of the non-motor symptoms that can decrease patient’s quality of life and increase patient’s disability. Therefore, early detection of the cognitive function is important for patients with parkinson disease. The aim of this study was to find the association between the severity of Parkinson’s disease and cognitive disturbance using the Montreal Cognitive Assesment Indonesian version (MoCA-Ina) Method: This study used cross-sectional design. The research subject was a parkinson disease patients’ who went to Neurology Clinic at Haji Adam Malik General Hospital Medan and network hospital who met the inclusion and exclusion criteria of the study. The number of sample was 39 subjects. To determine the relationship between the severity of parkinson disease and cognitive function, the Gamma test was used. Results: There was a significant correlation between the severity of Parkinson's disease and cognitive function (p = 0.001, r = -0.858). There was a very strong correlation between the severity of Parkinson's disease and cognitive function, and the negative correlation means the higher the severity of disease, the lower the cognitive function. From this study, the most correlated domains were delayed memory, naming (r = 0.962), orientation (r = -0.944), visuospatial (r = -0.929), abstraction (r = -0.874), language (r = -0.674), attention (r = -0.592). Delayed memory could not be statistically analyzed because delayed memory were all impaired in all subjects. Conclusion: There was a correlation between the severity of Parkinson's disease and cognitive function with a very strong correlation strength. The cognitive function domains that correlate strongly with Parkinson's severity were delayed memory, naming, orientation, visuospatial and abstraction.

Abstract

Introduction: The prevalence of heart failure is known to increase day by day, as well as the incidence of rehospitalization and death. One way that can be done is to optimize health status and perform cardiac rehabilitation. Aerobic exercise is recommended for patients with heart failure. Aerobic exercise has been shown to increase functional capacity in patient with heart failure. The aim of this study is to assess the relationship between aerobic exercise with functional capacity improvement in patient with heart failure in Haji Adam Malik General Hospital, Medan. Method: This study was conducted using prospective cohort method on 35 patients with heart failure in a cardiac rehabilitation program at Haji Adam Malik General Hospital, Medan. Functional capacity and 6-Minute Walk Test (6MWT) data were collected before and after aerobic exercise. Univariate and bivariate data analysis were performed using SPSS. Results: 35 samples were included in this study, with an average age of 54,57±1,28 years and dominated by male. The result showed an increase in distance after aerobic exercise program compared to the initial examination (373,6±133,64 vs 192,57±64,68) with p-value <0,001. There was an increase in functional capacity from 3,55±1,12 MET to 6,6±2,44 MET after aerobic exercise (P<0,001). Conclusion: There is a significant relationship between aerobic exercise and functional capacity improvement in patients with heart failure at Haji Adam Malik General Hospital, Medan.

Abstract

Introduction: TID ratio derived from ratio of LV volume in stress and rest phase of MPS. It is said that this phenomenon has been a useful marker of severe CAD, that can be present with atypical angina. This occurs due to global myocardial hypoperfusion caused by severe and extensive CAD. Method: This study was an analytic observational study with a retrospective cohort design in CCS patients who underwent Myocardial Perfusion SPECT at Haji Adam Malik General Hospital from January 2022 to April 2023.  All participants underwent MPS with Tc99m sestamibi with pharmacology Adenosine stress test and coronary angiography. The MPS could be performed before or after coronary angiography without any revascularization procedure between the two examination preocedures. An unpaired t-test analysis was performed to find the mean difference in TID values in the mild CAD and moderate-severe CAD groups. Results: The study subjects totaled 93 people with an average age of 55.87±7.44. It was found that the TID value was significantly different between the two groups of mild and moderate-severe CAD based on Syntax score, 0.906±0.13 vs 1.03±0.11 in the mild vs moderate-severe CAD group (p<0.001). Bivariate analysis showed that in this study, the ratio of TID was only associate with LV ESV and LV EDV in stress phase of MPS (p = 0.001). Conclusion: There is a relationship between TID and CAD severity based on Syntax score with higher TID values in patients with moderate-severe CAD compared to patients with mild CAD. TID ratio was only depends on severity od CAD and directly proportional to the volume of LV in the stress phase, suitable to the theory that said TID ratio comes from ratio of LV volume in stress and rest phase of MPS.

Abstract

Introduction: Acute coronary syndrome results from rupture or erosion of an atherosclerotic plaque. D-dimer is a direct marker of fibronolysis events and an indirect marker of hypercoagulable state associated with atherosclerotic plaque formation and acute coronary syndrome. The purpose of this study was to determine the relationship between D-dimer levels and the severity of coronary lesions in ST Elevation Myocardial Infarction patients at H. Adam Malik Hospital Medan. Method: This study was conducted with a cross-sectional method on 72 ST Elevation Myocardial Infarction patients at H. Adam Malik Medan Hospital who underwent coronary angiography from December 2022 to May 2023. Definition of ST Elevation Myocardial Infarction followed the definition of Universal Definition of Myocardial Infarction and the severity of coronary lesions was calculated based on syntax score. Data were analyzed univariately and bivariately using SPSS to assess the association of D-dimer levels with syntax score with p<0.05 considered statistically significant. Results: The total study subjects were 72 people with an average age of 56,5410,84years. Most of the study samples were male 56 people (77.8%). The median D-dimer level was 605 and mean syntax score 22.027 + 14.61. There is a significant relationship between D-dimer levels and the severity of coronary lesions based on the syntax score (p: <0.001). Conclusion: There is a significant relationship between D-dimer and severity of coronary lesions in patients with acute myocardial infarction with ST segment elevation at H. Adam Malik General Hospital, Medan.
