Abstract

Background: About 80-90% of patients LBP stated that they did not do any attempts to treat his illness. Spondylolisthesis (spon-dee-lo-lis-thee-sis) is a condition where one of the spinal vertebrae to shift forward or backward with the next vertebra. The disruption that can be caused due to this condition include a painful tap on regio lumbar muscle spasm, occurs a decrease in muscle strength, limitation of motion, can also happen pain in the limbs. The purpose of this research is to know the influence of the use of the Lumbo Sacral Orthosis semirigid type to the strength of  Trunk muscles in case of Low Back Pain (LBP) e. c Spondylolisthesis.Subjects and method:  this type of research was quasi-experimental design pre-test and post-test design. Location of the study Area General Hospital in Yogyakarta, Gadjah Mada University Academic Hospital, Dr. Soeradji Tirtonegoro Hospital, Tugurejo Semarang Hospital. The samples in this research are  35 patients with purposive sampling.Results :  there  was the influence of the use of the Semirigid Type LSO against muscle flexion trunk before and after treatment (Z =-5.099 with p value = 0.000),  trunk rotation muscle (Z =-4.600 with p value = 0.000), trunk extension muscle strength (Z =-4.472 with p value = 0.000).Conclusion:  As for the results of this research are the greatest influence has decreased muscle strength of the trunk after wearing the LSO within one month is on the muscle group Flexor (0.75), then extensor muscles (0.58) and rotator trunk muscles (0.65).Keywords: Lumbo Sacral Orthosis semirigid type, The strength of Trunk muscles, Low Back Pain (LBP) e. c Spondylolisthesis.
The Impact of Reuse Dialyzer Application on Routine Hemodialysis Patients Adequacy and Its Correlation with Cost Efficiency in Type D Hospital Rulita Ayu Kusuma Ningtiyas

Abstract

Background: Hemodialysis (HD) is one of the high cost service unit in the hospital which it implemented the cost effciency policy by the application of reuse dialyzer. According to claim recieved from Jaminan Kesehatan Nasional (JKN) system, the HD procedure for type D hospitals received the smallest claim payment if it compared to type A, B and C hospitals. This policy will be measured its effect on the effectiveness in the form of Ureum Reduction Ratio (URR) and Kt/V values and it associated with cost efficiency in type D hospital settings. Objectives The aims of this study were to 1.) Measure the corellation between the policy of using reuse dialyzer and the HD adequacy value and 2.) Evaluate the cost efficiency policy implemented by type D hospital HD unit in the form of using reuse dialyzer. Methods: This study was conducted with a cross-sectional design study from URR and Kt/V data on routine HD patients at 4 of type D hospital in Yogyakarta and Cantral Java in period Januari-March 2021. HD adequacy values were compared between the groups using the new and reuse dialyser. Cost efficiency was calculated based on the cost difference between cost of the HD procedure using a reuse dialyser and the projected cost if it is carried out with a single-use dialyzer.Result and Discussion: HD adequacy data were obtained from 112 subjects consisting of 22 with a new dialyzer, 32 with a reused (R) dialyzer first and R2, 27 with R3-R4 and 31 with R5-R7.The analysis of the effect of using reuse dializer on URR and Kt/V in the group with the new and reuse dialyser was obtained p = 0,90 and 0,91. The HD adequacy value in using reuse dialyser was not significantly different from using the new dialyser. The result of the cost efficiency analysis was 20.55% with using reuse dialyzer when it is associated with a source of income for HD services in type D hospitals.Conclusion: The application of reuse dialyzer is proved that it did not affect HD adequacy achievement when it compared to the use of a new dialyzer, and saved costs on HD services in type D hospitals. Keywords: Hemodialysis, reuse dialyzer, HD adequacy, efficiency

Abstract

 Background: Sleman Hospital is a referral hospital for Covid-19 patients. The covid-19 pandemic appeared in the current budget year, the budget for handling COVID-19 is not yet in the 2020 Budget Determination Document (DPA). The duties as Commitment Making Officer (PPK) and procurement officer at Sleman Hospital are additional tasks outside of the main duties and functions. . The implementation of the COVID-19 procurement activities at the Sleman Hospital adjusts the translation of the existing funds as determined through the Director's Decree.Objective: To evaluate the implementation of the procurement of goods/services in an emergency period to fulfill the service needs of COVID-19 patients at the Sleman Hospital.Method ; This study uses a qualitative method. Evaluating the implementation of procurement of goods/services during the COVID-19 emergency period in the period April-December 2020. The research subjects are Commitment Making Officers (PPK), Budget User Authorities (KPA), Procurement Officers, Technical Implementation Officers (PPTK), and Goods Users .Results and Discussion: Sleman Hospital in carrying out goods/services procurement activities during the COVID-19 emergency through 3 stages, namely planning, implementation and completion of work. The implementation of the procurement of goods/services is carried out by the Commitment Making Officer (PPK) and the Procurement Officer. Procurement implementation uses the direct appointment procurement method. direct procurement, e-purchasing through LKPP e-catalog and fast tender/auction. Contract signing is carried out at the stage of execution of work. The use of general procurement procedures is carried out prioritizing the principle of procurement of goods/services in an emergency, namely the speed and accuracy of emergency handling. Goods/services procurement activities that use pure APBD funding sources have good achievements, namely above 70%.Conclusion: The implementation of procurement activities for goods/services at RSUD Sleman is guided by special emergency procurement regulations and general procurement regulations regarding government procurement of goods/services  sourced from pure APBD funds.

Abstract

Background: High-risk pregnancy is a severe threat to the health and life of pregnant women and babies, considering that this can increase the risk of complications during pregnancy, childbirth and puerperium. About 20 million women worldwide have high-risk pregnancies, and it is known that more than 800 die each day. Pregnant women have potential risks during pregnancy, childbirth and the puerperium. The mother's lack of knowledge about pregnancy health problems and health service providers can increase inappropriate handling risk by health workers. So, women should understand the concept of high-risk pregnancies to reduce these risks. Research Objectives: To determine the childbirth experiences in mothers at high risk in the delivery process at UGM Academic Hospital. Method: Qualitative Research with Descriptive-Narrative Design. Results: The mothers knew the risks of pregnancy, but they were less aware of the risks of other pregnancies. Mothers’ experience in choosing a place of delivery was influenced by personal experiences, family, friends, or other people. The availability of adequate hospital facilities, service flow, and ownership of insurance coverage were also other factors in choosing a maternity hospital. Conclusion: Mothers are aware of the risks of pregnancy, but they are less aware of other pregnancy risks. Suggestion: Mothers should be aware of the risks of pregnancy and need to learn more about other pregnancy risks. It is hoped that they will be better prepared if a high-risk pregnancy occurs.

Abstract

Purwantoro is a very strategic area, located in the middle of its surrounding subdistricts, and is a transit point to get to the surrounding sub-districts. In Wonogiri, there are nine general hospitals, but none are located in Purwantoro. The ratio of hospital beds per 1,000 population in Wonogiri is still 0.96. Indonesia is a country with the largest Muslim population (87.2%). The Muslim population of Wonogiri Regency reached 1,108,696 (97.65%), and specifically in Purwantoro, 99.06% are Muslims. So that awareness and need for sharia-based health services increased. This research is a feasibility study. Primary and secondary data obtained are collected and processed, then analyzed by the situation, demand, and needs, as well as financial analysis. Investment evaluation using the Investible Surplus Method (ISM). Situation analysis, both external and internal aspects, supports this project. Demand and needs analysis also have been fulfilled, but necessary to pay attention to the needs of male nurses. The total cost of the entire project excluding land purchases is Rp 42.231.739.375,00 through two-step constructions with an ISR of 16,81% in the ninth year.  The establishment of a sharia-based class D general hospital in Purwantoro Wonogiri is feasible to be established with a capacity of 60 beds at an area of 4.252 m2, an investment value of Rp. 42.231.739.375,00 through two-step constructions with ISR of 16,81 % for nine years.
