Abstract

Introduction: Flat Shoes are shoes that female students often wear. However, the flat model does not provide adequate support for the feet, which increases the risk of plantar fasciitis. Research on plantar fasciitis is more often associated with high heels. In contrast, many women now wear flat shoes daily, most of which are worn by medical students.  Purpose: This research aims to assess the relationship between the duration of flat shoe use and Body Mass Index with the occurrence of plantar fasciitis symptoms in female students. Methods: The design of this study is an observational analytic with a cross-sectional approach, using the consecutive non-random sampling method, the sample size consisted of 162 female respondents from the Faculty of Economics and Business at Trisakti University. Data was collected using a questionnaire to determine the duration of shoe use, the Pain Scale for Plantar Fasciitis questionnaire for plantar fasciitis symptoms, also weight and height measurements to obtain the Body Mass Index value. Data analysis using Chi-Square with a significance level of p<0.05.  Results: The prevalence of plantar fasciitis symptoms was 18.5%.  There was relationship between BMI and the duration of flat shoe use with plantar fasciitis symptoms (p=0.026 and p<0.001). Conclusion: This study concludes a significant relationship between the duration of wearing flat shoes and nutritional status with the symptoms of plantar fasciitis. This research is useful for providing information on risk factors for plantar fasciitis.

Abstract

Introduction: Low back pain (LBP) is a common health issueexperienced by nearly 80% of people throughout their lives. Thiscondition often recurs and can become chronic low back pain (CLBP) ifit lasts for more than 3 months, with a prevalence of approximately 23%and causing disability in about 11-12% of the population. CLBP is oftenassociated with depression, which can worsen pain intensity, function,and patient recovery. Purpose: This study aims to analyze therelationship between depression and CLBP, as well as the factors thatinfluence it. Methods: The study uses a cross-sectional analyticalobservation method with consecutive sampling of 170 respondents. Datawere collected through interviews and questionnaires, including painintensity measurement using the Numeric Rating Scale, depression levelsusing the Beck Depression Inventory-II, and disability levels using theOswestry Disability Index. Data collection was conducted at SumberWaras Hospital and dr. Mintohardjo Naval Hospital. Results: The studyresults show the highest prevalence of LBP in individuals over 65 yearsold (47.65%), and the majority of respondents were female (70.59%).There is a significant relationship between pain intensity and depression(p < 0.001) and between disability and depression (p < 0.001). Painduration was not significantly associated with depression (p = 0.211).Conclusion: In conclusion, pain intensity and disability level aresignificantly associated with the incidence of depression in CLBPpatients.

Abstract

Introduction: Subcutaneous endometriosis is a rare condition with endometrial tissue localized outside the pelvis and often mimics general endometriosis. The novelty of this study is an in-depth understanding of the specific risk factors associated with subcutaneous endometriosis after cesarean section and how optimal therapeutic approaches can provide better outcomes in patients with a history of endometriosis. This study aims to explore the clinical presentation, outcomes, and managerial approaches to this condition, as well as provide evidencebased recommendations for its management. Case Illustration: This study involved seven patients at Cipto Mangunkusumo Hospital (January 2022–December 2024). Data collected included demographics, age, BMI, initial symptoms, time of symptom onset, history of cesarean section, history of endometriosis, complications, nodule size, therapeutic approaches, and clinical outcomes. Discussion: Of the seven patients, three underwent one cesarean section, two underwent two operations, and two underwent more than two cesarean sections. A history of diffuse adenomyosis was found in three patients. Subcutaneous nodules located mainly in the cesarean scar, had highly variable sizes and caused significant pain (VAS 5–6). Dienogest therapy was given to three patients and significantly reduced pain. Conclusion:Subcutaneous endometriosis is often associated with a history of abdominal surgery, especially cesarean section. Extrapelvic location can be mistaken for keloid or fibroma, which hinders timely diagnosis. The novelty of this study lies in emphasizing the importance of multidisciplinary collaboration in the management of subcutaneous endometriosis, especially involving plastic surgeons to improve aesthetic results and prevent recurrence. Treatment involving hormonal therapy or surgery, based on the size and location of the nodule, is essential for optimal results.

Abstract

Introduction: Ramsay-Hunt syndrome is a complication of the varicellazoster virus that causes inflammation of the geniculate ganglion cranialnerve VII. This disease is a rare case in children. This case report aims tohighlight the importance of diagnosis, early examination, andappropriate therapy to prevent complications and poor prognosis. CaseIllustration: A 9 year-old girl presented with clear fluid-filled blisters inher left ear for four days before going to the hospital. Two days after thelesions came out, she complained of her face feeling tilted to the right,difficulty close her left eye, and the corner of her mouth being moredrawn towards the normal side. The left auricular helix fossa regionshowed a rash of multiple vesicles in groups containing clear fluid withan erythematous skin base, distributed unilaterally according to thedermatome. The patient was diagnosed with Ramsay-Hunt syndrome.She was treated with a combination of oral acyclovir andmethylprednisolone injections. Discussion: Antiviral therapy within 24–72 hours after the onset of the rash is very effective for healing of the rashand reducing the duration of pain. Corticosteroid as an antiinflammatory can reduce peripheral nerve damage and edema.Combination of corticosteroid with acyclovir for 7 days in herpes zosteraccompanied by facial paralysis is very effective in significantly reducingacute pain and reducing the risk of complications. Conclusion:The exactdiagnosis and combination of antiviral with corticosteroid therapy inchildren can be beneficial in reducing the duration of lesions and pain aswell as the risk of complications.

Abstract

Introduction: An unhealthy lifestyle among medical students maycontribute to inflammation, reduced vascular elasticity, andincreased blood pressure. However, cardiovascular risk in theseyoung adults remains less explored compared to hypertensivepatients. Purpose: This study compares inflammatory markers andvascular elasticity in prehypertensive and normotensive medicalstudents. Methods: A case-control study utilizing simple randomsampling was conducted in the medical student population, included54 prehypertensive or normotensive subjects with no history ofhematological disorders, autoimmune diseases, cancer, diabetesmellitus, or cardiovascular medication use. Vascular elasticity,blood pressure, and inflammatory markers were assessed using anaccelerated photoplethysmograph, a digital sphygmomanometer,and a hematology analyzer. Results: A total of 25 subjects (46.3 %)had suboptimal vascular elasticity, 30 (55.6%) had a highneutrophil-to-lymphocyte ratio (NLR), and 40 (74.1%) exhibited alow-risk platelet-to-lymphocyte ratio (PLR). The Chi-square testrevealed a significant difference in NLR between prehypertensiveand normotensive students (p = 0.000; OR = 7.0; CI = 2.3–20.7),while no significant difference was observed in PLR (p = 0.525).Vascular elasticity also differed significantly between groups (p =0.000; OR = 5.2; CI = 2.0-13.2). Conclusion: Prehypertensivesubjects had higher NLR, and those with suboptimal vascularelasticity had higher prehypertension risk. These findings highlightthe need for cardiovascular risk assessment in young adults.

Abstract

Introduction: Mild Cognitive Impairment (MCI) is a condition characterized by cognitive decline that does not interfere with daily activities but increases the risk of progressing to Alzheimer's Dementia (AD). Early detection of MCI progression to AD is crucial for early intervention. Purpose: The purpose of this meta-analysis to evaluate the performance of Convolutional Neural Networks (CNNs), an artificial intelligence used to analyze complex data such as images, in predicting the conversion of MCI to AD using MRI data. Methods: A meta-analysis was conducted following PRISMA guidelines, utilizing articles from PubMed and Wiley Online Library. Inclusion criteria focused on studies that used CNN in conjunction with MRI to diagnose MCI. A total of 39 articles with 40 comparative studies were analyzed. Results: The CNN showed an average accuracy of 0.7910 (p < 0.0001), sensitivity of 0.7698, specificity of 0.8467, and an Area Under the Curve (AUC) of 0.8063. High heterogeneity (I² = 90.90%) indicated significant variation across studies. Sub meta-analysis revealed consistent performance despite the heterogeneity. Conclusion: CNN is potentially useful for predicting the progression of MCI to AD. Further research is needed to address heterogeneity, improve algorithms, expand datasets, and include more diverse populations.

Abstract

Introduction: Green nail syndrome is a common bacterial infection of the nails and is characterized by green discolouration of the nail plate caused by Pseudomonas aeruginosa. It frequently occurs in people who are exposed to constant water and due to mechanical trauma, especially in elderly patients. Case Illustration: We reported 2 cases treated with a combination therapy of oral ciprofloxacin, 1% acetic acid dressing, gentamicin ointment and showed a significant clinical improvement. There are two cases: a 67-year-old man and a 64-year-old woman complained of black-greenish discolouration on his right big toenail, which has been accompanied by changes in the shape of the nail for 3 months. Both patients have a history of frequent and prolonged contact with water—dermoscopy results in greenish-black discolouration in the nail plate.  The culture results of both patients, Pseudomonas aeruginosa was found. Combination treatment showed significant clinical improvement in both patients. Discussion: Dressing of 1% acetic acid was useful as an adjuvant to improve the ointment penetration, inhibiting the formation of Pseudomonas aeruginosa biofilms. Gentamicin and ciprofloxacin are effective against aerobic gram-negative bacterial infections, such as Pseudomonas aeruginosa. Conclusion: The use of ciprofloxacin is recommended, especially in elderly patients. Combination therapy with oral ciprofloxacin, 1% acetic acid dressing, and gentamicin ointment is an effective option for patients with green nail syndrome

Abstract

Introduction: Academic Health Systems (AHS) integrate healthcare, education, and research to enhance medical training and patient care. However, initiating AHS in institutions without an existing framework presents challenges in aligning stakeholder expectations and organizational structures. Purpose: This study aims to initiate and establish an appropriate AHS model for Ukrida AHS through Project Management (PM) principles and highlight the importance of PM principles in healthcare organizations. Methods: A qualitative research design was employed using semi-structured interviews with six key stakeholders. Thematic analysis identified recurring patterns in stakeholder perceptions, expectations, and challenges regarding AHS implementation. Data triangulation through a Focus Group Discussion (FGD) refined the findings. Results: The findings are categorized into three phases: Inquiry, which identified three key themes—stakeholder perceptions of their organization, understanding of AHS, and perceived benefits and challenges; Action, which translated these insights into concrete steps, including goal-setting, developing a Gantt chart, and an AHS model; and Reflection, which assessed the research process and emphasizing the benefits of using PM principles in healthcare organizations. Conclusion: Integrating PM with PAR facilitated AHS initiation at Ukrida, revealing benefits, challenges, and key priorities: leadership alignment, financial sustainability, faculty development, stakeholder collaboration, and data systems to ensure long-term success and scalability.
