Abstract

There has been emphasis on de-institutionalization of psychiatric care with a focus on collaborative care model (CCM) as a bedrock improving quality of life. This study measures the association of CCM and quality of life in schizophrenia. Schizophrenia receiving CCM and usual care (UC), each group was 41 and chosen by systematic random sampling follow this cohort retrospective study in Denpasar and Badung, Bali Province on April-November 2020. The outcomes are QoL (assessed by WHOQOL-BREF), socio-demographic, clinical characteristic, family support using Friedman questionnaire, stressor using PSS, coping using BRIEF-COPE, and medical adherence using MARS. Abstract

Chronic kidney disease is persistent abnormality or damage in kidney structure or function for more than 3 months. Prevalence of CKD is around 10 – 14% in general population and affects around more than 800 million individuals worldwide. Patients with CKD have higher risk of cardiovascular disease and synergistic pathological effects can speed up worsening of diseases. Radiology examination is one of the test that can be used to evaluate kidney and heart structural damage. The aim of this paper is to report chest rontgen findings in a man aged 54 years old with CKD with HF in 3 serials imaging result. Patient admitted to hospital caused by shortness of breath. Patient has history of hypertension, diabetes mellitus type 2, CKD stage V, and congestive heart failure. From physical examination, oxygen saturation was 97%, blood pressure was 154/115 mmHg, and extremity edema was found. Chest rontgen impression was bronchitis, dextra pleural effusion, and normal cardiac size. He got oxygen, Furosemide, Nicardipine, Aspirin, Irbesartan, Amlodipine, Calcium carbonate, and hemodialysis as therapies. After be hospitalized 2 days, patient’s condition was better, and therapy was continued outpatient. Chest rontgen examination result a year ago showed pulmonary edema, bilateral pleural effusion, and cardiomegaly with shortness of breath as major symptom. Chest rontgen examination result 6 months ago showed bronchopneumonia and cardiomegaly with fever, cough, nausea, and loss of appetite. In conclusion, most of CKD patients particularly in late stage have cardiovascular disease and radiology examination is important to detect and evaluate structural cardiac abnormality in CKD patients. Abstract

Early diagnosis of cardiac ischemia is crucial for effective management of acute myocardial infarction. The BASEL (Better Analysis of ST-segment Elevations and Depressions in a 12-leads-ECG) score has been shown to provide additional diagnostic value to the established electrocardiographic (ECG) criteria. This study aimed to evaluate the use of BASEL score to determine the severity of coronary lesions in patients with non-ST segment elevation acute coronary syndrome. This study used a cross-sectional approach and was conducted from January 2021 to January 2022. From a total of 90 subjects, more than three-quarters were male, while the mean age was 60.3 years. The median BASEL score was 2.3 (1–4.2). GRACE 2.0 score had a mean of 97.3±26. The SYNTAX I score had a mean of 25±15.6, the SYNTAX II – PCI median score was 34.5 (25.9-42.4), and the SYNTAX II-CABG mean score was 23.4±11.9. Abstract

The Covid-19 pandemic had a big impact on the world over the past two years., and it is estimated that 1 in 10 people have been infected. The development of prevention and treatment systems can reduce mortality, but a new problem has emerged in the group of survivors. New clinical complaints, Getting COVID-19 again or having ongoing symptoms of COVID-19 for four weeks after the first infection, can affect a wide variety of organ functions. The long-term effects of having COVID-19, like feeling tired, having trouble breathing, chest pain, difficulty thinking, and joint pain, can make life worse. The study was conducted using a qualitative descriptive method using an online questionnaire. The results showed that some residual symptoms that respondents still felt after four weeks since being infected with COVID-19, such as fatigue easily (80%), joint pain (63.3%), insomnia (56.7%), depressive symptoms (53.3%), shortness of breath (50%), impaired concentration and memory (50%), livelihood disorders (43.3%), cough (40%), respiratory disorders (36.7%), weight loss and appetite (33.3%), fever (26.7%). Overall, it was found that 20% of COVID-19 survivors experienced daily activity disruptions assessed using the Modifications of Barthel Index and EQ-5D-5L questionnaires.

Abstract

About 10 million people in Indonesia have been impacted by heart failure (HF), which is a complex clinical syndrome caused by insufficient cardiac output that is unable to meet the body's metabolic needs. The most frequent kind of HF, heart failure with preserved ejection fraction (HFpEF), has the fewest available treatments. Diuretics and SGLT-2 Inhibitors are the first-line treatments for HFpEF. This paper aim to determine the mechanism of sodium-glucose cotransporter-2 (SGLT-2) Inhibitors in HFpEF and the newest recommendation for it, so that it might be utilized as a reference for management in the future. This is a literature review study which conducting by gathering numerous books and journal articles which relevant with the topic. The mechanisms of SGLT-2 Inhibitors in HFpEF are not well understood, but some theories suggest that these medications affect preload, afterload, cardiac remodeling, blood pressure, arterial stiffness, and ketone body formation. They can also stimulate erythropoiesis, inhibit the myocardial Na+/H+ exchanger (NHE), and have anti-oxidant and anti-inflammatory effects, so they are no longer just seen as anti-diabetic medications. Recently, Perhimpunan Dokter Spesialis Kardiovaskular Indonesia (PERKI) published a Heart Failure guidelines and classify SGLT-2 Inhibitors as a class IB treatment for HFpEF.

Abstract

Being a member of the Tim Bantuan Medis (TBM) is required not only to have excellent health but also to have optimal physical fitness, bearing in mind that Indonesia is a disaster-prone area where disasters can occur anytime and anywhere. The research was conducted on members of the TBM of Fakultas Kedokteran dan Ilmu Kesehatan (FKIK) Warmadewa, aiming to compare the step exercise (SE) method to the resting pulse rate and recovery pulse rate in TBM students. The research was conducted at the Physiology Laboratory of FKIK Warmadewa, with ethical clearance Number: 323/Unwar/FKIK/EC-KEPK/IV/2023. This study used 71 samples of TBM students who were divided into 3 groups using a simple random method. P1 group did SE at a frequency of 100 x/minute for 216 seconds, group P2 did SE at a frequency of 110 x/minute for 196 seconds, group P3 did SE at a frequency of 120 x/minute for 180 seconds, exercises were carried out 3 times per week, during 4 weeks. All subjects were tested for cardiorespiratory fitness with the Harvard Step Test and compared the pre and post exercise results. Data analysis was performed using the paired t test method to see a comparison of the step exercise method on resting pulse and recovery pulse rate. Abstract

Introduction: Old age is a period that indicates a process of physical, mental, and social deterioration over time. There are several things that concern health in old age, one of which is functional status which is a major health problem in the elderly. A person's ability to perform daily activities can be assessed by their functional capacity. Physical functional capacity can be measured using the six-minute walk test (6MWT). There have been many studies that discuss the 6MWT, but none have specifically looked for the relationship between physical functional capacity and sleep quality in the elderly. Purpose: This study was conducted with the aim of knowing the relationship between physical functional capacity and sleep quality in the elderly at Gotong Royong Hospital. Methods: This study used an analytic observational research design with a cross sectional approach. Data were collected from demographic questionnaires, Pittsburgh Sleep Quality Index (PSQI), and direct 6MWT measurements. Data analysis in this study used univariate and bivariate analysis. Results: In this study, the results obtained p = 0.000 (p < 0.05) indicate that there is a significant relationship between physical functional capacity and the quality of sleep of the elderly at Gotong Royong Hospital. Conclusion: There is a relationship between physical functional capacity and elderly sleep quality at Gotong Royong Surabaya Hospital.
