Abstract

The problem at RSI Ibnu Sina Padang in 2023 is that the search for medical records is always done manually with the time needed for 1 medical record ± 10 minutes, this is because the medical records are not specified or scattered. The purpose of this study is to find out the design of an outpatient medical record search application at RSI Ibnu Sina Padang in 2023. By adopting the type of research, namely Research & Development (R&D) with the prototype development method. The research was conducted from January to July 2023. The number of informants was 5 people, namely 5 runner officers. Conduct data analysis of potentials and problems, collect data, design products, validate designs, improve designs, and test products. The results showed that the process flow for filling in components, especially in the form of searching was done manually which included outpatient registration proof sheets. Application design can be fully implemented. The test results show that the existing components are in accordance with the hospital format and the application of the search process is faster and more efficient than manual search. Based on the results of the study, it was concluded that the design of the outpatient medical record storage information system was in accordance with the hospital format. It is hoped that this research application design can be used in hospitals and this research can be continued by other researchers.

Abstract

One supporting element for medical record processing is the prompt return of files belonging to patients who have concluded their inpatient healthcare services. The established standard for the return of these medical records is within a maximum of 48 hours. This research aims to ascertain the timeframe the return of hospitalized patient records at Andalas University Hospital in Padang. The investigation employed a quantitative approach utilizing descriptive methods. The research took place in the storage room at Andalas University Hospital, Padang. The investigation took place starting from May to September 2023. The population of this study was 302 files with a sample of 75 medical record files with data collection time from 20 to 25 September 2023. The research applied the Simple Random Sampling technique. Data gathering involved observation and data processing techniques including editing, coding, processing, and cleaning. The analysis focused on univariate analysis. Results from Andalas University Hospital revealed that 31 files (41.3%) returned accurate medical records, while 44 files (58.7%) were found to have incorrect medical records returned. This indicates that many medical record files were returned after the stipulated timeframe of >48 hours. As a conclusion from the study, it's imperative for the head of the medical records room to assess and supervise the return process of inpatient medical record files. By doing so, this issue can be mitigated and delays in Submitting the medical records of a hospitalized patient anew files can be minimized.

Abstract

Procurement is an activity carried out exist to provide goods that initially did no exist. The problem with drug procurement that researchers found at the Lubuk Basung Regional Hospital Pharmacy Installation was that there were vacancies in several drug items at distributors so that the drugs that were supposed to be given to patients ran out of stock. The study took place between June and July 2023. In-depth interviews and a document examination were used to acquire data. The results of this research are that the Lubuk Basung Regional Hospital Pharmacy Installation already has an SPO, but the latest SPO has not been issued, planning is carried out to avoid vacancies and excess drug stock which result in expired dates in the Pharmacy Installation. The drug procurement process uses two methods, namely the direct procurement method (e-purchasing & PBF) and the donation method (grants). accordance based on the findings of this inquiry, it can be inferred that the drug planning process there is an SPO and the activities carried out are in accordance with SPO issued. The drug procurement process has been carried out well using two methods, procurement is carried out every 3 months and once a month according to the drug consumption pattern of the previous period. It is recommended to the Hospital the latest SPO be issued immediately, to carry out regular evaluations on every order of goods so that there are no errors or mistakes in receiving the goods.

Abstract

The Padang City Environment Agency reported that the city generated an average of 643.76 tons of waste per day in 2021–2022. According to the head of RW 016, residents dispose of waste at the TPS due to limited available land. This study aims to assess the impact of education on 3R (Reduce, Reuse, Recycle) waste management in RW 016, Pagambiran Ampalu Nan XX Village, Lubuk Begalung District, Padang City, in 2023. A quantitative descriptive method was used with pretest and posttest assessments to measure knowledge levels. The paired sample t-test was employed for data analysis. The study involved 40 housewives as respondents, and data were collected through field observation and questionnaires. After data processing and analysis using frequency distribution tables, results showed a significant improvement in waste management knowledge after educational intervention. The mean score increased from 29.10 before education to 36.23 after, with a p-value of 0.000, indicating a statistically significant difference. These findings demonstrate that education positively influences 3R-based waste management behavior. To support sustainable waste practices, it is recommended that local government and village authorities provide adequate infrastructure such as segregated waste bins, recycling facilities, and environmentally friendly

Abstract

The admission and discharge summary is a medical record form that is used to record a summary of the course of the disease from the time the patient entered the hospital until the patient was discharged. The research used is quantitative research with a descriptive approach. Data collection was carried out at Lubuk Basung Regional Hospital from 27 May to 1 July 2023., and the sample consisted of 88 entry and exit summary forms from the medical records of patients suffering from hypertension. Research through observation with a checklist table. Computerization is used to process data and carry out univariate analysis. The results of the study showed that there were still incomplete filling out of entry and exit forms on patient identification items (40.9%), important patient reports (20.5%), patient permission (12.5%), good record keeping (15.9%) . ), and scribbled and clearly legible (17.05%). The results showed that the summary of admissions and discharges for hypertensive patients was still incomplete. In conclusion, this shows that the summary of admissions and discharges for hypertensive patients is still incomplete. It is recommended to carry out evaluations and reviews, as well as outreach and evaluation with doctors, nurses and other related personnel. It mainly deals with quantitative analysis of incoming and outgoing summaries.
