Abstract

Decree of the Minister of Health Number 377/MenKes/SK/III/2007 concerning the Professional Standards of Medical Recorders and Health Information states that one or the competencies that must be possessed by medical recorders is the classification and coding of diseases, health-related problems and medical actions. The purpose of this study was to determine how the influence of competence as a coder on the accuracy and precision of coding. This type of research uses qualitative research, namely research conducted with the aim of describing or analyzing the results of a state of research objectively. This research was conducted with a cross-sectional approach. In this study, the sampling technique used was Purposive sampling. With a total of 7 informants as coders at the Imelda Buruh Indonesia General Hospital. The coder staff already had a D-III education background in Recording and Health Information. The coder staff already had a D-III education background in Recording and Health Information. Medical resumes that were included in the category of incorrect diagnosis codes were 10 (31%) and the correct ones were 22 (69%). Based on the results of the study, all officers have a D-III education background in Health Recording and Information. The age of officers ranges from 21-30 years and the length of service ranges from 8 months to 6 years, and of the 7 informants, 3 respondents have good knowledge and 4 other respondents have sufficient knowledge

Abstract

Incompleteness in filling out medical resumes will greatly affect the assessment of Hospital accreditation. This study aims to determine the completeness of filling out medical resume forms based on the MIRM 15 SNARS Edition 1.1 assessment elements at RSU Imelda Buruh Indonesia Medan. This study uses quantitative and qualitative methods, with a population of all medical resume forms at RSU Imelda Buruh Indonesia Medan. A sample of 93 medical resumes was rounded up to 100 medical resume forms. Based on the results of the study, it was found that the completeness of the medical resume forms was 76 medical resume forms (70%) and 30 medical resume forms (30%) were incomplete. Based on the SNARS Edition 1.1 assessment score, the number of complete medical resume forms was 76% with a score of 5 (partially fulfilled) because the achievement range was 29% -79%. The highest completeness of the medical resume form at RSU Imelda Buruh Indonesia is found in the components of patient history, diagnostic examination, diagnosis, therapy procedures and actions, drugs given and drugs when the patient is discharged from the hospital, patient health condition (current status) and follow-up instructions as much as (100%) medical resume forms and the lowest completeness is found in physical examination (15%) indications for patient hospitalization (6%), comorbidities (12) and patient/family signatures as much as (24%) medical resume forms. The researcher's suggestion is that continuous socialization should be held to increase knowledge for medical personnel about filling out the completeness of the medical resume form according to SNARS at RSU Imelda Buruh Indonesia Medan

Abstract

Chronic kidney disease (CKD) has become a growing global public health priority associated with high morbidity, mortality, and healthcare costs. Patients with chronic disease often require a combination of several drugs. The administration of these drug combinations is generally expected to have a beneficial effect, but in some cases drug interactions are found that result in ineffective treatment and the emergence of unexpected events and cause quite adverse effects both on clinical and on treatment costs. This study aims to determine the incidence of Drug Relate Problems (DRPs) in CKD patients at the Inpatient Installation of Imelda Workers General Hospital Medan. This study was a non-experimental study with descriptive observation that data collection was done retrospectively and analysed with descriptive methods. The sampling technique used the Slovin formula with a total of 60 patients during the January - March 2024 period. Analysis of drug interaction data based on Drug Interaction Checker and Drug Interaction Fact and classification of Drug Relate Problems (DRPs) according to Cipolle et.2012. The results showed that there were 3 occurrences of DRPs in CKD patients, namely: Too Low Drug Dosage 30 patients (68.18%), Unnecessary Drug Therapy there were 10 patients (22.73%) and Patient Non-Compliance there were 4 patients (9.09%). Interaction Based on Severity Major 27 cases, Moderate 202 cases, Minor 46 cases. Interaction based on mechanism of action Pharmaceutics 1 case, Pharmacokinetics 9 cases, and Pharmacodynamics 23 cases

Abstract

Pelayanan kesehatan pada Jaminan Kesehatan Nasional (JKN) menyebutkan bahwa jaminan kesehatan adalah jaminan berupa perlindungan kesehatan agar peserta memperoleh manfaat pemeliharaan kesehatan dan perlindungan dalam memenuhi kebutuhan dasar kesehatan yang diberikan kepada setiap orang yang telah membayar iuran atau iurannya dibayar oleh pemerintah. Setiap orang yang memiliki kartu BPJS dan membayarkan iurannya, maka biaya kebutuhan dasar kesehatannya akan ditangguhkan oleh BPJS melalui proses klaim yang diajukan oleh pihak rumah sakit kepada BPJS untuk dipergunakan dalam memfasilitasi biaya penanganan medis (Menkes RI, 2013).

Abstract

Medical record is a file that contains records and documents regarding identity, examination, treatment, action, and other services that have been provided to patients. The purpose of this study was to determine the percentage of completeness of the informed consent form in pre cataract surgery patients. This type of research is quantitative research that only describes the state of the object, there is no intention to generalize the results. The time of this research was carried out in June-July 2021 at the Medan SMEC Eye Specialist Hospital in 2021. In conducting this study, the researchers took a sample of 60 files. Based on the results of research conducted at the SMEC Eye Specialist Hospital, Medan, the provision of medical information was carried out entirely by authorized officers. But in providing medical information, some officers did not explain in its entirety, such as only providing 9 medical information out of 11 medical information that should be given to patients. The conclusion of this study is that the completeness of filling out the informed consent form in the medical record file at the SMEC Eye Specialist Hospital has not reached 100% complete, according to the results of the study, namely the average percentage of completeness of the types of information in the complete informed consent form is 40 (67%). and incomplete 20 (33.3%). It is hoped that the Director will conduct training on the importance of completing the informed consent form for officers
