Abstract

Penelitian bertahap telah dilakukan'untuk mempelajari reaksi-reaksi immunitas selular yang terjadi selama infeksi Plasmodium vinckei petteri malaria pada mencit LACA. Fokus kajian pada penelitian tahap pertama ini adalah mempelajari profil perubahan jumlah sel fagosit mononuklear (mononuclear phagocytes) dalam sumsum tulang, darah tepi dan dalam limpa, selama infeksi malaria pada mencit yang diimunisasi dan yang tidak diimunisasi. Imunisasi dilakukan dengan penyuntikan secara intravena 108 parasit stadium darah yang dibebaskan dari eritrosit dengan larutan saponin dan difiksasi dengan larutan formalin. Kedua kelompok mencit, yang diimunisasi dan yang tidak diimunisasi diinfeksi dengan 103 eritrosit terinfeksi, pada hari ke 15 sesudah imunisasi. Jumlah sel fagosit berinti satu beserta sel prekursornya dievaluasi berturut-turut pada hari ke 0, 3, 6, 9 dan 12 sesudah infeksi. Hasil penelitian menunjukkan bahwa selama infeksi P. v. petteri, jumlah sel fagosit mononuklear meningkat di dalam sumsum tulang, kemudian di dalam darah tepi dan dalam limpa. Derajat peningkatan jumlah sel fagosit tersebut lebih tinggi pada mencit yang telah diimunisasi sebelumnya dibandingkan dengan pada mencit yang tidak diimunisasi. Tingginya peningkatan jumlah sel fagosit mononuklear pada mencit yang diimunisasi tersebut nampaknya berkaitan dengan keberhasilan sistem kekebalan tubuh hospes dalam menurunkan parasitemia. Peningkatan jumlah sel fagosit tersebut didahului dengan peningkatan jumlah sel prekusornya di dalam sumsum tulang, sehingga bisa diantisipasikan bahwa produksi dan proliferasi dari sel prekursor di dalam sumsum tulang juga meningkat dalam usaha memenuhi kebutuhan sel d.ewasa di dalam darah dan jaringan selama infeksi malaria.Key words: Plasmodium vinckei petteri -- malaria -- LACA mice -- mononuclear phagocyte -- cellular immunity

Abstract

Functional and structural abnormalities in diabetic kidney disease lead to intraglomerular hypertension and albuminuria. Systemic hypertension and genetically inherited defect on autoregulation in preglomerular arterioles will enhance kidney injury in those patients. ACE inhibitors could reduce albuminuria In patients with diabetic nephropathy and animal models. Hemodynamic improvement manifested by the reduction of albuminuria by ACE inhibitors is only seen in the experimental studies. However, recent studies have shown that ACE inhibitors, especially captopril, could give protective effects on kidneys in insulin-dependent diabetics with nephropathy. Captopril also reduces the risk of death, dialysis, and kidney transplantation in those patients.Key words: diabetes mellitus -- kidney disease -- ACE inhibitors -- albuminuria -- kidney preservation

Abstract

Principally, complications of post operative cataract surgery with pseudophakos implantation cover those complications due to the operation, and those due to the existence of pseudophakos within the eye ball. In Dr. Sardjito hospital, this operation has been done since 1988. There are many kinds of complication of this operation. This retrospective study is limited to those complications in the form of anterior uveitis, endophthalmitis, secondary glaucoma, and endothelial decompensation which occurred within 1992-1994 period.A study has been performed on 405 cases, consisting of 397 cases with posterior chamber pseudophakos implantation and 8 cases with posterior chamber pseudophakos implantation. In the same period, a study on 432 cases with extra capsular cataract extraction without pseudophakos implantation was performed. The complication in the group with pseudophakos implantation were: anterior uveitis (2.96%), secondary glaucoma (1,48%), endhopthalmitis (0,99%), endothelial decompensation (0.74%) and other complications such as iris prolapse and subluxation of pseudophakos. The complication in the group without pseudophakos implantation were anterior uveitis (0.23%), endothelial decompensation (0.23%), and prolapse of iris 0.46%. In this study, the complication in the form of endophthalmitis occurred 1 week after surgery in 3 cases, and 2 weeks after surgery in one case. One case was caused by fungi infection. The complication in the form of secondary glaucoma mostly related with the occurence of post anterior uveitis surgery. The anterior uveitis in post cataract surgery with pseudophakos implantation was considered to be related with the kind of lens implanted.Key words : post operative complications -- cataract surgery -- pseudophakos implantation -- anterior uveitis -- kind bf lens implanted

Abstract

A guideline for management of hyperbilirubinemia in the newborns based solely on total/indirect serum bilirubin levels is considered outmoded. An approach to the identification of newborns at risk of bilirubin toxicity due to the increase of unbound bilirubin is proposed. Unbound bilirubin can easily enter the brain and bind to the nerve cells.A peroxidase oxidation method using unbound bilirubin - analyzer (UB-analyzer) offers a rapid, accurate and reproducible measurement of unbound bilirubin. Phototherapy, either continuous or intermittent can be applied to term and preterm infants on the monitoring of unbound bilirubin levels.Temporary cessation of breast-feeding is not advised in healthy term newborns with hyperbilirubinemia. On the contrary, more frequent feeding is advocated. Caution and substitution should be taken in giving any drugs having a potent displacer on bilirubin-albumin binding, especially In premature infants.Key words : newborn infants --hyperbilirubinemia -- unbound bilirubin -- phototherapy -- displacing drugs
