RSU Haji Surabaya is a government-owned hospital in East Java province. Asset management performed by one of the installation of the Installation Management Information Systems (MIS-RS) and software (Software) is used Healthy Plus application which has been operating for the last 1 year. In managing the assets of RSU Haji there are several obstacles, namely: there are many outsiders who were not authorized to be in and out of the processing room information on data center space, loss of data, manipulation of data from unauthorized access, viruses, data theft, unauthorized access to the application. So that these constraints will not recur or become, the RSU Haji Surabaya need to conduct an audit to determine current conditions compared with conditions should be. The standard used is ISO 27002:2005 with the scope of clause 8 (eight), 9 (nine), 11 (eleven), and 12 (twelve). From the implementation of information systems audit, the resulting value of 1.75 Maturity Level are included in the initial category, which means much of the existing information system security on the SIM-RS Installation not in accordance with the ISO 27002 standard procedures. The study also produce recommendations for process improvement and information systems can be used to enhance the security of information on RSU Haji Surabaya. The development of computer network technology is currently growing exponentially along with the increasing users who make use of the computer network. In a computer network is addressing systems named IP address.Public IP address in use today is IPv4; The user who added the more led to exhaustion of IPv4 addresses are available on the internet. To overcome these problems developed types of IPv6. IPv4 can not directly linked with the IPv6 tunneling system is required to integrate both the IPS in a network of computers.In this final project will design a system of tunneling in a network of computers that use IPv6 as a server and client in which IPv6 data packet takes through the network IPv4 which is still existing and there occurs the process of Christianization engkapsulasian IP address, either peng-enkapsulasian IPv6 in IPv4 packet or vice versa.When using file-sharing whereby and the webserver by doing a test upload and download by the client. Skizofrenia is a clinical syndrome with a variety of psikopatology, usually severe , lasts long and the victim can be identified by a deviation from their thoughts, perceptions and emotions. There are many types of schizophrenia syndrome, Based on interviews with the doctor at Jiwa Menur mental hospital, schizophrenia syndromes ussually have similar symptoms, moreover some have only two to three distinguished symptoms. Those schizophrenia syndromes that have similar symptoms could potentially create errors in the process of determining the type of mental disorder of schizophrenia syndrome. Based on the above problems an expert system application will be made to diagnose the type of mental disorder from schizophrenia syndrome, and due to the unavailability of such applications that can help the doctor at the Jiwa Menur mental hospital. This system uses the fuzzy expert system method. This method will track each of the symptoms experienced by the victim of schizophrenia syndrome, and matching the symptons with existing rules to produces a diagnosis report. The results of diagnostic testing with expert knowledge of the system indicates that the system is able to detect the type of mental disorder on schizophrenia syndromes with 87,5% accuracy of the diagnostic result, which has been implemented to the eight victims. PT. Ume Sembada Gresik is a company that supplied the field of outsourced labor. The quality of human resources is one of the important factors to improve the productivity of the company´s performance. PT. Ume Sembada Gresik strive to improve the quality of the company to conduct an evaluation / assessment of performance in a way that is still manual. Performance appraisal system needs to be developed in the PT. Ume Sembada Gresik so as to assist in the performance appraisal problems. performance appraisal systems in this company are implemented using the Preference Ranking Organization Method For Enrichment Evaluation (Promethee) as determining the rankings. This method is part of meode Multiple Criteria Decision Making (MCDM). With Promethee methods, assessment of an employee will be assessed taking into account the differences between the criteria for each employee to another employee. Based on the results of the implementation and testing of systems, employee performance appraisal system on PT. Ume Sembada Gresik to result in any employee performance appraisals faster than manual calculation. These results can be demonstrated by the performance appraisal report. Hospital is one of organization which is focus in health services where is one of the effort is to encourage from the basic level service, like an health center for society. The hospital produces many complicated reports which is needed by the director and health official to keep the process of society service. Beside that, the reports is needed by the director to help analytyc process in order to take a policy. In this moment a hospital has used a system based on computerize to help patient service process. Although the system has not optimalized in utilising because of manual system was still used in hospitas. Because of the problem, hospital needs a new system that is called Reporting Information System, which can produces the reports in table or graphic that can be drilled down.The purpose of this final assignment is making of Reporting Information System that can help the hospital leader in order to analyze the problem dan make a decision. After that system has been created and tested (test case), can be concluded that the system has been appropriated with exptectation.because the initial objective is make a system that can produce the report in table or graphic that can be drilled down .
