Abstract

Aim Lipoaspirate, a wasted by product from liposuction procedure recently has been shown to contain abundant mesenchymal stem cells (MSCs). MSCs have been studied in many research areas to regenerate many cell lineages including, myogenic, cardiomyogenic, and angiogenic lineages. The large quantity of MSCs in lipoaspirate, makes it an attractive source for stem cells used in research and clinical applications. A simplified method which is suitable to be performed in a basic laboratory will facilitate development of stem cell research in developing countries. Therefore the outcomes from this study are expected to encourage the progress of stem cell research in Indonesia. Methods Lipoaspirate was digested using collagenase type I, followed by a basic filtration method. Purification of MSCs was done by cell culture for 2-3 days followed by supernatant removal. To confirm the homogenous population of MSCs, an analysis using flowcytometry was performed based on the MSCs minimal criteria developed by Mesenchymal and Tissue Stem Cell Committee of the International Society of Cell Therapy. Resuts MSCs were able to be obtained at 16.41 ± 8.22 x 108 cells per 120 ml lipoaspirate. The cultured cells showed fibroblastic morphology which is characteristic for MSCs and were able to be purified from non-MSCs cells. This was confirmed by flowcytometry assay showing expression of CD105 and the absence of HLA-Class II, CD 45, CD 34, CD14, and CD19. Conclusions This study has shown that it was feasible to isolate messenchymal stem cell from human lipoaspirate. The procedure was practicable to be performed within a basic laboratory. Abstract

Aim This study explored the expression of HIF-1α in hypoxic cardiac muscle in mice, and observed the evidence of apoptosis in hypoxia induced cardiomyocyte. Methods Male Sprague-Dawley rats, were randomized into 7 groups (n = 4 per group): control normoxia group that was exposed to atmospheric oxygen and hypoxia groups that were housed in hypoxic chambers (O2 level 8%) for 1, 3, 7, 14, 21, and 28 days respectively. Animals were sacrificed, hearts were rapidly excised, total RNA was extracted with an mRNA isolation kit and the expression of HIF-1α mRNA was then detected by real-time RT-PCR. Apoptosis was assessed by TUNEL method. Results For rat in hypoxia group, the expression of HIF-1α mRNA in cardiac myocytes was clearly up-regulated compared to the control normoxia group. Further, HIF-1α expression level elevated gradually and reached a peak at 21 days of hypoxia. No cell labeled by the TUNEL method was detected in the control group. Compared with the control group, the apoptotic index was significantly increased in the hypoxia group (P < 0.05). There was no significant correlation between the elevation of HIF-1α mRNA and the elevation of apoptotic index. Conclusion Systemic chronic hypoxia caused the elevation of HIF-1α mRNA and apoptosis in cardiac myocytes. (Med J Indones 2009; 18: 97-101) Keywords: TUNEL, RT-PCR, mRNA, apoptotic index

Abstract

Aim This study aimed to observe the role of the p53 gene in ovarian carcinogenesis, and identify the prognostic significance of the expression of the p53 protein expression in ovarian cancer. Methods Fourty one ovarian cancer patients were examined in this study by analyzing the mutations of p53 gene from their blood samples, and by analyzing the p53 protein expression from ovarian cancer tissues. Results Mutational analysis in "hot spot" exons (exons 5-9) and in "non hot spot exon" (exon 11) was performed by PCR-sequencing methods and p53 expression analysis was performed by immunohistochemistry method. Mutations of p53 were found in all samples, and the mutations were mostly found in “hot spot” exons, though few mutations were also found in "non hot spot" exon. The expression levels of p53 protein were found higher in advanced stages of ovarian cancer. Six month to two year post surgery follow-up of patients showed that high expression of p53 protein was in line with high mortality rate. Conclusion This study showed that the p53 gene could be used as a prognostic indicator of ovarian cancer. (Med J Indones 2009; 18:81-90). Keywords: p53, ovarian cancer, prognostic significance

Abstract

We report the case of the patient who had Ewing Sarcoma in whom radiological and hystopathological appearances revealed a tumor mass in the left big toe along with trans-artikular skip lesion on the left diaphysis of tibia. In Cipto Mangunkusomo Hospital since 1995 until 2004 we have found 20 Ewing sarcoma cases, but only one skip lesion Ewing sarcoma was found. The diagnosis of transarticular skip lesion in association of Ewing sarcoma was confirmed in clinicopathological conferrence. The initial evaluation of all patients included the recording of the medical history, physical examination, and hematological studies. Radiographs of the chest and the site of the primary tumor were made routinely. Systemic staging was performed with use of total-body bone scan. Ray amputation of left big toe and open biopsy from mass of mid-shaft of tibia had been done to confirm the diagnosis. The patient underwent induction chemotherapy and above knee amputation. Abstract

Common wart is the most frequent clinical lesion caused by human papillomaviruses (HPVs). Giant, chronic, isolated, and solitary lesion is uncommon. We reported a case of giant verruca vulgaris in a 66 year-old man on left dorsal pedis that slowly enlarged and hardened in 10 years. It was a solitary hyperkeratotic verruca vulgaris, grey brownish in color, circumscribe, round, and 3x2.4x0.4 cm in size. Histopathologic examination revealed compact hyperkeratosis/orthokeratosis, many koilocytes in the granular and upper spinous layer, acanthosis, and elongated rete ridges that were pointing radially toward the center of the lesion. Polymerase chain reaction (PCR) and in situ hybridization failed to identify HPV DNA. The patient was successfully treated with salicylic acid 40% ointment. Abstract

Aim Dose adjusment and drug selection is important in patient with renal dysfuction.This study was aimed to assess the accuracy of dose adjustment and drug selection for renal dysfunction patient at the Internal Medicine Ward FMUI/Dr. Cipto Mangunkusumo Hospital, Jakarta. Methods Patients ≥ 18 years old with estimated creatinine clearance < 60 mL/minute based on Cockroft-Gault formula were included in this study. The drugs assessed were those excreted by the kidney or having nephrotoxic effect. The appropriateness of drug selection is assessed based on the preserce or not contraindication or potential of drug-drug interaction. The accuracy of dose adjustment were assessed based on information available in various textbooks, literatures, and drug brochures. Data were collected between May to July 2007. Results Data obtained from 43 patients met the inclusion criterias demonstrated that 164 out of 385 drug prescriptions were mainly eliminated by the kidney or have nephrotoxic characteristic. Out of 164 drug prescriptions, 142 (86.5%) were appropriately adjusted, while the other 22 (13.5%) were inappropriately adjusted for the dose. There was only one contraindication for the usage of the drug and 15.1% potentially drug interaction. Conclusion Dose adjustment and drug selections in patients with renal dysfunction at the Internal Medicine Ward FMUI/Dr. Cipto Mangunkusumo Hospital are conducted appropriately. Abstract

Aim The objective of this study is to carry out morphometric measurements of the nose, in order to establish norms for the Indonesian as the representation of Asian, specifically for the Deutero Malay sub race. Method A cross sectional study was performed on 126 subjects, recruited from female students of Medical Faculty, the University of Indonesia, Jakarta. Subjects were restricted within Deutero Malay sub race (including ethnic groups of Aceh, Jawa, Sunda, Minangkabau, Riau and Bugis) in 3 generations. Subjects were photographed with digital camera in frontal, left lateral and basal view. Then from the photographs which were stored as JPEG files, linear as well as angular measurements were carried out. Results The intercanthal width is 3.56 cm ± 0.27, alar width 4.14 cm ± 0.28, length of the nose 4.0 cm ± 0.21, pronasion projection 2.29 cm ± 0.26 and tip angle 111.50 ± 4.4, nasofrontal angle 134.60 ± 7.3, nasofacial angle 36.30 ± 4.3, nasolabial angle 90.40 ± 8.3, and nasion projection 0.43 cm ± 0.22. Conclusions The data collected from this study can be used as norms for Deutero Malay cases and will serve as a guide for quick reference, when planning aesthetic or reconstructive rhinoplasties for the Deutero Malay patient. Abstract

Aim To investigate the effects of indigestible dextrin 2x2.3g/day and phytosterol 2x0.6g/day provided for 6 weeks in lowering serum LDL-cholesterol levels amongs hypercholesterolemic subjects. Methods A randomized clinical trial, two pararel groups, double blinded and randomly assigned to each different group was done in 16 subjects per-group. Results Before the, intervention the level of LDL cholesterol of both ID and FS group were 158.81 ± 17.74 mg/dL and 176.18 ± 25.31 mg/dL, respectively. After the intervention there was a significant reduction in LDL cholesterol level in both groups, i.e. among the ID group by 20.93 ± 12.65 mg/dL (13.24%) with p value of <0.001, while the reduction of LDL cholesterol level among the PS group was 21.87 ± 28.76 mg/dL (11.21%) with p value of 0.008. However, the reduction of cholesterol level between the two groups did not show any significant difference. Conclusion Consuming indigestible dextrin 2x2.3g/day and 2x0.6g/day phytosterol (PS) for 6 weeks will have the same ability to decrease the serum cholesterol level in hypercholesterolemic subjects. Abstract

Aim To examine those claims, i.e the effects of oxygenated water on hypertension and Diabetes Mellitus (DM).Methods In this clinical trial, 108 subjects of Diabetes Mellitus were recruited. Each group was divided randomly into 2 subgroups. One subgroup was given oxygenated water and the other subgroup was given non-oxygenated water for 2 period of intervention, 45 days and 90 days. Measured variables were, blood sugar and malondialdehyde (MDA).Results The study showed that oxygenated water could reduce post-prandial glucose in DM subjects. DM subjects with normal nutritional states, also had greater tendency of MDA reduction after consuming oxygenated water for 45 days. Most of subjects felt healthier after consuming oxygenated water.Conclusion The consumption of oxygenated water could improve the healing process for patients with diabetes mellitus. Abstract

Aims Microalbuminuria (MA) is a strong predictor of diabetic nephropathy and cardiovascular morbidity and mortality in patients with type-2 DM. The present study aimed to gather information on the prevalence and risk factors for MA in Indonesian patients with type-2 DM.Methods The DEMAND study was an international open cross-sectional 1-day survey on microalbuminuria prevalence and its risk factors in type-2 diabetic patients. This study reports the results of the Indonesian survey which was performed in primary care practice in Indonesia from June to December 2003. Normotensive or hypertensive adult patients of both genders with type-2 DM without known proteinuria and/or kidney disease were recruited into the study. Patients with known pregnancy, having menstruation or acute fever were excluded. A single urinary albumin/creatinine test was carried out in all patients.Results A total of 770 patients were eligible for analysis. Approximately 80% of the patients had micro-/ macroalbuminuria, while renal insufficiency was detected in about 36% of the 433 patients with available data. Target HbA1c (< 7%) was reached by only 40% of the 118 patients who had HbA1c values. Most antidiabetic treatment prescribed was oral (82%), while insulin was used by only 14% of patients. Goal BP (< 130/80 mm Hg) was achieved in only 9% of patients. The frequency of patients receiving antihypertensives was 52%, statins 18%, and aspirin 26%. The most frequently used antihypertensives were RAS blockers (45%), while diuretics were used in 7 % of the patients. The family history of DM was found in 43% of patients, the history of retinopathy in 16%, diabetic foot 9%, and history of smoking in 20% of patients.Conclusion These data reveal that Indonesia has one of the highest frequencies of silent diabetic kidney disease seen in any national group in the global DEMAND Study. Early detection, monitoring of vascular complications, and more aggressive multifactorial treatment aiming at renal and vascular protection are urgently needed for Indonesian patients with type-2 diabetes.
