Sesuai dengan standard ISO 45001 bahwa karyawan harus berpartisipasi dalam melakukan pencegahan kecelakaan. Untuk itu perusahaan telah menetapkan Program Hazob (Hazard Observation) untuk mengidentifikasi bahaya dan melakukan tindakan koreksinya. Penerapan Program Hazob masih dengan metode konvensional, mengisi lembar form, sehingga tidak efektif, efisien dan tidak berintegrasi dengan sistem lain. Transformasi digitalisasi diperlukan dengan merubah bisnis proses pelaporan dari konvensional ke aplikasi website atau mobile Apps. Hasil dari penelitian ini terjadi peningkatan kinerja operasional keselamatan kerja dengan program Hazob (i) pengisian form secara manual bertransformasi ke system digitalisasi aplikasi website dan mobile Apps; (ii) leading indikator pelaporan hazob meningkat menjadi 1.364 dari seluruh lokasi. Ini bermakna, bahwa perusahaan telah mengidentifikasi bahaya sejumlah tersebut dalam kuran waktu 3 bulan, dan melakukan tindakan koreksi sebelum terjadi kecelakakan. Data persentasi kontribusi menunjukan bahwa semua pihak bagian di perusahaan berkontribusi untuk melakukan pelaporan hazob. Hal ini menunjukan komitmen semua tingkatan untuk melakukan pecegahan kecelakaan. Angka terbesar pada tingkatan karywan ( 63 %) berkontribusi ; (iii) dampak implementasi berupa tingkat penurunan angka kecelakaan dengan Lagging Indikator sebagai berikut : LTIR = 0 (Nol), TRIR, yang sebelumnya 0.87 di bulan juni, turun menjadi 0 (nol) dari bulan Juli sampai September 2020, dan IFT, turun terus setiap bulan dan di bulan September di angka 12.33.

Abstract

Let G=(V,E) be a simple and connected graph. The adjacency matrix G is a representation of a graph in the form of a square matrix, with the size of the matrix determined by the order G. By defining a graph into a matrix, lots of research related to a spectrum has been done by researchers. Later, they defined the anti-adjacency matrix as a matrix obtained by subtracting a matrix with all entries equal to one and the adjacency matrix G. In this paper, we determine the characteristic polynomial of matrix anti-adjacency for corona product between complete graph K_m and K_1and hyper-octahedral graph H_m and K_1 with the eigenvalues.

Abstract

PT. Bakrie Autoparts produced BT-1804 products for 12 months, and of the 9537 units produced, there were 3572 product defects with various types of defects. Therefore, Six Sigma with DMAIC is used to minimize defect problems. In stages, define used Critical to Quality (CTQ), where 16 types of potential defects were found in the BT-1804 product. At stage Measure, the Pareto diagram measures the highest frequency of defects and the improvements needed. At stageAnalyze used the Fishbone Diagram, where this method focuses on repairing 1 type of defect in BT-1804, which is the type of machining defect that is the dominant defect. At stageImprove, correcting the root of the problem with FMEA (Failure Mode and Effect Analysis), where it was found that inaccurate machine settings had the highest RPN value of 100. So, suggestions were given by providing knowledge in the form of training related to production mechanisms and providing supervision of operator performance during the machine setting process. Keywords: Quality Improvement, Six Sigma, DMAIC; Failure Mode and Effect Analysis

Abstract

Computer networks have now become a primary need for some people, be it to do work or other things. The same happened at the site office of PT. LEN Industri Palembang all employees are very dependent on public network connections or the internet to do work or communicate with sites or head offices. Sometimes the network is disrupted so that it does not allow employees to do their work. Therefore, researchers will implement policy-based route based failover which aims to separate traffic and also failover as a backup connection, this can be implemented using a built-in tool from MikroTik, namely Netwatch and also 2 ISPs that work in a way when ISP 1 experiences interference, traffic will be diverted to ISP 2 or vice versa and by using policy based route traffic on the network can be separated so that it is not Make the network choked or full traffic.

Abstract

The program of self-declare halal certification by the Indonesian Government can increase the community's life level. Self-declare halal can increase people in business sales for the global market and improve the community business unit. In this case, guidance and companion strategy are required until the business actor can obtain halal certification. The data were collected on 20 business actors of MSME in Rupat Island, Bengkalis Regency, using Strategic Assumption Surfacing and Testing (SAST) with five indicators covering the commitment and responsibility, material, halal product process, the product, and monitoring and evaluation. The research results showed that the commitment and responsibility indicator is the highest indicator, becoming the priority of companion module proposal making of halal certification at 3T Areas (Least Developed, Frontier, and Outermost) Regions. Further research needs to identify the success factor of the business actors in 3T regions in gaining the halal certification program from the Government.

Abstract

In activities production PT. XYZ has several forms of production processes. It is a grinding process, which has the potential to give rise to noise coming from the tools used during the production process. The continuous noise produced can damage healthy hearing workers. To know the noise level produced during the grinding process on module 04 and module 05 of the Santos project and calculate the recommended time When exposed to noise. Data collection will use a sound level meter to gauge noise. Data calculations using distribution frequency and counting level noise equivalent ( Leq ) obtain point noise on Dilinta Sembiring and Saiful above the Threshold Limit Value (NAV), namely 86.87 dBA and 85.35 dBA. Calculated exposure time use method NIOSH recommends that Crossed Sembiring can work for 5 hours 19 minutes and Saiful for 7 hours 37 minutes. Abstract

 The Gajah Mungkur Reservoir is one of the popular destinations by the people of Wonogiri. The Regional Government of Wonogiri Regency plans to develop the Gajah Mungkur Reservoir in 2024. The development plan for the Gajah Mungkur Reservoir is predicted to have the potential to have potential impact on decreasing the performance of the road network and intersections in the surrounding area, so a study is needed regarding the current performance of parking, road networks and intersections, current generation and attraction, parking performance, road and intersection networks after development and handling traffic impacts due to development. The analysis method uses MKJI 1997. Based on the results of the analysis, it can be seen that the performance of Jalan Wonogiri - Manyaran - Blimbing/Bts. Provinsi DIY (B-T) in the 2023 Existing condition has a saturation degree of DS=0.43 with a service level of "B", while for Jalan Wonogiri - Manyaran - Blimbing/Bts. Prov. DIY (T-B), degree of saturation DS=0.39 with service level "B". The performance of Simpang 4 Pintu Masuk on the 2023 Existing has a degree of saturation DS=0.407; Queue Opportunity is 19.14% and the intersection delay is 7.89 seconds/pcu with a service level of "B". Performance of Simpang 4 Pintu Keluar in 2023 Existing has a degree of saturation DS=0.450; the Queue Opportunity is 21.59% and the intersection delay is 8.40 seconds/pcu with a service level of "B". In the existing conditions in 2023, the attraction of the Gajah Mungkur Reservoir is 155.38 pcu/hour and the generation is 116.88 pcu/hour. In 2024 construction period until 2027 after development, the performance of Jalan Wonogiri - Manyaran - Blimbing/Bts. Provinsi DIY is still in good condition with a service level of "C". However, in 2028 after development, the performance of the Jalan Wonogiri - Manyaran - Blimbing/Bts. Provinsi DIY has a decrease in performance and an increase in the DS value where the performance of the Jalan Wonogiri - Manyaran - Blimbing/Bts. Provinsi DIY (B-T) has DS = 0.76 with a service level of "D" while for Jalan Wonogiri-Manyaran-Blimbing/Bts. Prov. DIY (T-B) has DS = 0.60 with a service level of "C". The performance of Simpang 4 Pintu Masuk in 2028 with development has a degree of saturation DS=0.583; the chance of queuing is 30.58% and the intersection delay is 9.76 seconds/pcu with a service level of "B". The performance of Simpang 4 Pintu Keluar in 2028 with development has a degree of saturation DS=0.643; Queue Opportunity is 35.53% and the intersection delay is 10.44 seconds/pcu with a service level of "B". Parking performance, both existing and after development, is still able to accommodate all vehicles entering the Gajah Mungkur Reservoir. Recommendations for handling traffic impacts (Do Something) due to the Gajah Mungkur Reservoir Development consist of 2 alternatives, namely alternative 1 by reducing side obstacles by controlling street vendors on the road shoulder and prohibiting on-street parking while alternative 2 is by arranging separation of traffic directions on the Jalan Wonogiri - Manyaran - Blimbing/Bts. Prov. DIY. Of the two treatment alternatives, alternative 1 provides better results than alternative 2. Performance of the Jalan Wonogiri - Manyaran - Blimbing/Bts. Provinsi DIY experienced a decrease in the DS value for each direction from the previous DS=0.76 with a service level of "D" for the B-T direction to DS=0.74 with a service level of "C" while for the T-B direction from the previous DS=0.60 with service level "C" becomes DS=0.59 with the same service level, namely "C". The performance of Simpang 4 Pintu Masuk in the 2028 with Development & Do Something 1 shows good results, with a degree of saturation of 0.576; Queue Opportunity is 30.11% and the intersection delay is 9.69 seconds/pcu with a service level of "B". The performance of Simpang 4 Pintu Keluar in 2028 with Development & Do Something 1 shows good results, with a degree of saturation of 0.637; the Queue Opportunity is 34.95% and the intersection delay is 10.37 seconds/pcu with a service level of "B". Apart from that, it is also recommended to put several road equipment both inside and outside the Gajah Mungkur Reservoir location.

Abstract

A warehouse is a fixed facility or building that can store goods, raw materials, and finished materials used during production. Therefore, an effective warehouse layout design can help smooth production and storage processes. PT XYZ is a company that produces Phthalic Anhydride (PA). The company has a spare parts warehouse that stores materials, such as mechanical spare parts, electrical spare parts, chemicals, and consumables. The company's problem is that the storage layout's design is not organized, and the materials are not neatly arranged, so it takes time to search and ignore the types of fast-moving and slow-moving materials. Therefore, the purpose of this research is to redesign the layout of the company's warehouse to make it more efficient. The implementation of this research uses a comparison of 2 methods to choose the most optimal one to use, namely dedicated storage and class-based storage. The dedicated Storage method is used as fixed storage because the material place has been determined. In contrast, class-based storage is a method of grouping goods according to each group's type, size, and block boundaries. The results of the calculation of dedicated storage amounted to 472.75m. In comparison, the results of the calculation of class-based storage showed a total jar of 498.1m with an initial layout distance of 529.75m. Then, the most optimal layout proposal uses a dedicated storage layout proposal. Based on the implementation of the research, the benefits obtained from using this method are shortening the retrieval distance and making it easier for employees to find materials. Keywords: warehouse, layout, design, dedicated storage, class-based storage

Abstract

Computer network is a communication between two or more computers that can exchange information data. The development of a network is supported by adequate devices in that place. When the network has developed, it must be able to periodically monitor all connected network devices. Methodist School 2 conducts online platform-based learning, namely JIA (Jelajah Ilmu Acer) for the junior high, high school and vocational levels. Mehtodist 2 Palembang School uses Mikrotik as its network router, a problem was found that it could not control the network at Methodist School 2 Palembang regularly. This will make it difficult for a network administrator to supervise because they have to control and analyze the performance of their devices and network communications manually. With this problem, the author made an Implementation of Network and Server Monitoring on Mikrotik with Netwatch via Telegram Bot, the author configured several logs that will be integrated into the telegram bot in the Mikrotik tool, Netwatch. The telegram bot will send message notifications about the status of connected devices in the proxy, with this project it is hoped that the author can help network administrators to be able to help control and analyze the network at Methodist School 2 Palembang.

Abstract

In meeting the needs of spare parts in a supply chain, the warehouse is one of the most important parts. Warehouses have three main processes, namely receiving, storing, and distributing so, warehouse needs are certainly very important to help companies achieve their goals. This research uses an integrated Fuzzy Logic, House of Risk (HOR), and Supply Chain Operation Reference (SCOR) approach. Interviews, focus discussion groups, and questionnaires are the data collection methods used. From the results of data processing, 19 risk events and 27 risk agents were obtained. From the results of the pareto diagram, 14 risk agents were selected to focus on risk mitigation and have the ability to direct the reduction of risk. Furthermore, 14 risk agents were developed into 28 mitigation strategies and there were 10 prioritized risk mitigation strategies. Keywords: Risk Mitigation, Warehousing, SCOR, HOR, Fuzzy Logic, Pareto Diagram.
