Abstract

Immunization is the most cost-effective health prevention method to suppress the incidence of vaccine-preventable disease. Immunization Program in Jember district is still not optimal with many vaccine-preventable disease cases found. Another problem found in Jember is the management of logistics vaccine that is still not optimal. Based on the activity plan report of ORI Difteri 2018, it is known that the logistics needs of some vaccines do not comply with the allocation received by Jember regency of the province. This condition This was a descriptive study using questionaire and observations data as well as the documentation study of the annual Report of Health Pharmacy Installation (IFK) year 2019. Respondent to this study were immunization officers at primary health care of Jember District. The result of cold chain management evaluation is known that the most are good enaugh. In some primary health care are still hampered insufficient means so that the maintenance of vaccine is still not optimalBased on the result that cold chain management evaluation, the knowledge officer on cold chain management is still lacking good. In the some primary health care, it is known that the equipment canstraints are unavailable. While in the conformity of temperature only one primary helath care is not recommended. And stirage activities are mostly in accordance with SOP.

Abstract

Health financing in Indonesia has increased from 27 US $ (2003) to 115 US $ (2017) and will continue to increase as the population increases. The JKN era is also a burden where health financing managed by BPJS is always deficit every year. Hospitals as health service providers must be able to carry out operational cost efficiency as mandated by the JKN era, namely quality control and cost control. One of the cost efficiency methods is the implementation of lean management in hospital operational processes, especially the supply chain management process (procurement, inventory control, distribution planning and demand management). This study aims to evaluate the improvement process and cost efficiency after the implementation of lean management in the supply chain management process at the hospital. This study used a single holistic case study method with the unit of analysis at the Lean team level at Panti Rapih Hospital, Yogyakarta. Research subjects are employees in the team who are directly involved in the implementation of lean management. Selection of research subjects based on purposive sampling. Total sample of 7 respondents for in-depth interview process. The research instrument used an interview guide. The result of this research is that lean management which is applied in the logistic unit of the neat orphanage hospital can reduce waste such as motion, inventory, and waiting time. In addition, another benefit that can be felt by management is increasing cost efficiency. The conclusion of this study is that lean management is recommended to be applied in health services such as hospitals.

Abstract

Adolescents are particularly vulnerable to a variety of health risks, especially about sexual promblems and reproductive health problems. The government develops adolenscent health programs in Indonesia by using the Adolenscent Friendly Health Service (PKPR) at the health care level. Adolescent health programs in 2019 at Magelang city has not achieved performance targets, so that it becomes attention to optimizing the PKPR program. This research is to find out cause the implementation of the PKPR program in Magelang Tengah health center which is not optimal with influencing factors. The method of research was qualitative with descriptive approach. The technique of collecting data was through in-depth interviews with subject sellection with purposive sampling criteria. Data analysis used content analysis. Analyzed variables were communication, resources, dispositions or attitudes, and bureaucratic structure. The results showed that implementation of PKPR in health center has not been optimal because have some constraints which influenced by several factors, such as the lack of communication between PKPR teams in coordinating scheduling activities, the understanding and clarity about PKPR program owned by officers was limited, the function of public service room in health center has not been optimal to provided service to adolescent, and monitoring and evaluation was limited. Therefore, it needs to communication and coordination was scheduled regularly so that has a good team management, and needs to monitoring and assessment of SNPKPR by Department of Health regularly in order to know the achievement and factors which influence the unoptimability of implementation PKPR program in the health center.

Abstract

One of the high turnover rates in the hospital occurs in nurses. More attention is needed because turnover can be detrimental to the organization, in terms of cost, resources, and effectiveness of nursing. The research objective is to obtain empirical evidence whether there is an effect of compensation, job stress and job satisfaction on turnover intention at Ananda Bekasi Hospital in 2020. The research method is to use causality design based on the time dimension of one short study. The sample used was saturated sampling, that is, the entire population was sampled for 100 inpatient nurses. The data analysis method uses Multiple Linear Regression Analysis. The results found no negative and significant effect between compensation for turnover intention. There is a positive and significant effect between work stress on turnover intention. There is a negative and significant effect between job satisfaction on turnover intention, and there is an influence between compensation, job stress and job satisfaction on turnover intention. Research findings, that the variable that most influences turnover intention is work stress variable. Managerial implications are expected Ananda Bekasi Hospital to calculate salaries based on the remuneration system, the appropriate assignment system, conduct workload analysis and nurse career development

Abstract

One of the determinants of achieving quality control and cost control is the implementation of tiered referral health. This research was motivated by the absence of research on the implementation of referrals from FKTP to FKTL. Data from RSUP Dr. Wahidin Sudirohusodo’s annual report showed in 2015 and first half of 2016, >60% cases in class A hospital were level 1 severity case which should still be handle in class D and C hospitals. This affects the accumulation of patiens in tertiary hospitals and inefficient use of resources. It is a qualitative research with phenomenology approach to determine the implementation of a tiered referral system between advanced health facilities in JKN era in South Sulawesi. The results shows most of referral were made due to facilities limitation and inavailability of specialist.  There were referrals which made based on patients requests not based on clinical reasons. It is also found hospital staff who did not know about the existing of standard operational procedure of referral in the hospital. Some hospital staff has no knowledge on the referral regulation nor about hospital classification. This research is expected to be able to contribute to the development of science and can be used as input for hospitals and BPJS in an effort to improve the optimization of tiered referral services. Based on the results of the study it is recommended to improve facilities and infrastructure that can support the provision of referral services in hospitals.

Abstract

Health development starts from the smallest unit of society, namely the family. The Ministry of Health establishes an operational strategy for health development through the Healthy Indonesia Program with a Family Approach (PIS-PK). Data collection activities carried out in the PIS-PK aim to obtain health data for each family member by the Puskesmas. This study aims to determine the description of the index of healthy families in the working area of Puskesmas Batang I by using 12 indicators of healthy families related to the Health Promotion Program. This research is a quantitative study carried out in a descriptive observative manner, namely by analyzing the data narrative. Desa Kecapak has an index value of 0.08 (Unhealthy Family), Kelurahan Sambong has an index of 0.2 (Unhealthy Family), Kelurahan Proyonanggan Utara  has an index 0.13 (Unhealthy Family), and Kelurahan Proyonanggan Tengah has an index 0 , 21 (Unhealthy Family). Puskesmas Batang I work area has an index of 0.18 and is in the Unhealthy Family category. The families that were recorded had not had total coverage, which was still 54.2%. The health promotion program of the Batang I Puskesmas includes data collection on Clean dan Healthy Behavior/Perilaku Hidup Bersih dan Sehat (PHBS), training for posyandu cadres, self-awareness surveys/Survey Mawas Diri (SMD), and guidance for Alert Village/Desa Siaga.. The Health Promotion Program can refer to data on the healthy family index and PHBS by looking at the low indicator coverage of healthy families. Puskesmas can cooperate with educational institutions so that the number of families being recorded is maximized. Based on the healthy family index coverage figures, the health promotion program must synergize with the Disease Eradication Program, especially pulmonary TB disease, hypertension, and mental disorders. It is hoped that the development of posyandu and posbindu cadres can increase the coverage of the healthy family index, especially in increasing family knowledge about infectious and non-communicable diseases

Abstract

Background UNICEF and WHO estimate that if all babies were breastfed for at least the first six months of their lives, morbidity and malnutrition rates would significantly decrease worldwide. both of these organizations promote the policy of initiating early breastfeeding by campaigning for a baby-friendly hospital policy, one of which encourages early breastfeeding initiation to be carried out. The aim of our study is to review the available evidence regarding the positive effects of early breastfeeding initiation on the success of exclusive breastfeeding, to suggest the most appropriate strategy to support it.Research Methods The research taken was research published in 2000-2019, obtained through major databases such as PubMed, Google Schoolar, and then examined to obtain original documents. Research searches related to the effectiveness of early initiation initiation (IMD) and the factors that influence it use the main terms "early initiation of breastfeeding", "effectivity" and "determinants of". A total of 31 literature conclusions and results of research in making this review. The 31 selected literatures have met the specified inclusion criteria.The Result Giving breastfeeding initiation is best done no later than 1 hour postpartum. IMD (initiation of early breastfeeding) is effective in stimulating milk production, producing antibody protection, reducing postpartum maternal bleeding, and in the long term affecting the duration of breastfeeding. IMD has also been studied as having the effect of reducing neonatal mortality by preventing morbidity such as diarrhea, pneumonia, UTI, meningitis and neonatal sepsis. Factors that influence the success of IMD include geographical, socioeconomic, individual health, education and maternal knowledge about IMD, duration of labor, maternal employment, hospital policy, support from health workers, labor delivery, gestational age, and maternal health conditions.Conclusions Breastfeeding initiation has been extensively studied increasing the success of exclusive breastfeeding and contributing to the reduction in infant mortality caused by infection. However, the success rate of breastfeeding initiation in several developing countries in the world still varies. Many factors influence the success of breastfeeding initiation, which still need to involve health promotion, because the implementation of breastfeeding initiation needs to involve many parties, in addition to the general public as well as to health workers who are tasked with helping in the maternity unit.

Abstract

Latar belakang: Duplikasi nomor rekam medis merupakan masalah yang sering terjadi di puskesmas, salah satunya adalah Puskesmas Labruk. Berdasarkan hasil pendahuluan, dari 2.501 sampel berkas rekam medis didapatkan 10,3% berkas yang terduplikasi. Dampak dari duplikasi ini adalah tidak lengkapnya kronologi penyakit pasien, penggunaan map dan formulir yang berlebihan. Tujuan dari penelitian ini adalah untuk menganalisis penyebab duplikasi nomor rekam medis berdasarkan dukungan organisasi dan dukungan manajemen di Puskesmas Labruk.Metode: Jenis penelitian adalah kualitatif dengan teknik pengumpulan data yang digunakan adalah wawancara dan observasi. Objek penelitian adalah berkas rekam medis dan subjek penelitian adalah petugas pendaftaran, kepala rekam medis dan kepala puskesmas.Hasil: Hasil penelitian menunjukkan bahwa faktor yang menyebabkan terjadinya duplikasi dari dukungan organisasi adalah tidak adanya SOP, tidak adanya KIUP, dan nomor rekam medis tidak dibuat otomatis pada SIMPUS. Faktor yang menyebabkan duplikasi dari dukungan manajemen adalah tidak dilakukannya evaluasi dan diskusi untuk mengatasi masalah duplikasi.Simpulan: Dukungan organisasi dan dukungan manajemen adalah penyebab dari terjadinya duplikasi, apabila terdapat kekurangan pada dukungan organisasi ataupun manajemen maka duplikasi akan terus meningkat.

Abstract

Establishing an excellent hospital image is critical as many people are still experiencing trust issues for hospitals in Indonesia, especially in major cities. A hospital's image comes from its quality of service, the patient's trust in the quality of service, and the patient's satisfaction. This study tries to provide empirical evidence on the effect of quality of service and patient confidence on the hospital's image mediated by patient satisfaction. A quantitative - causality research method is used. Primary data was taken from survey to patients who are taking drugs in the outpatient pharmacy installation unit at Taman Sari Hospital, West Jakarta. This research uses AMOS-path analysis, and the results showed the quality of service and patient's trust had a positive and significant effect on the hospital's image with patient satisfaction as an intervening variable. Partially, the quality of service has a positive and significant effects on both patient satisfaction and hospital image. Patient confidence has a significant positive effect on patient satisfaction but it is insignificant to the hospital's image. Research findings show that a patient's trust has a positive impact on the hospital's image when mediated by patient satisfaction. Suggestions from this research are conducting training, i.e. speed training not to exceed the standard response time as well as competency, skills training and service excellence training for pharmaceutical personnel. Improving the quality of pharmacy facilities such as waiting rooms to make it more comfortable is also suggested
