Abstract

The well-known hypervolemia associated with normal pregnancy averages 40 to 45 percent above blood volume in non-pregnant women after 32 to 34 weeks. The case was on Mrs. FN, 40 years old, a patient reffered due to sinus bradycardia before the insertion of permanent pacemaker. After 6 month-use of the permanent pacemaker, she became pregnant with triplet pregnancy. This case report evaluated the patient's condition from her hypervolemic condition to her cardiac function.

Abstract

Objective: This study aims to determine the enforcement of diagnosis, management and description of the referral system of PEB and eclampsia patients at Level I Health Facilities and South Konawe Hospital in January-December 2017Materials and Methods: This study used an observational descriptive design, carried out in the Maternity Room of South Konawe Hospital on 1 January-31 December 2017. There were 47 cases of PEB/Eclampsia from 420 total cases. Most cases of PEB/Eclampsia in Konsel Hospital suffered by pregnant women aged> 35 years, as many as 27 cases (59%). age 21- 34 years, 15 (31%) and the lowest among women aged <20 years, as many as 5 cases (10%). Multigravida, which is 38 cases (81%) and 9 cases 19%) in primigravida. Pregnancy> 34 weeks is 46 cases (98%) and 1 case (2%) at <34 weeks gestation. There were 39 referral cases (83%) all of which were counseled and 8 non-referral cases (17%), where completeness of the tools, medicines and referral letters obtained 39 cases (100%) incomplete.Results: The administration of antihypertensive drugs was given to 89% of cases, and MgSO4 was not given to 100% of cases of PEB/Eclampsia referred to RSUD Konawe Selatan due to fear of officers in providing such therapy to PEB/Eclampsia patients. Examination of urine protein as a support for PEB/Eclampsia cases was recorded in 69% of referral cases at the Konawe Selatan General Hospital.Conclusion: From the whole data, it was found that the pre-requisite conditions had not been fulfilled properly. Referral should be well prepared in order to achieve patient safety and decrease in MMR.

Abstract

Objectives: To present three cases of young women with suspected ovarian malignancy with the aim to evaluate the role of intraoperative frozen section in suspected malignant ovarian neoplasms in women with young age.Case Report: Mrs. M, 25 years old, is a nullipara with a history of fertility treatment. She presented with bilateral ovarian cysts suspected to be malignant and an elevated CA-125 level. Intraoperative frozen section showed borderline ovarian tumor, allowing for conservative management. Mrs. A, 32-years old, with right cystic ovarian neoplasm suspected to be malignant and elevated CA-125. Frozen section showed results of cystadenocarcinoma, patient’s family was informed of the result during the surgery and the operator continued to manage the patient conservatively. Ms. N, 27 years old, was referred with suspected ovarian malignancy and the differential diagnosis of peritoneal tuberculosis. Frozen section confirmed the presence of tuberculosis infection, and the patient was managed accordingly.Conclusion: Despite conservative management being top priority treatment in young-aged women with ovarian neoplasm, additional information of frozen section allows for more accurate management.

Abstract

Objective: To identify the effectiveness of beet juice administration on hemoglobin level of anemic pregnant women.Materials and Methods: This was an experimental quantitative study using pretest-posttest design with a total sample of 30 patients who were divided into 2 groups of treatment groups (beetroot + Fe) and control group (Fe). The treatment was carried out for 14 days. Before and after the treatment, the samples were subjected to hemoglobin examination. Data in this study were the difference between pre-test and post-test values, the results of normality test with Shapiro Wilk, and, to identify the difference between pre-test and post-test, the results of Wilcoxon Test for abnormal data and Paired T-Test for normal data.Results: Mann Whitney Test results between treatment and control groups revealed p (0.023) < alpha (0.05). There was a significant difference in hemoglobin levels between TM III pregnant women with anemia receiving beets and those receiving Fe tablets only.Conclusion: Minimum Hb increase before and after the administration of Fe and beetroot juice to pregnant women was 17.7 gr% and the maximum Hb level was 0.8 gr%.

Abstract

From normal epithelial cells to invasive cervical cancer take a long time. It needs a step-by-step process. Normal cervical cells may be infected with the high-risk human papillomavirus (HPV). The infection may persist or be cleared. The HPV persistence may change normal cells to invasive cancer. Recent findings showed that the intrauterine device (IUD) might have a protective effect on cervical cancer. Inflammation in the cervix induces the immune response that may alter normal cells' progression to cervical cancer. This article will explain the change from normal cells to invasive cervical cancer, the role of the IUD in cervical cancer prevention.

Abstract

Objectives: to analyze the relationship between characteristics, parity and maternal reproductive history with the risk of cervical pre-cancerous lesions.Materials and Methods: Cross Sectional Survey, in 2016 Non-Communicable Disease study, with a population of women aged 25-64 years, living in urban areas. A total of 1,547 samples were selected from the census blocks that were prepared to IVA examination. The variables in this article are maternal characteristics, menarche, age of first sexual intercourse, number of partners, parity and contraceptive use, as independent variables and cervical pre-cancerous lesions as dependent variables.Results: Factors influencing cervical pre-cancerous lesions were: Menarche with a value of P = 0.018; Age of first sexual intercourse with a value of P = 0,000; Number of sexual partners with a value of P = 0.023; and parity with a value of P = 0.049. Multivariate test results show that the age of first time having sex less than 20 years has a 2.3 risk; Mothers with high parity (> 4 children) and having more than one sexual partner have a 16.5 risk.Conclusion: Menarche, age of first sexual intercourse, number of sexual partners and parity affect the incidence of cervical cancerous lesions. The most influential Factors are: the age of first sexual intercourse less than 20 years, and mothers with high parity who have more than one sexual partner.

Abstract

Objective: The purpose of this study was to assess patients’ perception on tiered maternal referral system (during pregnancy, delivery, and postpartum) in a hospital of West Sumatra of Indonesia.Materials and Methods: The design was a cross-sectional interview survey. A convenient sampling technique was used to recruit 134 sample. Our study was approved by the Committee of the Research Ethics of the Faculty of Medicine, Andalas University number 002/KEP/FK/2018. An interview guideline was prepared and translated into Bahasa Indonesia. There were two interviewers recruited and trained to collect information for the study. Each interview took approximately 12 minutes and were recorded using mobile phone. Data from the interviews were analysed by using QSR NVIVO 10 and MS Excel 2010.Results: The vast majority of the respondents reported that there was sufficient understanding of the services and the structure of the overall tiered referral system. The complaints addressed the issue of administrative procedure, accessibility, and equity, waiting time and card activation, ease of obtaining referral letter in the primary level of healthcare, and transport duration when the patient has to be referred to advance level of healthcare.Conclusion: The functional use of electronic referral system is an urge for solution. Future studies on patients’ perception and assessment toward healthcare providers’ opinion should be carried out to increase the service quality.

Abstract

Objective: The purpose of this study was to determine the maternal outcomes of severe preeclampsia at RSUD Dr. Soetomo Surabaya in January 2013-December 2014.Materials and Methods: This research was a descriptive study with cross-sectional design to observe maternal characteristics and maternal outcomesof severe preeclampsia. Data were retrieved from medical records of severe preeclampsia patients admitted to Obstetric Ward of Dr Soetomo Hospital, from January 2013 to December 2014. This study used total sampling for collecting its data. These data were proccessed descriptively and presented in graphic, tables, and short description.Results: From January 2013 to December 2014 there were 386 (44.2%) cases of severe preeclampsia that were included in this study from a total of 874 cases available. The maternal outcomes of severe preeclampsia consisted of 42 cases (10.9%) of HELLP syndrome, 36 cases (9.3%) of pulmonary edema, 225 cases (58.3%) of sectio caesarea, 7 cases(1.8%) of antepartum bleeding with 5 cases (1.3%) of placenta previa and 2 cases (0.5%) of solutio placenta, 2 cases (0.5%) of postpartum bleeding, 8 cases (2.1%) of eclampsia, 31 cases (8%) of impending eclampsia, 5 cases (1.3%) of acute kidney injury, and 2 cases (0.5%) of maternal death.Conclusion: In conclusion, this study shows that severe pre-eclampsia patients have high prevalence of mortality and morbidities that affects maternal outcomes. It also reccommends that all patients with severe preeclampsia need to receive intensive maternal and fetal care. It is necessary to do careful complication examination, prevention of seizures using magnesium sulfate, and continous fetal and maternal monitoring.
