<![CDATA[ABSTRAK Pengisian formulir informed consent di Puskesmas Rampal Celaket masih dilakukan secara manual karena belum adanya informed consent secara elektronik. Namun, ketersediaan fasilitas seperti komputer, handphone, dan jaringan internet dapat dimanfaatkan untuk mendukung proses digitalisasi informed consent. Studi ini bertujuan untuk mengembangkan aplikasi Zoho Forms guna meningkatkan kelengkapan dokumen informed consent tindakan medis di Puskesmas Rampal Celaket. Penelitian ini menggunakan metode pengembangan sistem dengan pendekatan prototyping untuk merancang formulir informed consent di Puskesmas Rampal Celaket. Identifikasi kebutuhan sistem diperoleh melalui wawancara mendalam dan forum diskusi kelompok terfokus (FGD). Pengembangan prototipe diujicoba kepada pengguna sehingga sesuai dengan kebutuhan pengguna. Hasil dari penelitian ini berupa E-informed consent melalui aplikasi Zoho Forms. Hasil ujicoba menunjukkan peningkatan signifikan dalam kelengkapan pengisian informed consent setelah digitalisasi. Sebelum digitalisasi, kelengkapan dokumen hanya mencapai 3%. Namun, setelah memanfaatkan Zoho Forms, kelengkapan meningkat hingga 100%. Aplikasi Zoho Forms menyediakan fitur isian wajib, validasi otomatis, dan penyimpanan data secara digital, yang membantu mengurangi kesalahan dan meningkatkan efisiensi dalam pemberian layanan. Penelitian ini memberikan kontribusi dalam pengembangan solusi digital yang praktis dan berbiaya rendah di fasilitas pelayanan kesehatan primer, serta menawarkan model yang dapat ditiru untuk meningkatkan mutu dokumentasi dan keselamatan pasien.Kata kunci : Digitalisasi, Informed Consent, Prototipe, Zoho Forms ABSTRACT The completion of informed consent forms at Puskesmas Rampal Celaket is still carried out manually due to the absence of an electronic system. However, the availability of facilities such as computers, mobile phones, and internet connectivity can be utilized to support the digitalization of informed consent. This study aims to develop a Zoho Forms–based application to improve the completeness of medical informed consent documentation at Puskesmas Rampal Celaket. The research employed a system development method using a prototyping approach to design the electronic informed consent form. System requirements were identified through in-depth interviews and focus group discussions (FGDs). The prototype was then tested by end-users to ensure alignment with their needs. The outcome of this study is an e-informed consent system developed through Zoho Forms. The trial results indicated a significant improvement in the completeness of informed consent forms after digitalization, which was 3% using manually documentation and increased to 100% after implementing Zoho Forms. Zoho Forms provides features such as mandatory fields, automated validation, and digital data storage, which help reduce errors and enhance efficiency in service delivery. This study contributes to the development of a practical and low-cost digital solution for primary healthcare facilities, while also offering a replicable model to improve documentation quality and patient safety.
