Abstract

Referrals for delivery are a crucial component of maternal health care systems to prevent more severe maternal complications. In primary midwifery care, referral decisions are influenced not only by the mother's clinical condition but also by midwifery service factors. This study aims to analyze the clinical and service determinants of referral decisions for delivery in primary midwifery care. The study used an analytical observational design with a retrospective approach. Data were obtained from the delivery register, partograph, and Puskesmas referral book for 2024 with a population of 174 and a sample of 121 deliveries. Data analysis was performed using univariate and bivariate analysis using Fisher's Exact Test and limited determinant modeling. The results showed that delivery referrals occurred in 8.3% of cases. Severe obstetric complications and certain midwifery service factors played a major role in the decision to refer deliveries. This study confirms that delivery referrals are an outcome of the primary midwifery service system that is influenced by the interaction of clinical and service factors

Abstract

Background: Cow's milk is a widely consumed source of protein and calcium, but it cannot be consumed by individuals with lactose intolerance or a cow's milk protein allergy. Plant-based milks, particularly almond milk, have the potential to be an alternative because they are lactose- and casein-free and contain essential nutrients.Objective: This study aimed to analyze the protein and calcium content of almond milk with varying compositions as an alternative to cow's milk.Methods: This study was an experimental study with a completely randomized, one-factor design. Treatment variations were based on differences in the proportions of raw materials. Protein content was analyzed using the Bradford method, while calcium content was analyzed using the complexometric method. Data were analyzed using a one-way ANOVA test with a 95% confidence level.Results: Almond milk is a plant-based milk product obtained from processed almonds and is known to have high nutritional content and provide various health benefits. This beverage is becoming increasingly popular because many people cannot consume cow's milk due to allergies to cow's milk protein or lactose intolerance. Thus, almond milk is an alternative to cow's milk and a plant-based milk option other than soy milk for individuals with animal lactose intolerance. To assess the quality of almond milk, tests were conducted on the essential mineral content for the body, namely calcium (Ca) and iron (Fe), in commercial almond milk beverage products on a household scale consisting of samples A, B, and C. The milk samples were prepared using a 65% nitric acid solution, then diluted before being analyzed for mineral content. The test results showed that all samples contained calcium and iron minerals. The average calcium levels in samples A, B, and C were 114.92 ppm, 166.07 ppm, and 163.80 ppm, respectively, with the highest levels in sample B and the lowest in sample A. Meanwhile, the average iron levels in samples A, B, and C were 3.78 ppm, 3.42 ppm, and 2.44 ppm, respectively, with the highest iron content in sample A and the lowest in sample C. Based on the results of statistical analysis using one-way ANOVA, it was found that the calcium and iron content in the three commercial almond milk products showed significant differences. Conclusion: Almond milk has potential as an alternative to cow's milk, especially as a source of protein and calcium for individuals with special dietary needs.
GENDER DIFFERENCES IN REPRODUCTIVE HEALTH RISKS, HEALTH CARE SEEKING BEHAVIOR, AND PSYCHOLOGICAL DISTRESS AMONG ADOLESCENTS Roichatul Djannah

Abstract

Adolescent reproductive health is an important aspect of physical, psychological, and social development. Biological changes in adolescence can give rise to a variety of different reproductive health complaints between males and females. This condition has the potential to be related to psychological distress, especially if adolescents experience worries, discomfort, or limited access to information and health services. This study aims to analyze the differences in reproductive health risks and psychological distress between adolescent boys and girls. The study used a quantitative design with a cross-sectional approach. The research sample amounted to 198 adolescents, consisting of 72 adolescent boys and 126 adolescent girls. Reproductive health variables were measured through the components of C1 general conditions, C2 specific men, C3 specific women, C4 health care behaviors, and total reproductive health risks. Psychological distress was measured using the K10 adaptation instrument. Bivariate analysis was performed using Chi-Square, Mann-Whitney, and Spearman correlation tests. The results showed that there was a significant difference in reproductive health risks based on sex with a p value of < 0.001. Adolescent girls are more in the category of moderate reproductive health risk than boys. Psychological distress was also significantly different between males and females with a p< value of 0.001. Adolescent girls show a higher proportion of severe distress than boys. Total reproductive health scores and distress scores were also higher in adolescent girls. These findings point to the need for gender-sensitive reproductive health and mental health interventions in school settings.

Abstract

Adolescent reproductive health is a public health issue requiring promotive, preventive, and youth-friendly service approaches. National studies indicate that knowledge, attitudes, media exposure, school support, parental roles, and the availability of adolescent-friendly health services such as PKPR and youth information and counseling centers such as PIK-Remaja influence adolescent reproductive health behavior. This article aimed to synthesize determinants and improvement strategies for adolescent reproductive health based on five Indonesian articles published between 2019 and 2023. A systematic literature review with narrative synthesis was applied. The reviewed articles included cross-sectional quantitative studies, a qualitative study, and community service articles addressing adolescent sexual behavior, sexual media exposure, PKPR utilization, PIK-Remaja, reproductive health education, peer cadres, and parental involvement. The synthesis identified four major themes: (1) knowledge alone is insufficient to prevent risky sexual behavior without attitude formation, social control, and media literacy; (2) PKPR services should be youth-friendly, confidential, and culturally sensitive; (3) PIK-Remaja, peer cadres, and reproductive-health-oriented schools are effective educational and mentoring channels; and (4) promotion, reproductive health education, and parental roles contribute to better adolescent service utilization. In conclusion, adolescent reproductive health improvement requires an integrated model based on schools, families, youth-friendly health services, digital media literacy, and peer counselor strengthening
