Abstract

Angka Kematian Ibu (AKI) yang fluktuatif memerlukan pencegahan melalui pelayanan Antenatal Care (ANC) yang teratur. Masalah utama dalam penelitian ini adalah adanya kesenjangan antara capaian pemeriksaan ANC dengan kemandirian ibu hamil dalam mengunjungi fasilitas kesehatan, di mana banyak kunjungan justru diperoleh melalui program jemput bola. Penelitian ini bertujuan untuk menganalisis hubungan perilaku pemanfaatan pelayanan kesehatan terhadap keteraturan kunjungan pemeriksaan kehamilan yang dimoderasi oleh program sweeping ibu hamil di wilayah kerja Puskesmas Kaleke, Kabupaten Sigi. Metode yang digunakan adalah eksplanatori dengan pendekatan kuantitatif melalui Structural Equation Modeling (SEM) berbasis Partial Least Square (PLS). Populasi penelitian adalah seluruh ibu hamil periode Juni-Agustus 2025 sebanyak 303 orang, dengan sampel 76 responden yang dipilih menggunakan teknik proportionate random sampling dan pengumpulan data melalui kuesioner. Variabel penelitian meliputi perilaku pemanfaatan pelayanan kesehatan sebagai variabel independen, keteraturan kunjungan ANC sebagai variabel dependen, dan program sweeping sebagai variabel moderasi. Hasil penelitian menunjukkan pengaruh langsung yang positif dan signifikan antara perilaku pemanfaatan terhadap keteraturan kunjungan ANC dengan nilai koefisien jalur 0,231 (p=0,017). Namun, program sweeping sebagai variabel moderasi memiliki pengaruh negatif yang signifikan terhadap keteraturan kunjungan dengan nilai koefisien –0,515 (p<0,001). Simpulan penelitian ini adalah perilaku ibu yang baik secara signifikan meningkatkan keteraturan kunjungan ANC, namun program sweeping yang terlalu dominan justru berpotensi menjadi faktor penghambat kemandirian ibu untuk berkunjung secara rutin ke fasilitas kesehatan. Penguatan perilaku dan kesadaran mandiri ibu tetap menjadi kunci utama dalam meningkatkan kepatuhan pemeriksaan kehamilan di masa depan.

Abstract

The screening policy for prospective brides and grooms is a strategic intervention to prevent stunting from upstream. This research aims to evaluate the implementation of the prospective bride and groom screening policy in Dolo District, Sigi Regency, in 2025. The research uses a qualitative approach with a case study design. Data was collected through in-depth interviews, observation and documentation. The informants were determined purposively, consisting of 11 people including Community Health Center officers, KUA, cadres, and prospective brides and grooms. Analysis uses the Miles & Huberman model. The research results show that policy implementation is not optimal. The six variables of the Van Meter & Van Horn model are the main findings: (1) policy standards experience a reduction in meaning to administrative formality; (2) resources experience systemic deficits in medical logistics and budget; (3) the characteristics of implementing agents are characterized by procedural informality without local SOPs; (4) the implementer's attitude is positive but the workload is pressured; (5) fragmented interorganizational communications without fixed protocols; (6) socio-cultural conditions in the form of "good day" traditions are the main obstacle. The conclusion of this research is that the policy is administratively successful but not optimal in clinical substance. Strengthening regional regulations, data interoperability and incentives for officers is needed.

Abstract

The prevalence of stunting in Central Sulawesi is 27.2%, still above the national average of 24.2%, with Sigi Regency as one of the highest areas (24.6%). In Dolo District, the prevalence reached 22.34% (218 out of 976 toddlers) in 2024, with priority loci in Wisolo, Poi, and Waturalele Villages. Low maternal knowledge, inappropriate parenting patterns, and poor sanitation are the main obstacles in achieving the target of a 14.2% reduction by 2029. This study aims to analyze the causes of stunting and develop improvement strategies through a SWOT analysis. The qualitative research method with a strategic management approach (environmental observation, formulation, implementation, and evaluation) uses IFE, EFE, IE, and SWOT matrix analysis. Informants included the head of the Community Health Center, stunting program managers, village midwives, and the Stunting Reduction Acceleration Team. Data analysis involved environmental observation, formulation, implementation, and evaluation using the IFE, EFE, and IE matrix instruments, as well as a SWOT analysis. The analysis identified key contributing factors, including a lack of nutritional knowledge, parenting practices, and access to sanitation. Strategies implemented included the One Million Eggs program, Tanggang Shining, and specific and sensitive nutrition interventions. The SWOT analysis mapped the organization's position to formulate data-driven improvement strategies to optimize existing resources by strengthening convergence between 14 Regional Apparatus Organizations (OPDs) and utilizing real-world data to address field coordination challenges, ensuring the achievement of the stunting reduction target. Optimizing strategic management through cross-sectoral program integration and strengthening public education is key to accelerating stunting reduction.Keywords: Strategic Management; Stunting; SWOT Analysis.
