Background & Objective: Diabetes mellitus is a chronic disease that can lead to serious complications and death if not properly managed. Therefore, patients require effective self-management behaviors, which are influenced by patient empowerment. This study aimed to determine the correlation between patient empowerment and self-management among patients with type 2 diabetes mellitus at Gading Rejo Community Health Center. Method: This study used a quantitative method with a descriptive correlational design and a cross-sectional approach. The population consisted of all patients with type 2 diabetes mellitus registered at the health center. The sample included 54 respondents selected using purposive sampling. Data were collected using the Diabetes Empowerment Scale and the Diabetes Self-Management Questionnaire. Data analysis was performed using the Gamma test. Result: The results showed a significant correlation between patient empowerment and self-management, with a p-value of 0.001 and a correlation coefficient of 0.771, indicating a strong relationship. Conclusion: In conclusion, better patient empowerment is associated with better self-management among patients with type 2 diabetes mellitus. Therefore, it is important for healthcare services to enhance patient empowerment through continuous education, family support, and active patient involvement in decision-making.
