Abstract

Type 2 diabetes mellitus (DM) is a chronic disease that requires continuous drug therapy. In the use of oral antidiabetic drugs, type 2 DM patients are prone to errors due to suboptimal drug use in terms of both dosage and frequency. These medication errors are referred to as Drug-Related Problems (DRPs). This study aims to determine patient characteristics, patterns of oral antidiabetic prescription, and analyze DRPs related to the selection of oral antidiabetic doses in type 2 DM patients at Majalengka Regional General Hospital. This study is a retrospective observational study with a cross-sectional approach, using data from 85 outpatient medical records that met the inclusion criteria, namely type 2 DM patients with comorbid hypertension who received oral antidiabetic therapy during the period January-December 2024. The results indicated that most patients received combination therapy with two oral antidiabetic drugs (52%). One case (1.17%) of DRPs was found in the category of metformin overdose, which has the potential to cause drug toxicity in the form of lactic acidosis. These findings indicate that although DRP cases are relatively low, monitoring drug use in the form of medication reviews is still necessary as a preventive measure against DRPs.
