Abstract This study aims to examine the implementation of the Universal Health Coverage (UHC) policy in Metro City, which in 2024 successfully obtained the UHC Award and became the only region in Lampung Province to achieve this award. This achievement is interesting to examine considering the ongoing challenges at the national level, particularly in terms of increasing National Health Insurance (JKN) participation and equalizing access to health services. This study uses a descriptive qualitative approach, relying on primary data obtained through in-depth interviews with relevant stakeholders, as well as secondary data in the form of regional policy documents, BPJS Kesehatan participant reports, and official archives of the Metro City Government. Data analysis was conducted using the interactive model of Miles, Huberman, and Saldana. The results of the study indicate that the success of UHC implementation in Metro City is influenced by several key factors, including strong political commitment from the regional head, budget support from the Regional Budget (APBD) for financing Regional Contribution Assistance Recipients (PBI), and integration and validation of participant data carried out across regional government agencies. Furthermore, public service innovations, such as a proactive approach and the use of digital health services, as well as solid coordination between the local government, BPJS Kesehatan, and health care facilities, also play a significant role in supporting the achievement of UHC. The findings of this study are expected to serve as a reference for other regions in accelerating the implementation of UHC in a sustainable manner.
