Abstract

Dementia is a neurodegenerative disorder characterized by a significant decline in cognitive function. Some studies suggest that structural changes in the brainstem can contribute to cognitive impairment. However, the relationship between the structural anatomical parameters of the brainstem and cognitive function impairment based on the Indonesian version of the Montreal Cognitive Assessment (MoCA) Score in patients with clinical dementia is still not widely studied. This study is an analytical observational study involving 65 patients with clinical dementia who underwent a head MRI examination at Ngoerah Hospital, Denpasar, Bali. The analyzed brainstem structural anatomical parameters included the diameter of the midbrain anteroposterior (AP), midbrain-to-pons ratio, superior cerebellar peduncle (SCP) width, middle cerebellar peduncle (MCP) width, and interpeduncular angle. The relationship between these parameters and cognitive function was measured using the MoCA-Ina score and analyzed by correlation and logistic regression tests. The average diameter of the AP midbrain was 11.1 cm (± 1.63 cm), the median midbrain-to-pons ratio was 0.19, and the median width of SCP and MCP was 1.8 cm and 7.9 cm, respectively. The results of the analysis showed that there was a sufficient positive relationship between the diameter of the AP midbrain and the midbrain-to-pons ratio (r = 0.329, p < 0.001) and between the width of SCP and MCP and the midbrain-to-pons ratio (r = 0.432 and 0.279, p < 0.001). The midbrain-to-pons ratio and MCP width were significantly related to the MoCA-Ina score (r = 0.265 and 0.447, p < 0.001).

Abstract

Spinal cord tumors are uncommon neoplasms that pose significant clinical challenges due to their potential to cause progressive neurological deficits. These tumors may originate from the spinal cord parenchyma, meninges, nerve roots, or surrounding vertebral structures, and are classified by location as intramedullary, intradural extramedullary, or extradural. Although they represent a small fraction of central nervous system tumors, their impact on motor, sensory, and autonomic function can be profound. Magnetic Resonance Imaging (MRI) is the modality of choice for the assessment of lesions within the spinal canal, providing detailed information on lesion location, morphology, and compressive effects. Accurate imaging is essential for diagnosis, surgical planning, and optimizing patient outcomes. This case series describes three patients presenting with spinal tumors and associated neurological symptoms. The first case involves an 18-year-old female with progressive right-hand hemiparesis. MRI revealed an intradural intramedullary lesion at the C5–C6 level with an intramedullary syrinx, suggestive of hemangioblastoma, confirmed histologically by vascular proliferation and atypical cells. The second case is a 42-year-old male with chronic neck pain and left arm weakness for seven months. MRI showed an intradural extramedullary mass at C2–C3 and central disc protrusion, diagnosed histopathologically as cellular schwannoma. The third case features a 49-year-old female with paraparesis, lower back pain, urinary incontinence, and a history of cervical adenocarcinoma. MRI revealed an extradural metastatic mass at T7–T9 levels, suggestive of metastatic mass. MRI is the best imaging modality for evaluating and diagnosing patients with suspected spinal tumors.

Abstract

Osteoporosis adalah penyakit yang ditandai dengan penurunan massa dan kualitas mikroarsitektur tulang, serta peningkatan risiko patah tulang, terutama pada wanita lanjut usia. Wanita lebih rentan terhadap osteoporosis karena faktor hormonal dan fisiologis. Penelitian ini bertujuan mengevaluasi hubungan ketebalan lemak subkutan dan visceral dengan kejadian osteoporosis pada wanita ≥50 tahun. Penelitian observasional dengan desain potong lintang ini melibatkan 39 wanita yang menjalani pemeriksaan DXA di RSUP Prof. Dr. I.G.N.G. Ngoerah pada Januari–Februari 2025. Ketebalan lemak diukur menggunakan ultrasonografi, dan diagnosis osteoporosis berdasarkan T-score DXA. Hasil analisis menunjukkan ketebalan lemak subkutan abdomen ≥23,5 mm (OR 19,0, p < 0,01) dan lemak visceral abdomen ≥10,2 mm (OR 76,0, p < 0,01) berhubungan dengan peningkatan risiko osteoporosis. Ketebalan lemak subkutan paha ≥10,5 mm (OR 4,5, p = 0,041) dan dorsal ≥14,5 mm (OR 10,6, p = 0,003) juga menunjukkan hubungan signifikan. Analisis regresi logistik menunjukkan ketebalan lemak subkutan abdomen ≥23,5 mm (aOR 5,1, p = 0,008) dan lemak visceral abdomen ≥10,2 mm (aOR 12,2, p = 0,002) sebagai determinan risiko osteoporosis yang signifikan. Penelitian ini menunjukkan pentingnya penilaian ketebalan lemak, khususnya lemak visceral, dalam mengidentifikasi risiko osteoporosis pada wanita lanjut usia.
