Abstract

Pregnancy is a physiological process accompanied by anatomical and hormonal changes that prepare women for childbirth. One of the most common complications during vaginal delivery is perineal rupture, which may cause postpartum pain, bleeding, infection, pelvic floor dysfunction, and reduced maternal quality of life. A non-pharmacological approach to prevent this condition is perineal massage using olive oil, which helps improve perineal tissue elasticity. This community service activity aimed to improve the knowledge and skills of second-trimester pregnant women through education and demonstration of perineal massage at Sarpi’ina Sinambela Clinic. The study used a pre-experimental design with a one-group pretest-posttest approach involving 50 pregnant women selected through total sampling. Data were collected using knowledge questionnaires and skill observation checklists before and after the intervention. The results showed an increase in the mean knowledge score from 62.4 to 85.7 with a mean difference of 23.3, and an improvement in skill level from 24% to 84% of participants being able to perform perineal massage correctly. Statistical analysis indicated a p-value of 0.000, showing a significant difference between pretest and posttest results. In conclusion, education and demonstration of perineal massage using olive oil were effective in improving pregnant women’s knowledge and skills in preventing perineal rupture. This program is expected to enhance maternal preparedness for childbirth and support safer and more comfortable delivery outcomes..

Abstract

Minor wounds are among the most common health problems experienced by communities in daily life. Inadequate first aid management may increase the risk of infection and delay the healing process. Community empowerment through health education and the utilization of local medicinal plants is a strategic approach to improving self-reliance in wound care. This community service program aimed to evaluate the effectiveness of education and training on the preparation of herbal gel from rambusa leaves (Passiflora foetida L.) as an alternative first-aid treatment for minor wounds. A quantitative pre-experimental study with a one-group pretest–posttest design was conducted among 100 participants in Sei Pematang Pasir Village, Medan Labuhan District, Indonesia. Participants received interactive lectures, demonstrations, hands-on practice, discussions, and educational leaflets. Knowledge was assessed using pretest and posttest questionnaires, while practical skills were evaluated through observation checklists. Data were analyzed using descriptive statistics and a paired sample t-test. The proportion of participants with good knowledge increased from 14.0% before the intervention to 85.0% afterward. The mean knowledge score improved significantly from 60.2 ± 9.8 to 88.7 ± 7.2, with a mean difference of 28.5 points (p < 0.001). Furthermore, 88.0% of participants demonstrated good skills in preparing the herbal gel, and participation remained high throughout all program activities. These findings indicate that education and practical training effectively enhanced community knowledge and skills in utilizing Passiflora foetida L. as a herbal gel for minor wound management, supporting community empowerment and the sustainable use of family medicinal plants (TOGA) in primary health care.

Abstract

Gastrointestinal disorders remain one of the most common health problems in the community, while the utilization of Family Medicinal Plants (TOGA), particularly Justicia gendarussa Burm.f. (gandarusa) leaves, as a promotive and preventive health approach is still limited due to inadequate public knowledge. This community service program aimed to improve community knowledge regarding the utilization of gandarusa leaves to support gastrointestinal health through TOGA-based health education. A pre-experimental study with a one-group pretest–posttest design was employed. The program was conducted in Teluk Nibung Village, Tanjungbalai City, North Sumatra, involving 100 participants selected using purposive sampling. The educational intervention consisted of interactive lectures, group discussions, demonstrations on the preparation of gandarusa herbal remedies, and the distribution of educational leaflets. Data were collected using knowledge questionnaires administered before and after the intervention and analyzed descriptively using the paired sample t-test. The findings demonstrated a substantial improvement in participants' knowledge, with the proportion of respondents categorized as having good knowledge increasing from 12.0% to 82.0%, while the poor knowledge category decreased from 32.0% to 0.0%. The mean knowledge score significantly increased from 58.6 ± 10.4 before the intervention to 86.8 ± 8.1 after the intervention, with a mean difference of 28.2 points (p < 0.001). Participant engagement was also exceptionally high, as all participants completed the registration, pretest, educational sessions, and posttest. TOGA-based health education proved effective in enhancing community knowledge regarding the utilization of Justicia gendarussa Burm.f. leaves to support gastrointestinal health. This program is recommended for sustainable implementation to strengthen community empowerment and promote the evidence-based use of local medicinal plants.

Abstract

Clean and Healthy Living Behavior (CHLB) is a promotive and preventive approach to improving public health, one of which is through proper Handwashing with Soap (HWWS). However, the implementation of correct HWWS practices remains suboptimal among residents of Sei Pematang Pasir Village. This community service program aimed to enhance community knowledge and practical skills regarding CHLB through education and HWWS training. The methods included observation, interviews, interactive health education, demonstrations, hands-on practice, and evaluation using pre-tests, post-tests, and observation sheets. The results demonstrated significant improvements in participants’ knowledge of CHLB, the benefits of HWWS, critical times for handwashing, and the six-step handwashing technique. Participants also showed better practical skills in performing HWWS according to World Health Organization (WHO) recommendations, particularly in using soap, cleaning between the fingers, thumbs, fingertips, and rinsing hands properly. These findings indicate that combining health education with demonstrations and practical training effectively improves community knowledge, skills, and CHLB practices, thereby contributing to the sustainable prevention of infectious diseases and promoting better public health outcomes.
