Abstract

Gaps in the achievement of drug-resistant tuberculosis case detection in Cilegon City still occur. The target of 85% case detection for RO-TB has been achieved at only 31%, with an enrollment rate of 30%. Evaluation of program implementation is needed to identify obstacles and opportunities for improving outcomes. This study aimed to analyze and evaluate the implementation of the RO-TB case-finding program. This qualitative study, using a phenomenological approach, was conducted in nine community health centers (Puskesmas) in Cilegon City, Banten. Evaluation indicators include acceptability, appropriateness, fidelity, sustainability, and implementation cost. There were 25 informants in this study, including key informants, main informants and supporting informants.Data will be collected through in-depth interviews. A questionnaire will be used to gather qualitative information. The informants in this study included heads of community health centers, health department, community health workers, pulmonary specialists, business representatives, and the media. Data analysis used the Miles and Huberman principles and triangulation. The TB RO case detection program was well-received by all Community Health Centers (Puskesmas), but trained human resources were not evenly distributed, facilities and infrastructure were inadequate, targeting needed to be reviewed, and cross-sectoral collaboration was not well-established. The SWOT analysis identified strong potential support from academics to send expert teams, industry willing to channel CSR funds and develop policies supporting the program, and the mass media to promote the TB RO case detection program. To enhance the effectiveness of the Drug-Resistant Tuberculosis case finding program, providing training for health workers and regional analysis for determining relevant targets needs to be done regularly.

Abstract

Non-communicable diseases (NCDs) such as hypertension, diabetes, and cardiovascular disease are major health problems in Indonesia, particularly among adults and the elderly. Health screening is essential for early detection, yet its implementation in primary health centers faces various challenges. This study aims to analyze barriers, supporting factors, and strategies to optimize health screening. This qualitative case study was conducted in five Puskesmas in Cilegon City, selected through purposive sampling. A total of 42 informants were involved, including Puskesmas heads, program managers, health workers, cadres, community members, and health office representatives. Data were collected using in-depth interviews, focus group discussions (FGDs), participatory observations, and document reviews. Data were analyzed thematically using Braun and Clarke’s framework, guided by the theories of Policy Implementation and the Theory of Planned Behavior (TPB). The main barriers included limited human resources, inadequate facilities, fragmented reporting systems, and low community awareness. Supporting factors were leadership commitment, active cadres, outreach innovations, integration with other services, and cross-sectoral collaboration. Optimizing health screening requires strengthening reporting systems, improving human resource capacity, cross-sectoral advocacy, and community-based approaches. Policy support, application integration, training, and local-level.

Abstract

Aplikasi Sehat IndonesiaKu (ASIK) is a digital-based healthcare service recording and reporting application aimed at replacing manual recording and simplifying the reporting process for various health programs. Field experience shows that there is still a gap between the reporting results in the application and manual recording, resulting in serious consequences for the validity of health data. This study aims to evaluate the implementation of the digitalization policy for reporting non-communicable disease screening through the ASIK application in North Lampung Regency in 2025. The evaluation was conducted by examining four main factors, namely the technical aspects of the application, human resources, organizational support, and health accessibility. This research uses a qualitative approach with a case study design using triangulation methods in the form of in-depth interviews, observation, and documentation. Data were collected through in-depth interviews, document review, and field observations at primary health facilities, and analyzed thematically using triangulation of sources and methods. Barriers to ASIK reporting are a multidimensional problem that causes a gap between policy design and field readiness. The research results show four main factors causing low reporting, namely technical application factors, limited human resources, weak organizational support, and regional accessibility constraints. This study recommends integration and strengthening of the ASIK system, improved infrastructure support and reporting management in the regions, and further quantitative studies to assess its impact on the quality of NCD screening data.
