Abstract

Abstrak Early Warning Score (EWS) merupakan ukuran yang dipakai sebagai dasar mengevaluasi deteriorasi kondisi fisiologis pasien, yaitu untuk mengidentifikasi dan merespons pasien dengan penyakit akut. Tujuan dari penelitian ini adalah untuk memberikan gambaran tentang implementasi EWS dalam mendeteksi deteriorasi pasien di rumah sakit melalui tinjauan literatur sistematis yang berkaitan dengan paradigma dan filosofi keperawatan. Metode yang diterapkan adalah tinjauan literatur dengan pencarian artikel dari database PubMed, Google Scholar, dan CINAHL dalam rentang waktu 2020-2025. Terdapat 8 (delapan) artikel yang sesuai dengan kriteria inklusi dan dianalisis berdasarkan desain penelitian, populasi, variabel, alat, dan hasil penelitian. Hasil analisis menunjukkan bahwa EWS terbukti efektif secara klinis sebagai prediktor luaran pasien, termasuk penurunan Lama Tinggal (Length of Stay/LOS), penurunan tingkat kematian (mortalitas), dan prediksi admisi ICU. Selain itu, penerapan sistem elektronik (digitalisasi) dalam penilaian EWS terbukti dapat meningkatkan efisiensi perawatan dan memungkinkan intervensi lebih cepat. Evaluasi ini sangat bergantung pada kepatuhan dan kemampuan berpikir kritis tenaga kesehatan yang memanfaatkannya. Kata Kunci: EWS, deteksi dini, filsafat keperawatan, paradigma keperawatan. Abstract The Early Warning Score (EWS) is a metric utilized to evaluate the decline in a patient’s physiological state, specifically for recognizing and addressing patients presenting with acute illnesses. The objective of this study is to present an overview of how EWS is implemented to identify patient deterioration in hospitals via a systematic literature review related to nursing paradigms and philosophy. The method applied involves conducting a literature review by exploring articles from the PubMed, Google Scholar, and CINAHL databases from 2020 to 2025. Eight articles that satisfied the inclusion criteria were examined in terms of study design, population, variables, instruments, and research outcomes. The findings indicate that EWS has been clinically proven as an effective predictor of patient outcomes, including reducing Length of Stay (LOS), mortality rates, and predicting ICU admissions. Furthermore, the adoption of electronic systems in EWS evaluations can significantly enhance care efficiency and allow for quicker interventions. Effective EWS assessment heavily relies on the adherence, critical thinking, and analytical skills of the healthcare workers utilizing it.
