Ulkus kornea bakteri (keratitis bakteri) merupakan kegawatdaruratan oftalmologi dan salah satu penyebab utama kebutaan kornea di negara berkembang, termasuk Indonesia. Infeksi ini jarang terjadi pada epitel yang utuh dan umumnya dipicu oleh faktor predisposisi seperti trauma okuler, penggunaan lensa kontak, maupun penyakit sistemik. Kondisi ini ditandai dengan kerusakan barier epitel dan infiltrasi supuratif pada stroma, yang dapat memicu pencairan jaringan secara cepat dan berisiko tinggi menyebabkan kebutaan monokuler akibat pembentukan jaringan parut (sikatriks) permanen pada aksis visual. Laporan kasus ini bertujuan untuk mengkaji pendekatan diagnostik, patofisiologi, serta terapi medikamentosa pada pasien ulkus kornea bakteri. Seorang laki-laki berusia 46 tahun datang dengan keluhan mata kiri perih dan berair sejak tiga hari sebelum masuk rumah sakit. Perjalanan penyakit diawali riwayat trauma mekanik minor berupa sensasi "kelilipan" sepuluh hari sebelumnya. Pemeriksaan fisik segmen anterior menunjukkan adanya defek epitel disertai infiltrat stroma supuratif berwarna putih kekuningan, dengan penurunan tajam penglihatan yang signifikan menjadi 6/120. Dari berbagai faktor risiko epidemiologis yang ada, trauma minor pada pasien ini bertindak sebagai port de entry utama yang merusak barier anatomis epitel kornea. Secara molekuler, invasi bakteri memicu pelepasan enzim proteolitik bakteri dan aktivasi Matrix Metalloproteinases (MMPs) dari sel imun pejamu (host) yang memicu kaskade stromal melting. Onset yang akut dan karakteristik lesi purulen berbatas tegas secara klinis menyingkirkan diagnosis banding keratitis jamur maupun etiologi non-bakteri lainnya. Pasien menerima tata laksana empiris agresif berupa kombinasi antibiotik topikal spektrum luas Levofloxacin (fluorokuinolon generasi ketiga) dan Gentamicin untuk mencapai cakupan ganda (dual coverage) yang adekuat, disertai antibiotik sistemik Ciprofloxacin dan air mata buatan suportif. Penggunaan kortikosteroid dihindari pada fase akut untuk mencegah komplikasi perforasi kornea. Diagnosis dini dan inisiasi terapi antibiotik spektrum luas kombinasi sangat krusial untuk menghentikan progresivitas infeksi. Meskipun sterilisasi bakteri berhasil dicapai dengan prognosis quo ad vitam dan sanam yang bonam, prognosis penglihatan (quo ad functionam) tetap ad malam karena resolusi stroma pada aksis visual akan digantikan oleh jaringan fibrosis permanen. ABSTRACT Bacterial corneal ulcer (bacterial keratitis) is an ophthalmologic emergency and a leading cause of corneal blindness in developing countries, including Indonesia. The infection rarely occurs on an intact epithelium and is generally triggered by predisposing factors such as ocular trauma, contact lens wear, or systemic diseases. It is characterized by epithelial barrier disruption and suppurative stromal infiltration, which can trigger rapid tissue liquefaction and carries a high risk of monocular blindness due to permanent scar (cicatrix) formation on the visual axis. This case report aims to review the diagnostic approach, pathophysiology, and rationalization of medical therapy in a patient with a bacterial corneal ulcer. A 46-year-old male presented with a painful and watery left eye for three days prior to admission. The disease progression was initiated by a history of minor mechanical trauma due to a foreign body sensation ("kelilipan") ten days earlier. Anterior segment examination revealed an epithelial defect accompanied by a yellowish-white suppurative stromal infiltrate, with a significant decrease in visual acuity to 6/120. Among various epidemiological risk factors, minor trauma in this patient acted as the primary port de entry that breached the anatomical barrier of the corneal epithelium. Molecularly, bacterial invasion triggers the release of bacterial proteolytic enzymes and the activation of host Matrix Metalloproteinases (MMPs), leading to a stromal melting cascade. The acute onset and well-defined purulent lesion characteristics clinically ruled out the differential diagnosis of fungal keratitis and other non-bacterial etiologies. The patient received aggressive empirical therapy consisting of a combination of broad-spectrum topical antibiotics, Levofloxacin (third-generation fluoroquinolone) and Gentamicin, to achieve adequate dual coverage, alongside systemic Ciprofloxacin and supportive artificial tears. Corticosteroid use was avoided in the acute phase to prevent the complication of corneal perforation. Early diagnosis and initiation of combined broad-spectrum antibiotic therapy are crucial to halt the infection's progress. Although bacterial sterilization was successfully achieved with a bonam prognosis for quo ad vitam and sanam, the visual prognosis (quo ad functionam) remains ad malam because stromal resolution on the visual axis will inevitably be replaced by permanent fibrotic tissue.
