Abstract

Sectio Caesarea (SC) surgery causes severe acute pain due to abdominal and uterine wall trauma, which can delay mobilization and disrupt the lactation process. This study aimed to analyze nursing care and evaluate the effectiveness of simple inhalation therapy combining warm water and cajuput oil with lavender aroma in reducing pain intensity among post-SC patients. The research employed a descriptive qualitative case study design with a nursing care approach. The subjects consisted of 5 post-SC patients experiencing acute pain at Ruang Nifas. Data collection was conducted through interviews, direct observations using the Numeric Rating Scale (NRS) before and after the intervention, and nursing documentation over 3 days of care. The initial assessment revealed that all 5 respondents experienced moderate-to-severe acute pain, with pre-test scores ranging from 5 to 7. Following 10–15 minutes of daily inhalation therapy for 3 consecutive days, a consistent reduction in pain intensity was observed across all respondents, with post-test scores on the third day dropping to a range of 2 to 3 (mild pain). The patients also demonstrated more relaxed facial expressions and improved mobility comfort. In conclusion, the application of simple lavender inhalation therapy effectively reduces acute pain intensity in post-SC patients, making it a highly recommended independent nursing intervention in postpartum wards.

Abstract

Hyperbilirubinemia is a common condition in newborns characterized by yellow discoloration of the skin and sclera due to elevated serum bilirubin levels. If untreated, severe hyperbilirubinemia can lead to kernicterus and permanent brain damage. Phototherapy as the main treatment has limitations such as dehydration and skin rashes, necessitating complementary therapies such as infant massage. This study aimed to identify the application of infant massage in reducing bilirubin levels in an infant with hyperbilirubinemia. A descriptive case study method was applied to 1 male infant aged 2 days with total serum bilirubin of 9.87 mg/dL undergoing phototherapy at Dahlia Room, Kardinah Hospital, Tegal. Infant massage was performed for 10-15 minutes once daily for 4 consecutive days using pure coconut oil with moderate pressure. Bilirubin levels were measured daily using a Transcutaneous Bilirubinometer JM-105. Results showed a progressive decrease in bilirubin levels from 9.87 mg/dL to 5.20 mg/dL (total reduction of 4.67 mg/dL). Bowel movement frequency increased from 2 to 4-5 times/day, sleep quality improved from restless to restful, body temperature remained stable within normal range, and infant comfort improved. The combination of infant massage and phototherapy demonstrated synergistic effects in accelerating bilirubin excretion through enhanced intestinal peristalsis and disrupting enterohepatic circulation. Infant massage is effective, safe, and low-cost as a complementary therapy for hyperbilirubinemia and can be performed by trained healthcare providers or parents after proper education. This study provides daily clinical descriptions of changes in bilirubin levels, defecation frequency, sleep quality, and infant comfort during the implementation of baby field massage combined with phototherapy.

Abstract

Hemorrhagic stroke is a condition of bleeding in the brain that can cause neurological disorders as well as psychological problems, one of which is anxiety. Unresolved anxiety can affect physiological conditions and hinder the patient's recovery process. Murottal therapy is a spiritual-based non-pharmacological intervention that can be used to help reduce anxiety levels. This study aimed to describe the implementation of murottal therapy in reducing anxiety among patients with hemorrhagic stroke in a stroke unit.This study employed a descriptive method with a case study approach involving a hemorrhagic stroke patient experiencing anxiety. Data were collected through nursing assessment, observation, the Zung Self-Rating Anxiety Scale (ZSAS), and the implementation of Surah Ar-Rahman murottal therapy. The therapy was administered for 10–15 minutes daily for three consecutive days.The results showed a 24.1% reduction in anxiety levels following the administration of murottal therapy. This was indicated by a decrease in the ZSAS score from 58 to 44 after three days of intervention. The patient appeared calmer, more relaxed, more comfortable, and was able to rest better. Murottal therapy can be used as a complementary nursing intervention to help reduce anxiety and support the recovery process in patients with hemorrhagic stroke. These findings suggest that Surah Ar-Rahman murottal therapy has the potential to become a simple, safe, cost-effective, and easily applicable spiritual nursing intervention as a complementary therapy for patients with hemorrhagic stroke.
