Performance assessment is an important factor in producing high-quality exemplary health workers. Objective and accurate assessment is essential to select the best candidate. Based on data collected through interviews at the Kosambi Public Health Center, the data recapitulation process for 35 employees is still done manually using simple tools. The absence of a specific system makes the assessment susceptible to subjectivity bias and time-consuming, resulting in suboptimal decisions. To solve these problems, a decision support system was developed to determine the exemplary health worker using the Multi-Objective Optimization On The Basis Of Ratio Analysis (MOORA) method based on five criteria: discipline, responsibility, ethics, SKP, and length of service. This system is implemented in a web-based application using the PHP programming language and MySQL database. The results of this study facilitate the agency in determining the exemplary health worker quickly and accurately. This is evidenced by the User Acceptance Testing (UAT) results using a questionnaire with a feasibility percentage of 82.74%. Based on the final calculation using the MOORA method on 35 employees, three best exemplary health workers were recommended, namely dr. Anis Julianti in the first rank with a final score of 0.21852, followed by drg. Fitriyani Z in the second rank with a score of 0.21007, and dr. Viedya Wildan in the third rank with a score of 0.20976.
