Abstract

Type 2 diabetes mellitus (T2DM) is a chronic metabolic disorder characterized by hyperglycemia caused by insulin resistance and impaired insulin secretion. This study aimed to analyze the effectiveness of combination therapy with metformin and Sodium-Glucose Cotransporter-2 (SGLT2) inhibitors on glycemic control compared with metformin monotherapy. An analytical observational cross-sectional design with a retrospective approach was applied to 120 T2DM patients treated at the Endocrinology Clinic of Mataram General Hospital from January to December 2024. Medical record data included HbA1c, fasting plasma glucose, body mass index (BMI), and lipid profile. Data were analyzed using an independent t-test and multivariate linear regression. The results showed a mean HbA1c reduction of 1.42 ± 0.38% in the combination group, greater than the 0.87 ± 0.31% reduction in the monotherapy group (p < 0.05). The combination group also showed reductions in fasting plasma glucose and body weight without an increased risk of hypoglycemia. Multivariate analysis identified combination therapy as an independent predictor of improved glycemic control (β = 0.62; p = 0.001). Thus, metformin and SGLT2 inhibitor combination therapy was more effective than metformin monotherapy in improving glycemic control in patients with T2DM.
