Abstract

Introduction : Arteriovenous fistula (AVF) is the preferred vascular access for patients undergoing hemodialysis because of its lower complication rates and better long-term patency. However, inadequate self-care behaviors remain a major cause of AVF complications, including thrombosis, infection, and vascular access failure. Animated video-based education has emerged as a promising educational strategy to improve patient understanding and self-care behavior. Methods : A quasi-experimental study with a pretest–posttest control group design was conducted among 80 hemodialysis patients in a tertiary hospital in Bandung, Indonesia, from November 2025 to January 2026. Participants were assigned into an intervention group (n = 40) receiving animated video-based education in addition to routine nursing education and a control group (n = 40) receiving routine education only. Data were collected using the Scale of Assessment of Self-Care Behaviors with Arteriovenous Fistula in Hemodialysis (ASBHD-AVF). Data were analyzed using paired t-tests, repeated-measures ANOVA, and multiple linear regression. Results : The intervention group demonstrated a significant improvement in AVF self-care behavior scores from 45.68 ± 6.42 at baseline to 67.45 ± 5.73 after the intervention (p < .001), whereas the control group showed only minimal improvement. Repeated-measures ANOVA revealed a significant interaction effect between group and time (F = 86.47, p < .001) with a large effect size (partial η² = .526). Multiple linear regression analysis identified participation in animated video education as the strongest predictor of post-intervention AVF self-care behavior (β = .641, p < .001). Participants also reported positive perceptions regarding the clarity and usefulness of the animated videos. Conclusion : Education through animated videos significantly improved AVF access self-care behaviors among patients undergoing hemodialysis and may serve as an innovative nursing educational strategy to enhance patient self-management.
