Abstract

Background: Primary dysmenorrhea is common among adolescents and may interfere with learning activities. Among adolescent, dysmenorrhea can significantly disrupt daily activities and learning, thereby reducing quality of life and productivity. This report describes midwifery care for an adolescent with menstrual pain and a low haemoglobin concentration. Case presentation: A 15-year-old student reported lower abdominal and back pain on the first day of menstruation that interfered with activities, accompanied by restlessness. Her menstrual cycle was 29 days and lasted 6–7 days. Vital signs were stable, and the initial haemoglobin concentration was recorded as 10.5 g/dL. Care and outcomes: Nine follow-up visits were conducted from April to June 2025 using Varney’s seven-step framework and SOAP documentation. Care included menstrual education, heat application, relaxation exercises, dietary and rest counselling, family support, and spiritual practices aligned with the patient’s preference. During the subsequent cycle, pain was reported as mild, limited to the first day, and no longer interfered with activities. The last recorded haemoglobin concentration was 12.5 g/dL. Conclusion: Symptoms improved during follow-up; however, causality cannot be established because this is a single case, pain intensity was not measured using a validated scale, and multiple care components were delivered concurrently.
