Specific Background This literature review investigates the diagnostic role of the Reticulocyte Hemoglobin Equivalent in identifying iron deficiency anemia, particularly among patients suffering from inflammatory and chronic conditions. Knowledge Gap Although conventional parameters exist, traditional markers frequently face distortions under chronic inflammation, prompting the need to evaluate reliable and early diagnostic alternatives like Ret-He. Aims This study aims to evaluate the diagnostic performance, sensitivity, specificity, and cutoff values of the parameter for screening and diagnosing iron deficiency anemia in chronic disease settings. Results Findings from selected literature demonstrate that Ret-He provides an effective diagnostic tool with high sensitivity and specificity across cutoff values ranging from 21 to 33 pg, detecting iron deficits and therapeutic responses earlier than conventional hemoglobin tests. Novelty This work consolidates multi-population evidence showing that the parameter remains robust against chronic inflammatory processes that typically skew traditional ferritin measurements. Implications These insights confirm that utilizing this hematological parameter serves as a practical, rapid, and efficient screening strategy for assessing iron status in clinical practice. Highlights: The parameter effectively detects iron deficiency and iron deficiency anemia earlier than conventional erythrocyte indices due to rapid reticulocyte turnover. Values remain reliable and unhindered by inflammatory processes, successfully identifying functional iron availability in complex chronic disease populations. The marker accurately monitors early therapeutic responses to iron treatments before standard hemoglobin levels display measurable changes.
